      • 모반세포성 모반과 악성 흑색종의 CD44 발현에 관한 면역조직화학적 연구

        CD44 is the major human cell surface receptor for hyaluronate and mediate cellmatrix and cell-cell interactions involved in the mechanisms of tumor invasion and metastasis. Typical acquired melanocytic nevi appear to follow a life cycle characterized by active growth, maturation, and progressive involution, In contrast, congenital melanocytic nevi have a different course and prognosis with potential for development of melanoma, and metastatic spread is very common in malignant melanoma. We investigated the expression of CD44 in acquired and congenital melanocytic nevi, and malignant melanoma by immunohistochemical technique. Formalin-fixed and paraffin-embeded tissue from 5 compound nevi, 6 intradermal nevi, 5 congenital melanocytic nevi, and 5 malignant melanoma were immunolabeled with monoclonal antibodies directed against CD44. All acquired melanocytic nevi showed negative or weak staining for CD44. However, all congenital melanocytic nevi exhibited positive staining for CD44 with minimal irregularities according to location. In malignant melanoma, there was variable intensity of staining for CD44 in different areas of the tumor. These findings suggest that the different expression of CD44 in melanocytic nevi may contribute to different biological behavior between acquired and congenital meanocytic nevi. and CD44 seems to be related to metastasis and tumorigenesis of the malignant melanoma.

        In this case study, education costs are computed for each academic unit, i. e., undergraduate major or graduate major, of Hankyung National University using the expenditure data for the 2000 academic year. Education cost is defined as unit cost incurred in educating a student in a three-credit semester course for a given undergraduate major or graduate major. Education costs are computed in three steps. First, university administration and staff departments, undergraduate colleges, and graduate colleges are identified as cost centers in a stylistic manner. Second, costs are accumulated to each cost center. Finally, accumulated costs in the cost centers are either directly assigned to or indirectly allocated on the basis of estimated cost drivers to pertinent undergraduate majors or graduate majors. The results of this study would help evaluate the current status of education quality and/or education efficiency comparatively among undergraduate majors and graduate majors. This would hopefully lead to better resource allocations among the academic units of the university in the future. A caveat is that the education costs computed in this study are the actual costs based on actual expenditure data, not the future target costs to be incurred when the target level of education quality and efficiency is achieved. Thus it is not recommend to use the results of this study directly differentiating tuitions among academic units of the university.

