        돌출성 유경 점막하 근종의 질식 근종절제술 : Twisting - off Method

        목적 : 돌출성 유경 점막하 근종을 가진 환자들에 있어 파악기구를 이용한 근종절제술의 효용성에 대해 연구하였다. 연구 방법 : 환자를 쇄석위로 자세를 취하게 한후 질내를 소독하고 자궁경부주변블록 (paracervical block)을 시행한 후 파악기구 (grasping insturment: kelley clamp, tenaculum, ring forcep)로 근종을 잡아 비틀어 (twistingoff)제거하였다. 출혈이 있는 경우 거즈나 탐폰을 삽입하였다. 결과 : 총 33명의 환자 모두에서 성공적으로 시술이 시행되었으며, 대부분의 환자에서 출혈은 경미하였고 9건에서 질내에 거즈 혹은 탐폰을 삽입하여 지혈하였으며, 1명은 다량의 출혈로 응급 자궁적출술을 시행하였다. 1주일 뒤 추적검사상 이상소견을 보이는 경우는 없었고 평균 18주간의 추적검사상 재발하거나 합병증이 발생한 경우는 없었다. 결론 : 자궁을 보존할 필요가 있는 돌출성 유경 점막하 근종 환자에서 twisting-off method는 간단하면서도 안전하게 시술될 수 있고, 환자의 이환률 감소, 비용절감 등에서도 효과적이다. Objective : To evaluate the efficacy of the twisting-off method in transvaginal removal of the pedunculated submucous myoma Methods : After paracervical block and vaginal dressing with Povidone iodine, the myoma was grasped at its widest diameter and twisted to avulse it from its attatchment. Several kinds of grasping instruments, ie. kelley clamp, tenaculum, ring forcep, was used. If there was bleeding from its detachment site, vaginal gauze or tampon was inserted for bleeding control. Results : The procedure was perfomed successfully on all 33 patients (100%). In most patients, bleeding was scanty in amount. But in one patient, emergency hysterectomy was performed because of massive uterine bleeding. The myoma varied greatly in size. Largest volume was 62.24 ㎤, and mean volume was 13.79 ㎤. Pathology revealed classic leiomyomas in most cases. Some showed endocervical polyp or endometrial polyp. Follow up examination was done, and its mean interval was eighteen weeks and no patient was recurred. Conclusion : The transvaginal removal of pedunculated submucous myoma utilizing grasping instruments may be an effective and safe procedure and the complication is minimal. Twisting-off method was may be a safe alternative to abdominal myomectomy and hysterectomy. Reproductive capacity can be preserved, also.

        비틀린 혈관 문합이 개존율에 미치는 영향

        Surgical errors in the length and orientation of a flap pedicle can result in rotation or tension on the pedicle vessels. When these errors occur, along with the microvascular repairs associated with free flap transfer, thromobsis and flap failure may be the outcome. The purpose of this study is to determine whether patency rate could be influenced by the twisting error in microvascular anastomosis. Rats femoal artery and vein were dissected in 1.5cm length(A) and 1.7cm length(B). Group A : The artery was transected and twisted 90, 180, 360, 900 degrees. Group B : The artery was transected, shortened by 2mm, and twisted 90, 180, 360 degrees. Later arterial anastomosis was performed as a simple interrupted suture. The results were as follows: 1. Compared with non twisted group, twisted group showed an increase of thrombosis. The effect was increased with the twisting degree. 2. Compared with non shortening group, shortened artery showed an increase of thrombosis. 3. A high correlation was found between patency test performed at 2 hours and 7 days after blood flow reestablishment. 4. The patency rate decreased much more due to tension in shortened artery with twisting. This new model offers a clinically relevant experimental method for evaluating therapies for the prevention of thrombosis-related free flap failure.

