        Stability and Effectiveness of Laser and Liquiband Treatments in the Rat Model of Oral Ulcer

        Background and ObjectivesThis study was undertaken to assess the stability and efficacy of laser therapy (808 nm), Liquiband (a commercial topical skin adhesive product), and a combination treatment, for application in oral ulcers. Materials and MethodsThe oral ulcer rat animal model was used to determine the efficacy of photobiomodulation, Liquiband, or combination therapy. Ulcers were induced by injecting 60% acetic acid in the oral mucosa. Three days after ulcer confirmation, the chemically induced ulcers were treated with either laser (808 nm), Liquiband, or a combination of both, every two days for 8 days (4 sessions). Combination therapy was performed by first treating with laser followed by application of the Liquiband. Reduction in ulcer area size was subsequently determined, and animals were sacrificed at 5 and 10 days after the last administration, for histological observation of the extracted oral ulcer tissue samples. ResultsIn this study, treatment with either laser (808 nm) or commercial Liquiband product as well as the combined treatment significantly enhanced oral ulcer healing in the rat animal model. The effect of laser treatment is mainly attributed to collagen synthesis, whereas application of the Liquiband promotes vascularization and formation of granulation tissue. Our results indicate that further optimization of the combined therapy could synergistically and significantly enhance oral ulcer healing. ConclusionLaser or Liquiband treatment of oral ulcer in the rat animal model exert different outcomes, but both methods confirm significant effects relative to the untreated group. The combined treatment group with laser and Liquiband showed marked oral ulcer healing, but further optimization is required to get highly significant results and establish the potential synergistic effect of the combination therapy, as compared to the individual enhancing effects of healing oral ulcers.

        Syphilis is an infective disease mainly transmitted through sexual contact caused by Treponema pallidum. The natural course of syphilis infection is divided into primary, secondary and tertiary stage. Primary stage usually manifests as syphilitic chancre, which is ulcerative lesion developed at the inoculation site after incubation period. Chancre usually develops in the genital area, but can involve any sites of the human body. Currently, increasing diversity in sexual activity can lead to extragenital manifestation of chancre. A 53-year-old man presented with multiple asymptomatic oral ulcers located in upper and lower lips for 4 months. Skin lesion of other site including genital area was not apparent. Histopathological examination of the lips revealed plasma cell infiltration and vascular proliferation with endothelial swelling in the dermis. Spirochetes were demonstrated by Warthin starry stain. Venereal disease research laboratory (VDRL) test was positive with a titer of 1:32 and Treponema pallidum haemagglutination (TPHA) test, fluorescent treponema antibody IgM and IgG were reactive. From these findings, he was diagnosed with primary syphilis manifested as oral ulcers. Thus, clinicians should take primary syphilis into consideration in diagnosing ulcerative lesions of oral area.

        Curcumin Blocks Naproxen-Induced Gastric Antral Ulcerations through Inhibition of Lipid Peroxidation and Activation of Enzymatic Scavengers in Rats

        Curcumin is a polyphenol derived from the plant Curcuma longa, which is used for the treatment of diseases associated with oxidative stress and inflammation. The present study was undertaken to determine the protective effect of curcumin against naproxen-induced gastric antral ulcerations in rats. Different doses (10, 50, and 100 mg/kg) of curcumin or vehicle (curcumin, 0 mg/kg) were pretreated for 3 days by oral gavage, and then gastric mucosal lesions were caused by 80 mg/kg naproxen applied for 3 days. Curcumin significantly inhibited the naproxen-induced gastric antral ulcer area and lipid peroxidation in a dose-dependent manner. In addition, curcumin markedly increased activities of radical scavenging enzymes, such as superoxide dismutase (SOD), catalase, and glutathione peroxidase in a dose-dependent manner. Specifically, 100 mg/kg curcumin completely protected the gastric mucosa against the loss in the enzyme, resulting in a drastic increase of activities of radical scavenging enzymes up to more than the level of untreated normal rats. Histological examination obviously showed that curcumin prevents naproxen-induced gastric antral ulceration as a result of direct protection of the gastric mucosa. These results suggest that curcumin blocks naproxen-induced gastric antral ulcerations through prevention of lipid peroxidation and activation of radical scavenging enzymes, and it may offer a potential remedy of gastric antral ulcerations.

