A prospective study was conducted to see the changes of various immunologic indices and its significance in minimal change nephrotic syndrome (MCNS). Study population consisted of 51 children with MCNS (7 infrequent relapser; IR, 38 frequent relapser steroid depen- dent; FR/SD and 6 partial responder; PR), in whom diagnosis was confirmed by percutaneous renal biopsy. Disease activity was divided into 3 stages; stage I- relapse period, stage II-within 30 days of remission and stage III-over 30 days of remission. The following results were obtained: Urinary N-acetyl-b-D-glucosaminidise (nM/min/mg- creatinine) showed elevated levels in stage I (72.7±60.3) and stage II (23.6±16.2) compared with stage III (6.8±4. 8) and control (4.0±+1.6). Urinary neopterin (mmol/mol creatinine) showed in- creased level in stage I (280+20) and stage II (198+22) compared with stage III (167±63) and control (170±36). The total T cell count (%) showed no changes between stages, nor between study group and control. However, T cell subset analysis showed T, cell count (%) lower in stage I (20.4±11.6). And stage II (20.1±7.6) compared with stage III (28.3 ±10.4). The T,/T, ratio was lower in stage I (0.64±0.41) and stage II (0.75± 0.56) compared with stage III (1.58± 1.08), and this lowering of T,/T, ratio was mainly caused by increase of T, cells. The total B cell count (%) showed no changes between stages, nor between study group and control. During relapse period, serum IgG was lower than control (344±248 versus 1.045±300 mg/dl). Serum IgE was also elevated in study patients compared with control (316±350 versus 49±37 mg/dl), and the elevation of IgE was more marked in FR/SD and PR group than IR group (67% versus 25%). In summary, it can be concluded from the present study that intimate relationship exists between MCNS and immune abnormality. The most pronounced changes has been noted in 8 subset and serum IgE, and marked elevation of serum IgM could be used as a predictor for frequent relapse or partial response in MCNS children.

GPGPU를 이용한 파데브-레브리어 알고리즘 구현 및 성능 분석

In this paper, we implement the Faddev-Leverier algorithm using GPGPU (General-Purpose Graphics Processing Unit) to accelerate singular value decomposition. In addition, we compare the performance of the algorithm using CPU and CPU plus GPGPU for eleven × matrix sizes in order to decompose singular values, where =4, 8, 16, 32, 64, 128, 256, 512, 1,024, 2,048, and 4,096. Experimental results indicate that CPU achieves better performance than CPU plus GPGPU for ≤ 64 because of a large number of read and write operations between CPU and GPGPU. However, CPU plus GPGPU outperforms CPU exponentially in the execution time for ≥ 64.

Does the use of bedside ultrasonography reduce emergency department length of stay for patients with renal colic?: a pilot study

Objective The aim of this study was to evaluate the effect of adding bedside ultrasonography to the diagnostic algorithm for nephrolithiasis on emergency department (ED) length of stay. Methods A prospective, randomized, controlled pilot study was conducted from October 2014 to December 2014 with patients with acute flank pain. In the non-ultrasonography group (NUSG), non-contrast computed tomography was selected based on clinical features and hematuria in the urinalysis. In the ultrasonography group (USG), non-contrast computed tomography was selected based on clinical features and hydronephrosis on bedside ultrasonography. The primary outcome was ED length of stay. The secondary outcomes were radiation exposure, amount of analgesics, proportion of patients with diseases other than ureteral calculus, and proportion of patients with unexpected ED revisits within 7 days from the index visit. Results A total of 103 patients were enrolled (NUSG, 51; USG, 52). The ED length of stay for the USG (89.0 minutes) was significantly shorter than that for the NUSG (163.0 minutes, P<0.001). There were no significant differences between the two groups in the radiation exposure dose (5.29 and 5.08 mSv, respectively; P=0.392), amount of analgesics (P=0.341), proportion of patients with diseases other than ureteral calculus (13.0% and 6.8%, respectively; P=0.486), and proportion of patients with unexpected ED revisits within 7 days from the index visit (7.8% and 9.6%, respectively; P=1.000). Conclusion The use of early bedside ultrasonography for patients with acute flank pain could reduce the ED length of stay without increasing unexpected ED revisits.

