        COMPREHENSIVE RURAL DEVELOPMENT STRATEGIES OF KOREA AND THEIR IMPLICATIONS TO DEVELOPING COUNTRIES

        This paper analyzes a series of policies and programs for the comprehensive rural development of Korea, and proposes a suitable method for rural development in developing countries. Each policy is described and compared based on the background, implementation methods, progress, performance, implications, etc. This study provides suggestions for developing countries with useful practical tips on eradicating absolute poverty, including appropriate target regions and comprehensive development strategies adoption. This paper provides several policies and project support for rural development in developing countries. However, Korea`s successful experience will not produce the same effects on developing countries. Therefore, those suggestions need to be adopted with considerations on specific social, economic contexts of developing countries.

        Trauma surgery without proper compensation under the current Korean National Health Insurance System

        Purpose: This study aimed to evaluate the situations and problems of the current health insurance fees for trauma surgeries. Methods: We reviewed the medical records and billing data from trauma surgeries performed in the hospital from August 2012 to July 2014. The name and number of surgeries were investigated and the code and number of operations prescribed by surgeons were compared with the number of cases actually billed to insurance. In addition, the results returned by the Health Insurance Review & Assessment Services (HIRA) after deduction were investigated and compared to verify the insurance review findings. Results: During the study period, total 1,534 trauma surgical procedures were performed for a total of 253 trauma patients. Based on names, 1,092 procedures were performed; however, 442 cases (28.8%) could not be prescribed because of lack of proper insurance codes for the procedures. A total of 1,046.5 surgical procedures were prescribed by surgeons, adjusted by the insurance team, and finally billed to the HIRA; 162 bills were returned from the HIRA after rate reductions, corresponding to a reduction rate of 15.5%. The major reason for reduction was “fee criteria and limited number”. The compensation rate for billed surgical procedures was 84.5%. Conclusion: The high reduction and low compensation rate for trauma surgery under the current Korean National Health Insurance System need to be reviewed and improved. Furthermore, it is necessary to establish new criteria for surgical procedures fees for latest ones such as damage control surgery performed on severe trauma patients.

      • 『한국형 ODA 모델』중 농림업 분야 국제협력 프로그램 심화 연구(2차년도)

        연구의 배경 2010년 OECD/DAC 가입 이후 우리나라는 기존의 공여국과 차별화되는 우리나라의 비교우위에 입각하여 ‘한국형 ODA 모델’을 도입하였다. 개발협력 대상국의 빈곤퇴치와 지속가능한 농촌 발전을 위하여 국제적 원조규범을 존중하고 우리의 개발협력 여건을 고려하며 개발협력국 실정에 맞는 농림업 분야 국제개발협력의 수요가 늘어나고 있다. 이 연구는 ‘한국형 ODA 모델’ 중 농림업 분야 협력프로그램의 콘텐츠를 현실성 있게 보완하여 개도국에 적용 시 그 효과성을 제고할 수 있도록 할 필요가 있다는 배경 아래 수행되었다. 연구방법 이 연구는 ‘한국형 ODA 모델’ 중 농림업 분야의 프로그램에 대한 콘텐츠를 심화하기 위하여 2년간 시행한 연구사업의 최종 결과물이다. 이 분야 20개 협력 프로그램 가운데 5개 프로그램(농촌종합개발, 농업기술 개발 및 현장지도 지원 시스템, 농업생산기반, 식량작물 생산성 향상, 농산물 가공)은 첫해인 2014년에 수행되었다. 2015년에는 ‘한국형 ODA 모델’ 의 나머지 14개의 프로그램을 11개의 프로그램으로 통합하여, 이 중 5개의 프로그램은 외부 전문가에게 집필을 의뢰하여 수행하였다. 이 연구는 농림업 분야의 협력 프로그램의 전략서로서 대상국 개발협력에 적용 가능성을 높이기 위하여 과거 우리나라의 농정경험뿐만 아니라 국내외 개발협력 프로그램의 사례 등을 제시하고 이를 통해 개도국 적용 시 고려해야 할 착안점을 제시하고 있다. 연구결과 및 시사점 이 보고서는 11개 프로그램의 총괄보고서로서, 각 프로그램별로 한국의 시대별 농정변화, 한국의 국제협력사례, 국제기구의 협력사례 및 개도국 적용을 위한 착안점을 포함하여 제시하고 있다. 이 보고서와 별도로 작성된 각 프로그램별 보고서는 시대 흐름에 따른 우리나라의 정책적 변화를 기술하고 다양한 정책의 성패를 분석하여 시사점을 도출하고 있다. 국내외 개발협력 프로그램 사례를 제공하고 성패요인 분석 및 출구전략, 착안점을 제시함으로써 국제개발협력 시 유관기관에서 참고할 수 있도록 하는 것이 이 연구의 목적이다. 2014년 5개의 프로그램 보고서를 포함한 16개의 프로그램 보고서는 개발협력 대상국의 농업분야 협력사업의 발굴, 세부사업 선정 그리고 사업의 수행 시 참고자료로서 활용도가 높을 것이며, 농림축산식품부를 비롯한 유관기관에게도 관련 정책 수립의 기초자료로 활용할 수 있을 것이다. Background of Research Since Korea became a member of OECD/DAC in 2010, it has introduced so-called “Korean ODA Model” which would serve as a strategic guideline for development cooperation. It is intended to be a unique model differentiated from ones in other countries. There are increasing demands toward agricultural and rural development projects for poverty eradication and sustainability which are right both to the international aid norms and socio-political conditions of Korea. This research tries to enhance the effectiveness through complementing contents of cooperative programs in the Korean ODA Model. Method of Research Under the same background and research purpose, five programs-agricultural R&D and extension system, integrated rural development, agricultural production infrastructure, crop productivity, and agricultural processing- were covered by the research in 2014. The rest fourteen programs are consolidated into eleven programs, which are the topics of this research. It provides not only policy experiences of Korea in the fields of agriculture but implications for application to the context of partner countries. Research Results and Implications As a synthesizing report which sums up all eleven program reports, this research reviews agricultural policy changes in Korea by periods, and development cooperation project cases, and then it offers key points for application to the developing countries. Program reports describe policy changes related with each sector, analyze the factors of success and failure, and derive implications and exit strategies. It will be useful to partner countries which are currently in need of policy transformation, and the project implementing agencies as well. Sixteen program reports from two-year research will serve as basic references for those authorities and organizations concerned such as the Ministry of Agriculture, Food and Rural Affairs (MAFRA) in their formulation of cooperative projects, identification of needs and activities, and project implementation.

