Gateway to Clinical Prescription about Zoledronic Acid

Osteoporosis is a preventable disease associated with substantial morbidity and mortality characterized by loss of bone mineral density, progression to diminished skeletal integrity, extensive bone fragility, and an increased risk of fracture. Fractures are an important cause of disability among postmenopausal women, and the public health costs are substantial. Bisphosphonates, the most commonly used treatment for established osteoporosis, inhibit osteoclast-mediated bone resorption and reduce the risk of vertebral fracture. Intravenous zoledronic acid 5 mg once yearly is effective in reducing the risk of several types of fracture in patients with postmenopausal osteoporosis or recent low-trauma hip fracture. Moreover, improvements in BMD and reductions in markers of bone turnover are also generally observed. Zoledronic acid is generally well tolerated. Additional comparative data are required to definitively position zoledronic acid with respect to other agents. In the meantime, intravenous zoledronic acid 5mg once yearly is a convenient and effective treatment option that may have an advantage over some other agents, for which adherence to treatment regimens is a recognized problem.
Bisphosphonates가 Osteoclast 분화에 미치는 기전 연구

목적: Alendronate는 대표적인 제3세대 아미노-비스포스토네이트로서 일차적인 작용이 파골세포의 골 흡수 억제라 할지라도, 최근 비스포스포네이트가 조골세포에도 영향을 준다는 연구들이 제시되고 있다. 그러나 alendronate가 조골세포와 파골세포의 형성 및 분화에 미치는 정확한 기전에 대한 분자생물학적 수준에서의 연구는 매우 제한적이다. 그러므로 본 연구는 Bisphosphonates가 osteoclast 분화에 미치는 기전 연구 작용기전에 대해 조사하고자 한다. 방법: C2C12와 MC3T3-E1세포를 FBS가 포함된 DMEM, 배양액에 배양하면서 alendronate를 RT- PCR 방법을 이용한 ALP, osteocalcin(OCN), collagen I α1(Col 1), inhibitor of differentiation(Id) 및 cathepsin K(CTSK) 등의 조골세포와 파골세포 분화 관련 유전자의 발현을 조사하였다. Id 유전자의 발현 증가에 관여하는 전사인자를 TRANSFEC program을 이용하여 동정하고 이들 전사인자가 Id 유전자의 발현에 미치는 효과를 promoter 분석을 통하여 조사하였다. 결과: ALP, Col 1 및 OCN의 유전자 발현은 alendronate 처리 24시간까지는 두 세포주 모두에서 alendronate의 농도가 감소함에 따라 증가하는 경향을 보였다. Id-1의 유전자의 발현은 대조군에 비해 10-6 M에서 증가하여 10-8 M까지 유지되었으나, Id-2는 alendronate 농도에 따라 점차적으로 증가하였으며, 72시간에서는 Id-1과 Id-2 유전자 모두 발현량이 감소하였다. Id 유전자 프로모터 내 전사인자 가운데 CREB와 C/EBPβ는 Id 프로모터의 활성도를 각각 2배, 4배로 증가시켰으며, C/EBPβ와 alendronate를 병합 처리 시 Id-1 프로모터 활성도는 각각을 단독으로 처리했을 때보다 유의하게 증가하였다. 결론: alendronate에 의한 분화 촉진 효과는 C/EBPβ 전사인자와 상호 작용하여 Id 유전자의 발현 증가를 야기시키고 있음을 시사하고 있다.
표준화재 재하조건 콘크리트 중공슬래브의 피복두께에 따른 화재거동에 관한 실험적 연구

The concrete void slab structure with the existing mushroom slab, is the structure that maximizes the advantages, while minimizing the weakness with removing useless body force of the concrete part,located on the center of the slab cross-section, which does not need to support the structural weight. In this research, a fire test is performed to analyze how the blaze behave according to the thickness of slab cover, with the practical span length of concrete void slab for the slab length 7.5 m. With this heating test, we assumed the uniform-load-model considering fixed loads and live loads, and chose the standard fire test condition. We measured the temperature changes and the deflection character according to the depth from the heat exposure side, and assessed the resisting capability according to the standard KS F 2257-1. The result comes out with the EPS model can secure about 2 hour fireresisting-capability with 50 mm of cover depth. 기존의 무량판 구조와 동일한 콘크리트 중공슬래브구조는 구조적 하중지지 성능이 불필요한 슬래브 단면상의 중앙부 콘크리트를 제거하여 슬래브의 자중을 줄이고 무량판 구조의 단점을 보완하여 장점을 극대화시킨 구조형식이다. 본 연구에서는 콘크리트 중공슬래브 공법의 실제 스팬길이인 7.5 m에서 슬래브 피복두께에 따른 화재거동 영향성 분석을 위하여 화재실험을 수행하였다. 하중조건은 고정하중과 적재하중을 고려하여 실험체에 등분포 조건으로 사전 재하하였으며, 표준화재조건으로 재하가열 실험을 수행하였다. 슬래브의 화재가열 노출면으로부터의 깊이별 온도변화와 처짐변형 특성을 측정하였으며, KS F 2257-1평가기준에 의거하여 슬래브의 내화성능을 평가하였다. 실험결과 피복두께 50mm를 확보할 경우, EPS 중공체로 제작한 실험체의 경우 약 2시간 정도의 내화성능을 확보할 수 있는 것으로 나타났다.
자궁근종에 대한 GnRH agonist 치료에 따른 효과 -임상적 효용 및 전자현미경적 연구-

-Abstract- Administration of gonadotropin hormone-releasing hormone(GnRH) agonist, through a process of pituitary desensitization and down-regulation of receptors, inhibits the secretion of gonadotropin and sex-steroids and induces a reversible suppression of gonadal activity. This approach can be used as an effective endocrine therapy for some hormone-dependent tumors. We have used GnRH agonist, for the treatment of four patients with uterine leiomyomas, thereafter myometomy was performed in all cases. To assess the effect of treatment with GnRH agonist, ultrastructural changes of the tumor cells of leiomyoma are studied, along with measurement of changes in serum hormone levels(LH, FSH, and E2) and the sizes of the neoplasm. Electron microscopic findings of myoma cells after the use of GnRH agonist included the following : increased mitochondria, swelling of mitochondria with loss of cristae, clumping of nuclear chromatin, dilation of endoplasmic reticulum and perinuclear vacuolation, and loss of myofilaments. The use of GnRH agonist may be effective in reducing the size of a myoma considerably by primarily inducing medical hypophysectomy and would allow easier surgical removal.
