        압력 측정에 의한 요량 계측의 유용성 분석

        Uroflowmetry is non-invasive and easily performed to diagnose benign prostatic hypertrophy(BPH) frequent in aged men. Weight change during urination is usually measured to estimate the urinary flow rate by a load cell, but sensitive to any impacts against the bottom of the container, leading to unnecessary noise generation. Moreover, load cells are relatively expensive raising the production cost. The present study proposed a new technique, measuring hydraulic pressure on the bottom of the urine container to evaluate the urinary volume. Low cost pressure transducer enabled almost perfectly linear relationship between the urine volume and the hydraulic pressure. During both the simulated and human urination experiments, variance of the pressure signal was more than 50% smaller than the weight signal acquired by a load cell, which demonstrated that the impact noise was decreased to a great degree by pressure compared to weight measurement.

        체외수정시술로 출생한 쌍생아의 임상적 경과에 대한 비교 분석

        Purpose : To examine various neonatal outcomes and perinatal factors resulting from assisted reproduction compared to that of spontaneous conception. Methods : This is a retrospective study. The control cases were all twins of spontaneous conception born between periods from January 1995 to June 2000. The study cases were identified from twins conceived by assisted reproduction in the same time peried. A total of 460 sets of twins consisted of 250 twins of spontaneous conception and 156 twins of assisted reproduction were studied. The primary outcomes were neonatal morbidity and mortality and the secondary outcomes were perinatal factors including number, length and cost of hospitalization for the delivery. Results : No differences were seen in various neonatal factors including gestational age, birth weight and incidences of respiratory distress syndrome, patent ductus arteriosus, necrotizing enterocolitis, hyperbilirubinemia, sepsis, intraventricular hemorrhage and the length of hospitalizations. Lower one minute and five minute Apgar scores and frequently encountered electrolyte abnormalities were observed in neonates of assisted reproduction. In general, the second twin of assisted reproduction had increased incidences of respiratory distress syndrome, sepsis and necrotizing enterocolitis than the first twin. Increased frequencies of preterm labor, hospitalization and elective cesarean section were seen among mothers who underwent artifical conception. However, overall hospital costs in terms of mothers hospitalization for the delivery and neonates hospitalization did not show differences. Conclusion : Assisted reproduction twins had similar neonatal morbidities, mortalities and perinatal morbidities compared to those born by spontaneous conception.

        Corynebacterium glutamicum ATCC 13058 의 Histidine 생합성 유전자 hisC 의 클로닝과 염기서열 분석

        The chromosomal DNA fragments of Corynebacterium glutamicum ATCC 13058 containing genes involved in histidine biosynthesis were cloned in pBluescript SK(+). To investigate the structural organization and the regulation of these genes, recombinant clones complementing E. coli hisA, hisB, hisC, hisI and hisF mutants were isolated from genomic library. A recombinant plasmid containing a 5.6kb fragment of genomic DNA was able to complement E. coli hiA, hisB and hisF. But pCGH 13, carrying insert of 1.7kb complemented only E. coli hisC. The genetic data indicate that these genes are clustered in a small region f the chromosome. Nucleotide sequence analysis of the 1.7kb fragment of genomic DNA revealed one putative open reading frame which showed significant homology with the hisC gene of E. coli K-12 and Lacctococcus lactis.

        와류 현상을 이용하는 호흡기류센서

        The present study developed a new technique with no physical object on the flow stream but enabling the air flow measurement and easily incorporated with the devices for cardiopulmonary resuscitation(CPR) procedure. A turbulence chamber was formed in the middle of the respiratory tube by locally enlarging the cross-sectional area where the flow related turbulence was generated inducing energy loss which was in turn converted into pressure difference. The turbulence chamber was simply an empty enlarged air space, thus no physical object existed on the flow stream, but still the flow rate could be evaluated. Computer simulation demonstrated stable turbulence formation big enough to measure. Experiment was followed on the proto-type transducer, the results of which were within ${\pm}5%$ error compared to the simulation data. Both inspiratory and expiratory flows were obtained with symmetric measurement characteristics. Quadratic curve fitting provided excellent calibration formula with a correlation coefficient>0.999(P<0.0001) and the mean relative error<1%. The present results can be usefully applied to accurately monitor the air flow rate during CPR.