        Systematic Preventable Trauma Death Rate Survey to Establish the Region-based Inclusive Trauma System in a Representative Province of Korea

        Background: Trauma mortality review is the first step in assessing the quality of the trauma treatment system and provides an important basis for establishing a regional inclusive trauma system. This study aimed to obtain a reliable measure of the preventable trauma death rate in a single province in Korea. Methods: From January to December 2017, a total of 500 sample cases of trauma-related deaths from 64 hospitals in Gyeonggi Province were included. All cases were evaluated for preventability and opportunities for improvement using a multidisciplinary panel review approach. Results: Overall, 337 cases were included in the calculation for the preventable trauma death rate. The preventable trauma death rate was estimated at 17.0%. The odds ratio was 3.97 folds higher for those who arrived within “1–3 hours” than those who arrived within “1 hour.” When the final treatment institution was not a regional trauma center, the odds ratio was 2.39 folds higher than that of a regional trauma center. The most significant stage of preventable trauma death was the hospital stage, during which 86.7% of the cases occurred, of which only 10.3% occurred in the regional trauma center, whereas preventable trauma death was more of a problem at emergency medical institutions. Conclusion: The preventable trauma death rate was slightly lower in this study than in previous studies, although several problems were noted during inter-hospital transfer; in the hospital stage, more problems were noted at emergency medical care facilities than at regional trauma centers. Further, several opportunities for improvements were discovered regarding bleeding control.

        Risk factors for mortality of severe trauma based on 3 years` data at a single Korean institution

        Purpose: This study aimed to determine the mortality rate in patients with severe trauma and the risk factors for trauma mortality based on 3 years’ data in a regional trauma center in Korea. Methods: We reviewed the medical records of severe trauma patients admitted to Ajou University Hospital with an Injury Severity Score (ISS) > 15 between January 2010 and December 2012. Pearson chi-square tests and Student t-tests were conducted to examine the differences between the survived and deceased groups. To identify factors associated with mortality after severe trauma, multivariate logistic regression was performed. Results: There were 915 (743 survived and 172 deceased) enrolled patients with overall mortality of 18.8%. Age, blunt trauma, systolic blood pressure (SBP) at admission, Glasgow Coma Scale (GCS) at admission, head or neck Abbreviated Injury Scale (AIS) score, and ISS were significantly different between the groups. Age by point increase (odds ratio [OR], 1.016; P = 0.001), SBP ≤ 90 mmHg (OR, 2.570; P < 0.001), GCS score ≤ 8 (OR, 6.229; P < 0.001), head or neck AIS score ≥ 4 (OR, 1.912; P = 0.003), and ISS by point increase (OR, 1.042; P < 0.001) were significant risk factors. Conclusion: In severe trauma patients, age, initial SBP, GCS score, head or neck AIS score, and ISS were associated with mortality.

        When an aqueous liquid drop is impinged on a superhydrophobic porous surface, the liquid penetration may occur in the form of a liquid jet or micro droplets depending on an impact velocity. Here by impacting a liquid drop on a superhydrophobic surface with a single micro pore, we demonstrate that it is possible to dispense a single micro drop of a controlled size in a reproducible way, while proposing the impact condition (i.e., impact velocity) under which a single micro droplet is generated, as a function of a micro pore size and a liquid viscosity. It is found that a larger impact velocity is required to generate a single micro drop as a micro pore size decreases, while an impact velocity for a single micro drop generation appears to be rather insensitive to the liquid viscosity. Also, the diameter of micro drop is found to be roughly 1.5 times of the diameter of micro pore for all tested pore sizes, such that about 75 μm-diameter micro droplet is obtained with a 50 μm-diameter micro pore. As an application example, we demonstrate the successful printing of an array of micro droplets on a paper. We believe that the present approach can be a simple and effective strategy to micro droplet printing without the typical issues associated with droplet printing, such as clogging or satellite droplet generation.