        베체트병의 조기 진단

        베체트병은 전신적 혈관염으로 여러 장기에 걸쳐 나타날 수 있으며, 현재로선 확진을 위한 검사 방법이 없으므로 병의 진단은 임상적 증상에 기초하여 내려진다. 구강 및 외음부의 재발성 궤양, 피부 병소, 안구 병소, 및 이상초과민검사(pathergy test)에 대한 반응을 기초로 진단을 내리며, 이 외에도 위장관과 중추신경계에 증상이 나타날 수 있다. 이 중 재발성 구강 궤양은 대부분의 베체트병 환자에서 나타나며, 많은 경우 베체트병의 첫번째 임상 증상으로 발현되기 때문에 치과 영역은 베체트병의 조기 진단에 있어서 중요한 위치에 있다. 본 증례에서 6세 여환은 반복되어 나타나는 재발성 구강 궤양으로 인한 동통 및 섭식 장애를 주소로 본원 소아치과에 내원하였다. 문진 및 병력 채취, 그리고 임상 검사를 통해 치과 영역에서 베체트병을 조기 진단할 수 있었고, 추후 환자는 소아과, 피부과, 소화기내과, 안과와의 협의 진료 하에 베체트병으로 확진되었다. 치과에선 환자의 동통을 완화시키고 섭식을 돕기 위해 도포마취제를 처방하였으며, 추후 베체트병의 확진 하에 그에 따른 적절한 의과적 치료를 시행할 수 있었기에 보고하는 바이다. Behcet's disease(BD) is a multisystem vasculitis that almost any organ in the body may be involved. Its diagnosis bases on clinical criteria in the absence of any specific laboratory test to confirm it. The clinical criteria proposed for the diagnosis for BD include recurrent oral and genital ulceration, skin lesion, ocular changes, and positive response to pathergy test. Besides these essential features, BD may manifest itself on the gastrointestinal tract, central nervous system, and other systemic involvements. Among these clinical features, recurrent oral ulcers are present in most BD patients, often as the initial symptom, preceding other manifestations by many years. Therefore, dentists are in the important position for the early detection and diagnosis of BD. In this case report, we are presenting a case of 6 year-old girl, who came to the pediatric dentistry department for evaluation and treatment of pain and dysphagia caused by severe recurrent oral ulcers. Through taking detailed dental and medical history and clinical examination, we reported the possibility of BD to the medical team and prescribed topical lidocaine to relief pain during swallowing. The medical team, which involved pediatrics, ophthalmology, dermatology, and gastroenterology, finally confirmed BD through various examinations. Detection of the possibility of BD in the dental field helped early diagnosis and proper management by the medical team.

        Crohn`s disease is a chronic inflammatory bowel disease that can involve the whole gastrointestinal tract. The orofacial manifestation of Crohn`s disease, which is rare, can develop irrespective of intestinal involvement. These orofacial lesions are often misdiagnosed as simple oral ulcers. Corticosteroids are the mainstay of therapy for orofacial Crohn`s disease. However, infliximab, the chimeric monoclonal antibody to tumor necrosis factor-α, is now considered as a primary treatment because of the disease`s relatively high rate of steroid resistance. We present a case of deep oral ulcer and periorbital swelling in a 65-year-old woman. She was diagnosed with intestinal Crohn`s disease 7 years ago, which was in remission after treatment with an immunosuppressive agent (azathioprine). The patient was given the diagnosed with orofacial Crohn`s disease and successfully treated with infliximab. (Korean J Gastroenterol 2012;59:437-440)

        구강점막의 궤양성 병소

        An ulcer is defined as loss of epithelium. Although many oral ulcers have similar clinical appearances, their etiologies encompass many disorders, including trauma, infection, immunologic disease, and malignant oral cancer. Oral squamous cell carcinoma(SCC) occupying about 90% of oral cancer, usually manifests as unhealed ulcer over 2 weeks. Oral SCC can metastasize to the cervical neck lymph node, and therefore the surgical therapeutic modality for oral SCC could encompass the neck node dissection as well as wide excision for primary lesions, which should leave the post-operative complication of functional damage like dysphagia and facial deformity. Therefore, it is important to discriminate oral SCC from other ulcerative conditions to make a prompt management. The knowledge for the pathogenesis of the ulcerative lesions could help the clinicians to understand the differences of clinical features and to practice an appropriate therapeutics.

        대아프타성 구강 궤양에 대한 한방 치료 증례 보고

        Objective: This case study reports on the results of the Korean medical treatment of a major aphthous oral ulcer patient. Methods: A 19-year-old male Korean patient with a major aphthous oral ulcer received acupuncture, herbal medicine, and moxibustion for three weeks in a hospital. Results: After treatment, changes were observed in the numeric rating scale (NRS) from 8 to 5, World Health Organization oral toxicity scale (WHO OTS) from grades 3 to 2, oral perception guide from 11 to 15, and Oral Health Impact Profile-14 (OHIP-14) from 19 to 34. Conclusion: It is worth examining the progress of Korean medical treatment for a major aphthous oral ulcer patient.

        Pemphigus vulgaris is a rin, chronic intraepidermal bullous disease with potentially fatal outcome. Oral lesions precede skin lesions in at least $70\%$ of cases, and in cutaneous disease, concomitant oral lesions are encountered in $90\%$ of patients. This disorder involve the skin and mucous membranes, especially the oral and pharyngeal mucosa, but may also involve the nasal, oropharyngeal, laryngeal and esophageal mucosa. Oral lesions are initially vesicobullous but rapidly rupture, leaving a painful erosion that shows little tendency to heal. Pemphigus vulgaris affecting the oral mucosa is still diagnosed only after considerable delay, because oral ulceration in common, and clinicians believed the lesions to be caused by more common conditions such as recurrent aphthous stomatitis rather than a rare disorder such as pemphigus vulgaris. The definitive diagnosis of pemphigus vulgaris should be undertaken as early as possible, so that treatment can be started at an earl·y stage. Because of the presence of nonspecific oral ulcer, high degree of suspicion is often required to ultimately make the diagnosis of pemphigus vulgaris and then we report a case of pemphigus vulgaris with a literature review.

        Oral Symptoms Related to Vitamin B<sub>12</sub> Deficiency in a Patient with Crohn's Disease

        Crohn's disease is an inflammatory bowel disease which affects whole gastrointestinal tract from mouth to anus. Crohn's disease may present both oral manifestation and gastrointestinal symptom-abdominal pain, diarrhea, weight loss, anorexia, fever, and growth failure. The prevalence rate of oral manifestation is approximately between 0.5% and 20%. The oral lesion could be the first sign of Crohn's disease. We present a case of Crohn's disease in a patient who did not show typical oral manifestations but had nonspecific aphthous like ulceration and burning sensation for many years. Through this case, we suggest approaches for the diagnosis and treatment of the oral lesion of Crohn's disease.
