전위가 심한 부유견의 내고정술 후 임상적, 기능적 결과

목적: 동측의 쇄골 골절 또는 견봉 쇄골 관절 탈구가 동반된 전위된 견갑골 경부 골절은 고에너지 손상이며 불안정하다. 이런 부유견에 대한 쇄골 또는 견봉 쇄골 관절 탈구에 금속내고정술 및 견갑골 경부의 금속판 내고정술을 시행한 11예에 대해 추시 결과를 보고하고자 한다. 대상 및 방법: 다기관 연구로 1997년 8월부터 2004년 7월까지 쇄골 골절 또는 견봉 쇄골 관절 탈구가 동반된 견갑골 경부 골절을 대상으로 하였다. 견갑골 경부 골절은 수평 전위 (translational displacement) 25 mm 이상, 각형성 전위 45도 이상의 예를 대상으로 하였으며, 견갑골 경부 골절은 3.5 mm 재건 금속판 내고정술을 시행하였고 쇄골 골절 또는 견봉 쇄골 관절 탈구에 대한 내고정술을 각각 시행하였다. 9개월 이상 추시가 가능하였던 11예를 대상으로 하였다. 결과: 모든 예에서 골유합을 얻었다. 미국 견주관절학회의 기능 평가법 (ASES) 기능 평가 점수상 평균 89.2 (75∼95)점을 있었다. Rowe기능 평가 점수상 평균 89.1 (75∼100)점을 얻었다. 합병증으로 1예에서 외회전 근력약화가 있었다. 결론: 고에너지 손상에 의한 전위가 심한 부유견은 견갑골 경부와 쇄골 또는 견봉 쇄골 관절 탈구에 대하여 각각 내고정을 시행 후 좋은 결과를 보였다. Purpose: To evaluate the follow-up result of 11 cases that were operated with internal fixation of scapular neck and internal fixation of clavicle or acromioclavicular dislocation for severely displaced floating shoulder which was high energy injury and unstable. Materials and Methods: We examined the scapular neck fracture with clavicle fracture or acromioclavicular joint dislocation by multidisciplinary research from August 1997 to July 2004. The scapular neck fractures were operated in the case of translational displacement of more than 25 mm and angular displacement of more than 45 degrees with 3.5 mm reconstruction plate fixation and internal fixation for clavicle fracture or acromioclavicular joint perpormed simultaneously. And we evaluated 11 cases that can be followed up for more than 9 months. Results: We achieved bony union in all cases. In ASES functional score, we got average 89.2 (75∼95) points. In Rowe functional score, we got average 89.1 (75∼100) points. In complication, there was external rotation weakness in 1 case. Conclusion: In severely displaced floating shoulder due to high energy injury, we got good clinical and functional result after internal fixation for scapular neck and clavicle or acromioclavicular joint.

Triple Disruption of the Superior Shoulder Suspensory Complex

상부 견갑 현수 복합체(Superior shoulder suspensory complex)의 3중 붕괴는 몇몇 저자들에 의해서만 보고된 극히 드문 손상이다. 저자들은 오구돌기, 견갑극 골절 및 견봉-쇄골 관절의 탈구가 동반된 상부 견갑 현수 복합체의 3중 붕괴 환자를 경험하였기에 보고하고자 한다. 본 환자의 경우 견봉-상완 및 오구-상완 간격을 유지하기 위하여 관혈적 정복 및 내고정을 시행하여 상부 견갑 현수 복합체의 3중 붕괴 모두를 해부학적으로 복원하여 치료하였다. 술 후 6개월에 증상 없이 관절운동범위를 모두 회복하였다. 최종 추시인 5년에는 상부 견갑 현수 복합체의 기능이 완전히 회복된 것을 확인할 수 있었으며, 환자는 수술 결과에 대하여 크게 만족하였다. A triple disruption of the SSSC, an extremely rare injury, has been reported by only a few authors. We present a patient who had sustained a triple disruption of the SSSC: coracoid and scapular spine fractures, and an acromioclavicular joint separation. Treatment consisted of an anatomical restoration of the SSSC, with maintenance of the acromiohumeral and coracohumeral distances; this was achieved by open reduction and internal fixation of the fractures and of the separation. Six months after surgery, the injured shoulder was asymptomatic, with full range of motion. Five years after surgery, at the final follow-up, the function of the SSSC had been restored to the patients complete satisfaction.
