Objectives: To determine the educational needs related to an integrated health and oral health project for community dental hygienists. Methods: In this cross-sectional study, a survey was administered to 1,190 dental hygienists working in community health centers and 627 (about 53%) responded. The dependent variable was educational needs from oral health projects; the independent variables were region, job assignments, job position, and degree of self-development. Data were analyzed using descriptive statistics and chi-square tests, with PASW 20.0 (SPSS Inc., Chicago, IL, USA), and the significance threshold was .05. Results: The educational needs of community dental hygienists were found to be high, at about 58.5 points out of 100. The highest educational needs were for oral health projects linked to public health projects. In this regard, participants reported a high need for “identification of issues and projects suited to the current state of the region,” “establishment of strategies for integration of health and oral health projects,” and “prioritization.” Conclusions: Professional education for community dental hygienists should be expanded to include integration of health and oral health. It should be based on the assessment of these professionals’ educational needs.
급성 심근경색증 환자에서 조기에 적절한 재관류 요법을 시행받지못한 요인

N/A Objectives: Early application of reperfusion therapy such as thrombolytic agent or direct PTCA is utmost important to amximize the efficacy of reperfusion therapy in patients with aute myocardial infarction. However, early adequate reperfusion thearpy was given only in 15% to 36% pf patients with acute myocardial infarction and substantial patients were not benefited from reperfusion therapy, Therefore, we performed these study to evaluate the reasons for which patients with acute myocardial infarction cannot receive an adequate reperfusion therapy. Methods: We analyzed the initial electrocardiogram and various time delays from chest pain onset time, first hospital arrival time, transfer time, ER arrival time, and door to reperfusion time in 138 patients with acute myocardial infarction from Jan. 1991 to Oct. 1993 admitted to Wonju Christian Hospital. Patients were grouped as reperfusion group(n=55) who had reperfusion therapy such as thrombolytic trial or direct PTCA and conservative grou who had not received reperfusion therapy(n= 83). Results: 1) Eighty-three cases(60.1%) did not received an adequate reperfusion therapy. 2) Hospital arrival time of patients was 237±162min and 786±615min in reperfusion and conservative group(p<0.05) respectively. Patient time delay was 103±98min and 439±511min, first hospital time delay, 93±78min and 333±482min, and transfer time, 81±59min and 105±64min in reperfusion and conservative group respectively(p<0.05). Only 4.8% of patients was transferred from first hospital after reperfusion therapy. 3) Patient time delay was the most common reason of not receiving reperfusion therapy, and time delay and lack of adequate reperfusion therapy at the first hospital inspite of early arrival were the second place. Other reasons were contraindication to lytic therapy(4 cases), definite Q wave MI(11 cases), inconclusive EKG(4 cases), and delay in therapy(4 cases). Conclusion: The most common reason of not receiving reperfusion therapy was patient delay in seeking medical care(45.8%) and time delay at first hospital in 22 cases(26.5%). To maximize the effectiveness of reperfusion therapy, it is important to shorten hospital arrival time delay and widespread use of reperfusion therapy at first hospital is recommeded.
Estrogen activity of Silkworm (Bombyx mori) Pupa water extract and its fractions