        Circumferential twisting during route B equal-channel angular pressing

        <P><B>Abstract</B></P> <P>Equal-channel angular pressing (ECAP) is a representative severe plastic deformation process to fabricate bulk nanostructured materials by repetitive passes. Typically, there are four basic processing routes for the repetitive passes: route B is the most beneficial for grain refinement and homogenization of microstructure. In this work, investigation on deformation during route B in ECAP was conducted using the finite element analysis. It was found that the route B could lead to the inhomogeneous equivalent strain along the longitudinal direction of the workpiece by an unusual phenomenon called “circumferential twisting”. The circumferential twisting is primarily attributed to the deformation inhomogeneity for the cross-section induced during the 1 st pass of ECAP. This phenomenon occurs remarkably for the large-scale ECAP process because the twisting angle per unit length is almost constant. In addition, the higher the strain hardening exponent of the material, the greater the sample twisting during ECAP by the corner gap effect.</P>

      • The Effects of Direction on the Maximal Grip, Pusing, Pulling, and Twisting Forces

        Objective: The aims of this study were to understand the effects of task direction on the output (pushing, pulling, and clockwise and counter clockwise twisting) and the maximal grip forces and to explore the relationship between the grip and the output forces. Background: Thoughtful handle design improves the productivity and prevents the work-related musculoskeletal disorders. Method: Eighteen healthy volunteers without any history of neuromuscular disorders were recruited. A custom-made handle and a load cell measured the grip force and the output for three seconds. Force measurements along the vertical, coronal and sagittal axes were randomly repeated three times and averaged. The data were analyzed using the repeated measures ANOVA for the effects of task direction on the output and the maximal grip forces and simple linear regression for the relationship between the grip and the output forces. Results: There was no significant effect of direction on the output and grip forces. The pulling force was greatest and the pushing force was smallest in all directions. The order of grip force was pushing, pulling, clockwise twisting, and counter clockwise twisting in all directions. The output and grip forces were highly correlated in all directions. The regression coefficient of grip force was greatest in pulling and smallest in clockwise twisting. Conclusion: Findings of this study suggests that a smart handle design can improve the productivity more in pushing and pulling than twisting tools. Application: It can be valuable information for product designers to develop more productive hand tools and to prevent the musculoskeletal disorders in the upper extremities.

        Adhesion Tunable Bio-Inspired Dry Adhesives by Twisting

        We report on a control method by twisting the adhesion property of a bio-inspired dry adhesive with mushroom-shaped micro structures. The fabricated artificial dry adhesive has a strong adhesive force in all directions; however, this adhesive is considerably strong to detach simply for reversible usages. This study proposes twisting a mushroom-shaped artificial dry adhesive for easy a detachment method that makes a pull-off force decrease from 14 N/cm2 to nearly 0. In addition, the optimal process parameters of the twisting detachment are studied by experimentally comparing the pull-off force, net torque, and required time depending on the thickness of the adhesive and angular velocity of twisting. Result indicates that thin adhesive (1.2 mm) and high angular velocity (0.0436 rad/s) provide a good detachment performance. For the feasible application of the proposed method, a glass transportation robot was used for demonstration and a glass substrate was transported several times.

        Effect of Task Direction on the Maximal Pushing, Pulling, Twisting, and Grip Forces

        Objective:The aims of this study are to understand the effects of task (pushing, pulling, and clockwise and counter clockwise twisting) direction on the maximal output and their grip forces and to explore the relationship between the maximal output and the grip forces. Background: Knowing the normative maximal grip force is not enough to design a good hand tool. The industrial designers should understand the required grip forces in various motions toward a specific direction to make an effective and efficient hand tool. Method: Eighteen healthy volunteers participated in the series of isometric maximal output force tests. A custom-made force measuring equipment collected the output and the grip forces for three seconds. Force measurements along the vertical, coronal and sagittal axes were randomly repeated three times. Results: The pulling was strongest and the pushing was weakest in all directions. The effect of motion on the output forces varied in different directions. The corresponding grip force increased in the order of pushing, pulling, clockwise twisting, and counter clockwise twisting in all directions. The maximal output and their grip forces were highly correlated but the relationship was affected by motion and direction. The regression coefficient was greatest in pulling and smallest in clockwise twisting. Conclusion: The effect of motion on the output forces varied in different directions. Application: Findings of this study can be valuable information for industrial designers to develop more productive hand tools and work stations to help preventing the musculoskeletal disorders at work.

        Twisting된 Bi-2223 선재의 제조, 미세조직 관찰 및 임계 특성평가

        We fabricated Bi-2223 multi-filaments superconductor tape and evaluated the effect of twisting on the microstructural evolution and critical current. Twist pitches of the tapes are in the range of 70- 8mm and uniformly deformed. It was observed that grain size and the degree of texture decreased as decreasing pitch probably due to the formation of the irregular interface between Ag and filaments. In addition critical current of the tapes decreased to 6.5 A with decreasing pitch to 8mm, showing 48% of degradation compared to the untwisted tapes decreased to 6.5 A with decreasing pitch to 8mm, showing 48% of degradation compared to the untwisted tape(12.5 A). These reduction of critical current may be related to the interface irregularity smaller grain size worse texture and the presence of cracks due to the induced strain during twisting processing.