        흰쥐 부신 크로마핀 세포 칼슘통로 조절에 미치는 ATP의 효과

        ATP in quantity co-stored with neurotransmitters in the secretory vesicles of neurons, by being co-released with the neurotransmitters, takes an important role to modulate the stimulus-secretion response of neurotransmitters. Here, in this study, the modulatory effect of ATP was studied in $Ca^{2+}$ channels of cultured rat adrenal chromaffin cells to investigate the physiological role of ATP in neurons. The $Ca^{2+}$ channel current was recorded in a whole-cell patch clamp configuration, which was modulated by ATP. In 10 mM $Ba^{2+}$ bath solution, ATP treatment (0.1 mM) decreased the $Ba^{2+}$ current by an average of $36{\pm}6%$ (n=8), showing a dose-dependency within the range of $10^{-4}{\sim}10^{-1}mM$. The current was recovered by ATP washout, demonstrating its reversible pattern. This current blockade effect of ATP was disinhibited by a large prepulse up to +80 mV, since the $Ba^{2+}$ current increment was larger when treated with ATP ($37{\pm}5%$, n=11) compared to the control ($25{\pm}3%$, n=12, without ATP). The $Ba^{2+}$ current was recorded with $GTP{\gamma}S$, the non-hydrolyzable GTP analogue, to determine if the blocking effect of ATP was mediated by G-protein. The $Ba^{2+}$ current decreased down to 45% of control with $GTP{\gamma}S$. With a large prepulse (+80 mV), the current increment was $34{\pm}4%$ (n=19), which $25{\pm}3%$ (n=12) under control condition (without $GTP{\gamma}S$). The $Ba^{2+}$ current waveform was well fitted to a single-exponential curve for the control, while a double-exponential curve best fitted the current signal with ATP or $GTP{\gamma}S$. In other words, a slow activation component appeared with ATP or $GTP{\gamma}S$, which suggested that both ATP and $GTP{\gamma}S$ caused slower activation of $Ca^{2+}$ channels via the same mechanism. The results suggest that ATP may block the $Ca^{2+}$ channels by G-protein and this $Ca^{2+}$ channel blocking effect of ATP is important in autocrine (or paracrine) inhibition of adrenaline secretion in chromaffin cell.

      • 손상위험장기에 인접한 불규칙한 모양의 타겟 치료 시, 용적변조회전 방사선치료와 비동일평면상의 빔을 이용한 세기변조 방사선치료의 유용성 평가 및 비교

        Purpose: The purpose of this study was to compare volumetric modulated arc therapy(VMAT) with fixed-field intensity modulated radiation therapy(IMRT) using non-coplanar beam when the shape of target is irregular and the location is adjacent to organ at risk(OAR). Materials and Methods: The subjects of this study were a total of 6 patients who had radiation therapy for whole scalp(2 patients), partial scalp(2 patients), and whole ventricle(2 patients) by True Beam STX(Varian Medical Systems, USA). VMAT plans consisted of coplanar or non-coplanar arcs which can minimize the volume of OAR included in beamlets. All fixed-field IMRT plans consisted of non-coplanar beams using more than 2 angles of Couch. Results: The VMAT and IMRT plans were compared with regard to the maximum dose of both lens, both optic nerves, optic chiasm, and brain stem and the mean dose of both eyeballs and hippocampus. VMAT plans showed higher dose than ncIMRT plans at more than 6 of all OARs in every patient, and the ratio was from 1.1 times to 8.2 times. In case of total scalp and partial scalp, the volume of brain which received more than 20 Gy in the VMAT plans was 2 times larger than the volume in the ncIMRT plans. In case of whole ventricle, there was no significant difference. Target coverage was satisfied in both plans($PTV_{100%}=95%$). The maximum dose in target volume and required monitor unit(MU) of ncIMRT were higher than them of VMAT plans. Conclusion: Even though ncIMRT is less efficient than VMAT with regard to required MU and treatment time, the dose to OARs is much lower than VMAT and PTV Coverage is similar with VMAT. If the shape of target is irregular and location is adjacent to OAR, comparison VMAT plan with ncIMRT plan deserves to be considered.