소아 야뇨증의 진단과 치료

야뇨증은 소아뿐만 아니라 부모들에게 매우 애를 먹이는 현상이며, 때로는 정신적, 정서적 장애 또는 행동장애를 유발할 수도 있다. 적절한 치료를 하기 위하여서는 세심한 병력, 신체검사와 필요에 따라서 적절한 실험실 검사 및 방사선검사으로 단인증상성 야뇨증인지를 제대로 구별하여야 한다. 실제 임상에서 야뇨증의 치료는 그렇게 용이하지만은 않아 우선 어린이와 부모들이 야뇨증을 제대로 이해할 수 있도록 사전 교육을 통하여 적극적인 치료 의지를 갖도록 하는 것이 중요하다. 여러 가지 행동요법 및 야뇨경보기, 적절한 약물요법을 환자의 상황에 맞도록 적절한 치료 방법을 모색하여 보다 치료 효과가 높고 재발이 적도록 노력하여야 한다.

저신장 소아를 위한 성장호르몬 치료

Normal growth and development is of prime concern during childhood. The treatment of children with growth hormone deficiency has been revolutionized by growth hormone therapy. An improved height outcome with a final height within the target height range has been achieved. However, close follow-up with regular clinical and laboratory monitoring is essential for achieving the desirable height outcome. The theoretical unlimited supply of growth hormone has led to its wide spread use in a variety of disorders other than a growth hormone deficiency. Initially used in children with Turner syndrome, growth hormone is now used to treat chronic renal failure, an idiopathic short stature and intrauterine growth restrictions in addition to a wide array of newly emerging indications. This review summarizes the basics for a proper growth assessment, the differentiation of normal and abnormal growth causes of a short stature, and the indications for growth hormone treatment.

Transdermal Delivery of Cosmetic Ingredients Using Dissolving Polymer Microneedle Arrays

The efficiency of transdermal delivery of cosmetic ingredients is often limited by the outer layer of the skin, known as the stratum corneum, which can prevent diffusion of the cosmetic ingredients through the skin. A polymer microneedle array that dissolves in the skin can enhance the permeability of the skin to cosmetics. In this study, we prepared a polydimethylsiloxane (PDMS) mold to fabricate a microneedle array using laser-writing process which is a very simple and efficient method compared to conventional methods for preparing molds. Polyvinylpyrrolidone (PVP) and adenosine were used as a base material for the dissolving microneedles and a model cosmetic compound, respectively. Poly(ethylene glycol) dimethacrylate (PEGDMA) was copolymerized with PVP to control the properties of microneedles such as mechanical strength and solubility. PVP microneedle array was sufficiently sharp and with enough mechanical strength to create a transdermal pathway through the skin. The dissolution rate of the needle decreased with increasing PEGDMA content in the microneedle of PVP-PEGDMA copolymer. When adenosine was applied to the skin with the microneedle array, skin permeability to adenosine was improved by 150% compared to the control (without a microneedle array). These results indicate that the PVP microneedle array developed in this study has a potential to be used in cosmetics by combining with conventional cosmetic patches.

Cyclic AMP가 흰쥐 사구체 상피세포의 Heparan Sulfate Proteogllycan 유전자 조절에 미치는 영향

It is well known that the glomerular basement membrane heparan sulfate proteoglycan(GBM HSPG) synthesized by glomerular epithelial cell(GEC) has an important role in the permeability of glomerular basement membrane and cyclic AMP(cAMP) is involved in regulation of a wide variety of genes maybe including GBM HSPG gene. The direct effect of cAMP on GBM HSPG gene expression and metabolism was not evaluated as yet. Proteinuria represents an impairment of permselectivity function of glomerular basement membrane regulated by GBM HSPG and could be associated with increased glomerular level of cAMP in nephrotic syndrome of diverse causes. RPD-I(rat GBM HSPG core protein domain-I) detected a $gt;9.5kb transcript of GBM HSPG in RNA of rat GEC. Employing a riboprobe synthesized from RPD-I in Rnase protection assay, we examined whether cAMP regulated perlecan expression in the GEC. At 1, 6, 24 and 48 hrs of incubation, 1 mM cAMP caused 43%, 32%, 47% and 40% reduction in mRNA expression of perlecan, respectively. Immunoprecipitation showed a corresponding reduction of 51%, 70% and 68% in the synthesis of SO₄ labeled GBM HSPG by the GEC following 12, 24 and 48 hrs of incubation with cAMP. Our results show that decrease in GBM HSPG gene expression and synthesis by cAMP may be of relevance to proteinuric states characterized by activation of these mediators.