This study was conducted to evaluate the estrogen activity of silkworm (Bombyx mori) pupa extracts and their fractions. Powdered samples of freeze-dried silkworm pupa were extracted at room temperature (RT), $40^{\circ}C$, $60^{\circ}C$, $80^{\circ}C$, and $100^{\circ}C$ in water (D.W), chloroform, ethyl acetate, and methanol for 6h and then filtered (0.45 um). The extracts were then freeze-dried. The estrogenic activity of these extracts was then investigated by competition binding assays using estrogen receptor ${\alpha}\;(ER{\alpha})$ and $ER{\beta}$, and by evaluating their effects on the proliferation of the human breast cancer cell line, MCF-7. Among the extracts evaluated, water extracts prepared at RT showed the highest binding affinity to $ER{\alpha}$ ($IC_{50}$, 1.76 ug/ml) and $ER{\beta}$ ($IC_{50}$, 0.07 ug/ml). In addition, MCF-7 cells that were treated with 62.5 ug/ml of the RT extract showed the greatest increase in proliferation (2-fold; 1291.79%) when compared to control cells (659.82%). Next, the water extract that was prepared at RT (sample 1) was dissolved in D.W. and further fractionated using a Dowex 50W - 8X ($H^+$) column. The flow-through and wash were then pooled together and freeze-dried (sample 2). The bound materials were then eluted with 20 mM NaCl, after which they were applied to a Dowex 1X2 - 200 ($Cl^-$) column and washed with D.W. to remove the sodium ions. The eluants were then freeze-dried (sample 3). Of these fractions, sample 2 showed the highest binding affinity to ER{\alpha} ($IC_{50}$, 1.44 ug/ml) and $ER{\beta}$ ($IC_{50}$, 1.18 ug/ml). In addition, MCF-7 cells that were treated with sample 2 (15.6 ug/ml) showed the largest increase in growth (1159.39%) when compared to control cells (525.26%). Taken together, these results suggest that the fraction of the RT water extract of silkworm pupa referred to as sample 2 may be useful as a phytoestrogen.

Fire model shall be verified and validated to reliably predict the consequences of fires within its limitations. This study aims to predict pool fire phenomena with two rooms using CFAST and to compare CFAST simulation results with PRISME experimental data which can be applicable to the fire of nuclear power plant facility. Five different mass loss rate(MLR) are used in the simulation and the simulated results of specific quantities such as temperature, chemical composition are compared to the experimental data. From this study, the CFAST simulation results with the proper MLR show better similarity and trend with pool fire experimental data.
소아 응급실에 내원한 아나필락시스와 구강점막부종을 동반한 혈관부종의 임상 특성 비교

Objectives: We aimed to compare and distinguish the characteristics of anaphylaxis and angioedema, especially with oral mucosal involvement and treatment of patients who visited the Pediatric Emergency Department. Methods: We retrospectively analyzed patients under age 18-year-old who were diagnosed with anaphylaxis and angioedema with oral mucosal involvement and treated with epinephrine from May 2008 to May 2013 in a single Pediatric Emergency Department in Seoul, Korea. We evaluated their past history, possible triggering causes, symptoms, vital signs and treatment and discharge with education. Results: During the study period the total cases of anaphylaxis were 79 and angioedema with oral mucosal involvement were 218. The age of patients with anaphylaxis was significantly higher (6.6±4.9 years vs. 4.1±3.3 years). The heart rate relative to age was significantly higher in the anaphylaxis group (49.4% vs. 36.2%). After discharge from the Emergency Center, 3.8% of anaphylaxis patients were prescribed an epinephrine injection. Education to avoid the triggering factor was provided in 32.9% of anaphylaxis group and 17.4% in the angioedema group. Conclusion: Besides blood pressure, we should pay attention to the heart rate in pediatric patients with severe allergic reactions. More active follow-up of anaphylaxis and angioedema with oral mucosal involvement is needed to educate parents and prescribe emergency medication.

Capsaicin, a major pungent constituent of red pepper (Capsicum annuum L.) possesses a vast variety of pharmacologic and physiologic activities. Despite its irritant properties, the compound exerts anti-inflammatory and anti-nociceptive effects. Previous studies from this laboratory revealed that capsaicin, when topically applied onto dorsal skin of female ICR mice, strongly attenuated activation of NF-kB and AP-1 induced by the typical tumor promoter, 12-O-tetradecanoylphorbol-13-acetate (TPA), which may account for its anti-tumor promoting activity in mouse skin. In the present work, we have found that capsaicin suppresses TPA-stimulated activation of NF-kB through inhibition of $IkB{\alpha}$ degradation and blockade of subsequent nuclear translocation of p65 in human pro myelocytic leukemia HL-60 cells. Methylation of the phenolic hydroxyl group of capsaicin abolished its inhibitory effect on NF-kB DNA binding. Likewise, TPA-induced activation of AP-1 was mitigated by capsaicin treatment.