      • 비틀림-굽힘 혼합 고유 진동 모드의 분리에 대한 고찰

        If it is required that some certain structural resonance modes should not be excited in a specified frequency range and therefore, the relevant resonant frequencies are usually increased beyond the frequency range. However, in some cases, it also turns out to be a challenge that is difficult to achieve. This work proposes a new method to separate mixed vibration modes, specifically, mixed twisting and bending modes. For the purpose, the non-zero value of a mass product of intertia due to intrinsic asymmetric mass distributions of a structure in consideration is reduced. Analyses with a simple equation and a finite element model are employed to check the validity of the proposed method.

        부당승환계약 방지에 대한 비교법적 연구

        As life insurance market becomes saturated, replacement in life insurance tends to increase. Studies and reports, however, on replacement (churning or twisting) problems are hard to find. This paper attempts to examine cross-country replacement regulation comparison and to draw implications of current replacement regulations of Korea. According to German replacement regulation, the existing insurers argue that the replacing insurers improperly replace their life insurance, the accused insurers has to prove within a month that the accusation is not true. UK’s insurance suitability regulation includes detailed decision criteria on the life insurance suitability test in order to prevent producers’ subjective interpretation of the suitability. Most replacement laws of US states articulately stipulate the definition of replacement, the duties of producers, the replacing insurers and the existing insurers. The Codes of Conducts of Financial Service Councils of Australia adopt ‘the three-year claw back period’ clause to deter churning. Based on laws and regulations of these countries, current replacement laws of Korea appear not specific enough to deter churning and twisting. In oder to improve current replacement laws of Korea, this study suggests introducing the definition clause of replacement, adopting clauses the duties of producers, the existing insurers and the replacing insurers and finally imposing penalties on producers for early life insurance lapses like ‘the three year claw back period’clause. 생명보험 시장이 포화되면서 부당승환으로 인한 소비자 피해는 증가할 수 있다. 하지만 부당승환에 대한 문제나 법제에 대한 연구는 거의 없는 실정이다. 이 연구는 부당승환에 대한 주요국 법제를 비교⋅연구하여 국내 제도의 개선방안을 찾고자 한다. 독일은 승환보험사가 부당승환을 했다고 피승환보험사가 주장하면, 승환보험사는 1개월 내에 그것이 사실이 아님을 증명해야 한다. 영국은 적합성 요건에 내용적, 형식적 판단기준을 구체적으로 마련하여 판매자가 이를 주관적으로 해석할 여지를 원천적으로 방지하고 있다. 미국 모델법은 승환의 정의, 판매자, 승환보험사 및 피승환보험사의 책임을 구체적으로 규정하고 있다. 호주는 ‘3년 환수기간 조항’으로 재무상담사의 책임을 엄격하게 묻고 있다. 주요국들의 부당승환 법제는 우리나라에 비해 구체적이고 엄격하다는 차이가 있다. 부당승환 방지를 위한 제도 개선 방안으로는 (부당)승환에 대한 정의조항을 도입하고, 판매자, 승환보험사 및 피승환보험사에 대한 의무조항을 신설하고 나아가 조기 해지 계약에 대해서는 수수료를 환수하는 패널티를 부과하는 방안을 검토할 필요가 있다.

        뮤지컬 영화를 통해 본 장르 분석 연구

        The film ‘Chicago’ follows the constructive background of violent dramaturgy of crime, violence and murder that are easily found in the order genre films such as Western, Gangster, and Film Noir. However, the film draws a conclusion of integration genre for delivering the message in the end. After Velma and Roxie get out from the jail, a declined vaudevillian Velma suggests Roxie who is down on her luck after she got out from the jail to do the collaboration act with her. This return to the female-dominant reality indicates the unification of contradictory cause and salvation. Velma and Roxie return to the world of show business with the enormous success that they have been always longing to be reconciling and adapting with their new lives and livening up with challenge and passion. In a conclusion, Rob Marshall’s musical film Chicago is the challenge of a genre in order to emphasize the message it tries to deliver to the audience by applying the order genre to the integration genre. This is so called the genre twisting.