        전통미술작품 감상 및 관련 통합활동이 유아의 미적태도와 자아존중감에 미치는 영향

        This study aims to identify influence of integrated activities based on traditional art work appreciation on children’s aesthetic attitude and self-esteem. For this purpose, an integrated activities and appreciation program were developed and implemented to subjects. The subjects were 46 children under 5 years old from D kindergarten located in Jungrang-gu, Seoul. The program was implemented to experimental group and intended to examine the influence of the program on their expressive artistic ability and self-esteem compared with a comparative group conducting free expressive art activity. The conclusions from the research results are as follows: First of all, integrated activities based on traditional art appreciation activity has a positive influence on improvement of children’s artistic expressive ability. Especially, the program has positive effects on use of color and description of space composition, subordinate factors of art expression. Secondly, while there was no statistically meaningful influence on artistic level. Thirdly, integrated activities based on traditional art appreciation activity has a positive influence on improvement of children’s self-esteem. In conclusion, it is possible to say that integrated activities based on traditional art appreciation activity has a positive influence on improvement of children’s artistic expressive ability and enhancement of their self-esteem. 본 연구는 전통미술작품감상 및 관련 통합활동이 유아의 미적태도와 자아존중감 향상에 도움이 될 것인지 알아보고자 하였다. 이를 위해 전통미술작품 감상 및 관련 통합 프로그램을 구성하였으며, 서울시에 중랑구 소재 D유치원의 만 5세 유아 46명을 대상으로 적용하였다. 유아는 프로그램을 적용한 실험집단과 종래 표현중심 미술활동을 한 비교집단으로 구분하였으며, 사전과 사후 검사를 통해 프로그램 효과를 검증하였다. 검증결과 프로그램은 유아의 미적태도 중 표상 능력 향상에 긍정적인 영향력을 보였으나, 예술성 향상에 미친 효과는 미미했다. 자아존중감의 경우 긍정적인 영향력을 갖는 것으로 나타났다. 이로써 전통미술작품 감상을 통한 통합활동 경험은 유아의 그림 표상 능력 증진과 자아존중감의 향상에 효과가 있다고 할 수 있겠다.

        남자 간호학생의 분만실 실습 체험

        본 연구는 간호학과 남학생을 대상으로 분만실 실습 체험의 의미를 파악하고자 수행하였다. 2012년 7월 20일부터 12월 28일까지 분만실 실습을 마친 후 한 달 이내의 4년제 간호대학 3학년 남학생 대상으로 자료가 포화될 때까지 진행하였 다. 수집된 8명의 질적 자료는 Colaizzi의 현상학적 연구방법을 적용하여 6단계로 분석하였다. 분석결과 30개의 주제(themes), 9개의 주제묶음(clusters) 및 3가지 범주(categories)를 도출하였다. 도출된 범주는 ‘여성 프라이버시가 중시되는 실습환경이기 때문에 위축됨’, ‘남자이기 때문에 불평등함’, ‘분만간호를 볼 수 있는 단 한 번의 기회임’으로 구조화되었다. 이를 통해 볼 때 연구 참여자들은 성 역할에 대한 사회적 인식에서 여성 간호 분야의 학습에 제한을 받으면서도 전문직 간호사의 태도를 가지고 실습에 임하고자 하였다. 따라서 성 역할에 대한 사회적 인식이 개선될 수 있도록 전문직 간호의 이미지를 신장시키 는 전략과, 전문직 간호사의 역할 정립을 위하여 간호교육현장에서 성 차별적인 교육방법을 개선하기 위한 다각적인 연구의 시행이 필요하다. The special education setting in nursing practicum is required for male nursing students to become professional registered nurses. Therefore, this study identifies how male nursing students find and recognize the meaning of the practicum in a delivery room. Eight male nursing students were selected randomly in junior and senior grade. They had finished delivery room practicum within a month and were interviewed from Jul. 20th, 2012 to Dec. 28th, 2012 until the narrative data were fully saturated. The phenomenological method of Colaizzi was used. Thirty themes, nine theme clusters, and three categories were identified. The three categories were frightened in the 'female privacy first' environment, sexual inequality for male students, and only one observation chance of the delivery process. From this study, male students did their best against the gender role and female-only field. A strategy to improve the image of professional nursing is required to enlighten the social conventions regarding the gender role. In addition, multilateral efforts are expected to eliminate the gender discrimination in the nursing education system to establish the correct role of professional registered nurses.