        The increasing stress on the need for standardized rehabilitation programs for the crippled has faced the workers in the field with the problems of finding out an ideal model of rehabilitation programs which can most facilitate maximum development of the children in various educational aspects. As part of the basic research, the study has been conducted to analyze the data on the present status of the six schools for the crippled in the country, together with other related survey on the available literature. Most of the data on the present situation of the schools and the status of the workers were obtained through questionnaires. The findings derived from the analysis of those data are as follows: 1. The Present Status of the Rehabilitation Training The survey revealed only forty-nine percent(381) of the entire subjects of 784students had been put to the training, that classroom lesson comprised 50% and 43% of the curricula in primary and secondary courses respectively, and that in the rehabilitation programs, physical therapy accounted for 50%, occupational therapy 31% and speech therapy 19%. As for the qualification of rehabilitation workers, twenty of the twenty-five who are presently serving at the six schools are license holders, averaging three physical therapists, 0ne occupational therapist and 0.2 speech therapist at each school. Ninety-six percent are junior college graduates and fifty-six percent are aged 20∼25,forty percent 26∼30. Eighty percent fall into the monthly pay range of 140,000 to 210,000 won, leaving the others below or above the scale. The study also showed that the cause or motivation of their becoming the workers was out of a sense of duty; seventy-six percent replied that they volunteered to help the crippled. Principals, the workers and special teachers showed in that order their enthusiasm about their enthusiasm about the rehabilitation prgrams. It was also found in the study that the authorities concerned have held a special guidance session once a year, achieving their annual goal only 50% and that most of the schools (82%) organized their own rehabilitation programs. The tools and other materials used in the programs were mostly(67%) domestic products. Four schools (67%) prepared thir own evaluation devices for the achievements of the trainees. 2. Facilities and Equipment for Rehabitition Training In all, thirty-four kinds of equipment and tools are currently provided at the six schools to help the chidren maintain or develop their residual function in the deficient limb. Of them only eighteen kinds were owned by each school, aberaging more than one; they were mats, walkers, parallel bars, mirrors, puzzle patches, stairs, shoulder wheels, tilt tables, walking aids, safety aids safety walking belts restorators, over head pulleys, knee exercise units, rotary wrist macihnes, holdings and gloves. The rest of the tools or apparatus averaged less than one among the schools. Nine kinds of electro therapeutic apparatus were owned by them, but only electrostimulation test machined lamps aberaged more than one. As for the hydro therapeutic apparatus, eight kinds were provided among the schools. Each school had more than one set of these three devices: hydor collator, whirl pool and hydro collator pack, with hydro collator pack averaging 17.33. They maintained thirteen kinds of occupational therapy appraratus, of which only three were owned by every school. Most of the apparatus were meant to deal with metallic works. Eleven sorts of speech therapy devices were found; only breathing exercise apparatus averaged more than one. The rehabilitation facilities and equipment at the schools are far below sufficient both in number and kind to meet the need. 3. Teacher' Attitude and Morale Toward the Rehabilitation Programs The schools for the crippled generally try to put equal emphasis on every field needed to rehabiletate the children but it was found that they were, intentionally or unintentionally, putting varying degrees of stresses on indivdual parts. The findings allowed making a list arranged in order of their priority: physical therapy, occupational therapy, speech therapy, motor development training and play therapy. The teachers responded that facilities, equipments, materials, parental cooperation, administravtive support and the ability of the rehabilitation workers were not sufficient enough to meet expectations. When asked to list the priority of the urgent fields, approximately percent put physical therapy at the top, twenty-one percent human relationships. The study showed teachers thought that the sense of duty (35%), ability and qualifications(21%), training skill(15%), academic career(13%) should be considered as the objective determinants in selecting a teacher, and 34% opined that a worker should have “ardent desire and sense of service”. Others expressed their opinion that understanding children's psychology(20%), having good knowledge of basic theory of the rehabilitation(19%) should be prerequisite to the service. Seventy-five percent of the teachers responded that rehabilitation training should be greatly improved and that they positively cooperated with the workers in the programs. It was inferred that the school administrators were generally affirmative towards the programs. The work-load of the rehabilitation workers was regarded as almost the same as that of special teachers(37%). The public and the education authorities assessed the ability of the workers "fair"(52%). Seventy-six percent of those who replied opined the financial support”, was "not sufficient", and seventy-one percent insisted the Ministry of Education should ear-mark special fund for the rehabilitation programs. As part of promotion measures, the majority of the replied(52%) reommended that authorization of more courses and expansion of the existing courses for the workers should be made, and the great majority(71%) advocated raising the special allowances, which was found to be the first in the order of priority, followed by the government's administrative and legislative support(26%) and development and development and supply of materials and equipment for the rehabilitation(22%). The analysis of the above data helped deduct a reasonable rehabilitation practice pattern in which the rehabilitation programs are adopted with the same weight as other parts of the curriculum such as classroom lessons or extra curricula activities, and the programs should cover, with an equal emphasis on every field physical therapy(functional training), occupational therapy (vocational training), speech therapy, sensory training(perception-motor training) and psychological training. Shown in IV-B are the tentatively listed basic sets of facilities and apparat believed indespensable in the rehabilitation programs. 4. Suggestions The following are the suggestion based on the results of the comprehensive study on the present rehabilitation practice in the country. (1) The rehabilitation programs which is to facilitate maximum motor-skill development should compulsorily be included in the curriculum of the school for the crippled. (2) The present training course of the rehabilitation workers should be reorganized to produce highly skilled rehabilition experts who will satisfactorily meet the need on the part of the children. Also desirable is the change of the present title of the workers "rehabilitation attendant" to "rehabilitation training teacher". In parallel with the intensification on training and requirement of the teachers, an increase in emolument to or level of the other special teachers should be made. (3) It is recommended that the programs be extended to other fields of special education, in view of the fact that the rehabilitation programs are basically meant to maintain, improve or eliminate any human defects.

        A successful treatment of infants and children with congenital heart defects depends upon several factors: diagnosis must be accurate, the operative technique, including techniques of perfusion and myocardial preservation, must be suited to the needs of patients and intensive cares, including accurate anesthesia, must be applied through all stages of investigation and treatment, i.e. before, during and after the operation. Recently we performed a open heart surgery successfully for a six-years old girl who had suffered from frequent upper respiratory tract infection and mild to moderate exertional dyspnea with congenital heart defects (ventricular septal defect, Kirklin type II, with patent foramen ovale). The operation and postoperative course were very smooth. And now we report this with review of literatures.

        Intranasal delivery of Duox2 DNA using cationic polymer can prevent acute influenza A viral infection in vivo lung

