만성 콩팥병 환자에서 추정 사구체 여과율에 따른 25-Hydroxyvitamin D의 특성

Background/Aims: Accumulating data suggest that vitamin D deficiency is prevalent in patients with chronic kidney disease (CKD). However, comprehensive data are lacking for Koreans. The aim of this study was to survey vitamin D deficiency among patients with CKD in Korea and to identify the relationships among various factors. Methods: We conducted a retrospective cohort study of 444 patients who were divided into four subgroups by estimated glomerular filtration rate (eGFR) for comparisons of mean 25-hydroxyvitamin D [25(OH)D] level and other parameters. In addition, non-dialyzed patients were categorized into four groups based on 25(OH)D levels (<10, 10-19, 20-29, and ≥ 30 ng/mL), and risk factors for severe vitamin D deficiency (<10 ng/mL) were investigated. Results: Of patients with an eGFR ≥ 60 mL/min/1.73 m2, 43% (34/79) showed severe 25(OH)D deficiency, and the mean 25(OH)D level was 11.7±5.3 ng/mL. In CKD3 group, 53.2% (41/77) showed severe 25(OH)D deficiency, with a mean level of 11.3±7.2 ng/mL. In CKD4 group, 53.3% (49/92) had severe 25(OH)D deficiency, with a mean level of 11.0±6.2 ng/mL. Approximately 71% (139/196) of patients in CKD5 group showed severe deficiency, and the mean level was 9.2±5.9 ng/mL. Severe 25(OH)D deficiency was affected by winter season, renal function, diabetes, and low-density lipoprotein cholesterol. The serum parathyroid hormone level was inversely correlated with the 25(OH)D level, such that 25(OH)D < 20 ng/mL were associated with a steep increase in parathyroid hormone. Conclusions: Vitamin D deficiency is highly prevalent in the Korean population. Few patients met a sufficient 25(OH)D concentration, even in the early stages of CKD. Our data suggest that 25(OH)D level of 20 ng/mL is a threshold for a rapid increase in parathyroid hormone levels. (Korean J Med 2012;83:740-751)
Effect of structural geometry and crack location on crack driving forces for cracks in welds

<P><B>Abstract</B></P><P>This paper quantifies the effect of geometry (planar or cylindrical) and crack location (internal or edge cracks; weld center or interface cracks) on crack driving force for welded joints, via systematic elastic–creep and elastic–plastic finite element (FE) analyses for welded joints. For engineering estimates of crack driving forces for mismatched welded joints, the equivalent material approach is employed. It is found that the equivalent material concept works very well only for a planar geometry with an internal crack, such as the middle cracked tension specimen. For a planar geometry with an edge crack, it works reasonably well, but tends to provide conservative results for under-matching and for interface cracks. For a cylindrical geometry with an edge crack, the results are similar to those for a planar geometry with an edge crack, but caution should be exercised for over-matching, as non-conservative estimates are possible due to gross-section yielding. For a cylindrical geometry with an internal crack, excessively conservative estimates for under-matching are found, and thus an improved estimation method is desired.</P>

Objective : The purpose of this study was to evaluate the effects of low level lasers on bone healing and new bone formation around titanium dental implants in canine models. 18 oxidized surface treated implants and a Dens-bio laser were used. Study design : Low level lasers were irradiated with a total of 8J for 4 minutes by pulse wave type and 1 minute by continuous type. For the experimental group, a low level laser was used to irradiate the first premolar implant's insertion area at the time of insertion, a low level laser was used to irradiate the second premolar implant's insertion area daily for one week after implant insertion, and a low level laser was used to irradiate the third molar implant's insertion area daily for 2 weeks postoperatively. At the conclusion of the study, sacrificed tissue sections were made from investing tissue and observed under an optical microscope. Results : The rate of new bone formation around the implant showed no significant difference between the control group and the experimental group. New bone formation rates of the control and experimental group 2 weeks following implant placement were higher than that of immediately after implant placement and 1 week after implant placement. Conclusions : Based on these results, a low-level laser showed no statistically significant increase in bone formation following implant placement.
Traumatic Neuroma Following Mandibular Angle Reduction : A Case Report

A traumatic or amputation neuroma can develop as a result of nerve injury caused by hemorrhage, infection, ischemia, trauma, etc. A traumatic neuroma can be considered as the attempted regeneration of the amputated nerve rather than a true neoplasm. Occasionally, a traumatic neuroma will develop after oral or maxillofacial surgery. Here, we report a case of traumatic neuroma after mandibular angle reduction in a 24-year-old woman.
Feasibility of a polydioxanone plate as an adjuvant material in rhinoplasty in Asians

Background Nasal framework-supporting procedures such as septal extension grafts, derotation grafts, and columellar strut grafts are usually required in rhinoplasty in Asian patients because the skin envelope is tight, but the nasal framework is small and weak. Autologous materials are preferred, but they have some limitations related to the amount that can be harvested and the frequency of use. Therefore, synthetic materials have been used to overcome these limitations. Methods A total of 114 patients who received a polydioxanone (PDS) plate as an adjuvant material in rhinoplasty from September 2016 to August 2017 were retrospectively investigated. The PDS plate was used as to support the weak framework and to correct the contour of the alar cartilages. The PDS plate was used for reinforcement of columellar struts and septal L-struts, alar cartilage push-down grafts, fixation of septal extension grafts, and correction of alar contour deformities Results Primary and secondary rhinoplasty was performed in 103 and 11 patients, respectively. Clinically, no significant inflammation occurred, but decreased projection of the tip was observed in seven patients and relapse of a short nose was noted in five patients. Conclusions PDS plates have been used in the United States and Europe for more than 10 years to provide a scaffold for the nasal framework. These plates can provide reinforcement to columellar struts, L-struts, and septal extension grafts. In addition, they can assist in deformity correction. Therefore, PDS plates can be considered a good adjuvant material for Asian patients with weak and small nasal cartilage.

Hot Deformation and Cracking during Compression of 21-4N Steel

<P>Hot compressive deformation and cracking behaviors of cast 21-4N steel were investigated over a temperature range between 900°C~1250°C. The strength of the specimen at the outer surface of ingot is greater than that in the inner part because of the initial rapid solidification and corresponding fine structure at the outer surface. The initial flow stress hump in the stress-strain curves can be attributed to the dynamic recrystallization, which requires the initial strain energy build-up before the activation of grain nucleation process. With increase of temperature the critical stain for dynamic recrystallization decreases and at high enough temperatures, no critical strain for the build-up of strain energy is needed. At temperatures between 1000°C and 1200°C, homogeneous deformation occurred and no cracks were observed even after 50% strain. The cracks observed below 1000°C could be associated with the more difficult plastic flow in the presence of carbides and nitrides. The presence of fatal crack at 1250°C and the loss of hot ductility can be attributed to the partial melting in austenite grain boundaries at high temperatures.</P>