        <P>We studied the contribution of Duox2 in mucosal host defense against influenza A virus (IAV) infection in in vivo lung. We found that Duox2 was required for the induction of type I and III interferon (IFN)s and transient Duox2 overexpression using cationic polymer polyethyleneimine (PEI) leads to suppression of IAV infection in in vivo lung. Twenty mice (C57BL/6J) were anesthetized and challenged by intranasal administration of 213 pfu/30 mu l of IAV (WS/33/H1N1), and IAV-infected mice were euthanized at 1, 3, 5, 7, 10, 14 days post infection (dpi). Duox2 small hairpin RNA (shRNA) and pCMV-Duox2 formulated with PEI were inoculated to mice to assess the regulatory mechanism between Duox2 and IFN secretion. Following intranasal IAV inoculation, viral infection was significantly aggravated from 3 dpi in in vivo lung and viral titer was highest at 7 dpi. Consistent with this, Duox2 messenger RNA (mRNA) and protein expressions were significantly induced from 3 dpi in the lung tissue of IAV-infected mice. Viral titer was much higher in IAV-infected mice that were inoculated with Duox2 shRNA accompanied with lower survival rate and extensive lung pathologies. Interestingly, severe lung pathologies in IAV-infected mice were not observed and viral titer was significantly reduced in mice with pulmonary administration of pCMV-Duox2 formulated with PEI before IAV inoculation. Both mRNA and secreted protein levels of IFN-beta and IFN-lambda(2/3) were highly elevated in IAV-infected mice with pCMV-Duox2 formulated with PEI. Duox2 is necessary for the regulation of IFN secretion in in vivo lung, and pulmonary administration of Duox2 DNA using cationic polymer triggers the induction of type I and III IFNs resulting in more complete suppression of IAV infection.</P>

      • Cisplatin 병용화학요법으로 인한 오심, 구토의 예방을 위한 Granisetron(Kytril^ⓡ)과 MDL(Meroclopramide, Dexamethasone, Lorazepam) 병용요법의 비교

        Granisetron is known as the most selective 5-HT3 receptor antagonist and is reported to be effective in the prevention of acute emesis induced by cisplatin based chemotherapy. From June 1885 to December 1995, 32 solid tumor patients who were scheduled to receive cisplatin based combination chemotherapy were randomized either to granisetron or MDL(Metoclopramide, Dexamethasone, Lorazepam) prophylaxis. We evaluated 32 patients; 17 patients received granisetron 3mg, while 15 patients received MDL. The male to female ration was 12:5(Granisetron group) versus 12:3(MDL group), and moan age was 44.6 yrs(Granisetron group) versus 46.5 yrs(MDL group). Of 17 granisetron treated patients, 76.5%(13/17) obtained complete or major response in prevention of acute emesis, while 73.4%(11/15) of patients receiving MDL on first 24 hours of chemotherapy(P>0.05). No severe side effects were recorded in both groups. In granisetron group, mild headache and constipation were noted. In MDL group, the most common side effect was sedation(4/15, 26.7%), but extrapyramidal reaction was not noted in our patients. These results show that granisetron is an effective antiemetic in the control of cisplatin induced nausea and vomiting, and can be administered safely with minimal side effects.

        Fluorine Functionalized Graphene Nano Platelets for Highly Stable Inverted Perovskite Solar Cells

        <P>Edged-selectively fluorine (F) functionalized graphene nanoplatelets (EFGnPs-F) with a p-i-n structure of perovskite solar cells achieved 82% stability relative to initial performance over 30 days of air exposure without encapsulation. The enhanced stability stems from F substitution on EFGnPs; fluorocarbons such as polytetrafluoroethylene are well-known for their superhydrophobic properties and being impervious to chemical degradation. These hydrophobic moieties tightly protect perovskite layers from air degradation. To directly compare the effect of similar hydrophilic graphene layers, edge-selectively hydrogen functionalized graphene nanoplatelet (EFGnPs-H) treated devices were tested under the same conditions. Like the pristine MAPbI(3) perovskite devices, EFGnPs-H treated devices were completely degraded after 10 days. The hydrophobic properties of EFGnPs-F were characterized by contact angle measurement. The test results showed great water repellency compared to pristine perovskite films or EFGnPs-H coated films. This resulted in highly air-stable p-i-n perovskite solar cells.</P>

      • Morphology-Dependent Hole Transfer under Negligible HOMO Difference in Non-Fullerene Acceptor-Based Ternary Polymer Solar Cells

        <P>In the field of organic solar cells, it has been generally accepted until recently that a difference in band energies of at least 0.3 eV between the highest occupied molecular orbital (HOMO) level of the donor and the HOMO of the acceptor is required to provide adequate driving force for efficient photoinduced hole transfer due to the large binding energy of excitons in organic materials. In this work, we investigate polymeric donor:non-fullerene acceptor junctions in binary and ternary blend polymer solar cells, which exhibit efficient photoinduced hole transfer despite negligible HOMO offset and demonstrate that hole transfer in this system is dependent on morphology. The morphology of the organic blend was gradually tuned by controlling the amount of ITIC and PC<SUB>70</SUB>BM. High external quantum efficiency was achieved at long wavelengths, despite ITIC-to-PC<SUB>70</SUB>BM ratio of 1:9, which indicates efficient photoinduced hole transfer from ITIC to the donor despite an undesirable HOMO energy offset. Transient absorption spectra further confirm that hole transfer from ITIC to the donor becomes more efficient upon optimizing the morphology of the ternary blend compared to that of donor:ITIC binary blend.</P> [FIG OMISSION]</BR>
