Strain SPF4211, having hyaluronidase (HAase) inhibition activity, was isolated from P. davidiana (Carriere) Franch fruit (PrDF) sugar extract. The phenotypic and biochemical properties based on 16S rDNA sequencing and an API 50 CHB kit suggested that the organism was B. subtilis. To optimize the HAase inhibition activity of PrDF extract by fermentation of strain SPF4211, a central composite design (CCD) was introduced based on three variables: concentration of PrDF extract (X1: 1-5%), amount of starter culture (X2: 1-5%), and fermentation time (X3: 0-7 days). The experimental data were fitted with quadratic regression equations, and the accuracy of the equations was analyzed by ANOVA. The statistical model predicted the highest HAase inhibition activity of 37.936% under the optimal conditions of X1 = 1%, X2 = 2.53%, and X3 = 7 days. The optimized conditions were validated by observation of an actual HAase inhibition activity of 38.367% from extract of PrDF fermented by SPF4211. These results agree well with the predicted model value.

Postherpetic neuralgia (PHN) is one of the most difficult pain syndromes to treat. Invasive treatments may be considered when patients fail to obtain adequate pain relief from noninvasive treatment approaches. Here, we present three cases of PHN in the mandibular branch treated with ultrasound-assisted mental nerve block and pulsed radiofrequency treatment. None of the patients had adequate pain relief from the medical therapy, so we performed the mental nerve block on the affected side under ultrasound assistance. Two patients showed satisfactory pain relief continuously over 12 months without any further interventions, whereas one patient only had short-term pain relief. For the patient had short-term pain relief we performed pulsed radiofrequency treatment (PRFT) on the left mental nerve under ultrasound assistance. After PRFT, the patient had adequate pain relief for 6 months and there was no need for further management. (Korean J Pain 2014; 27: 81-85)

A vertebro-vertebral arteriovenous fistula is an abnormal communication between the vertebral artery and adjacent venous structures. Trauma is the most common cause of vertebral arteriovenous fistulas; however, the fistulas can also occur spontaneously. We herein report a case of a traumatic vertebro-vertebral arteriovenous fistula that occurred following the application of oriental acupuncture in the posterior neck region. A 64-yearold previously healthy female patient took over-the-counter medicine for cervical pain that occurred several months before the acupuncture session but showed no improvement. She had undergone oriental acupuncture treatment in the posterior lower neck region 1 month before visiting our hospital. After the treatment, she gradually developed tinnitus, accompanied by dizziness. Abnormal aneurysmal dilated vessels were observed in the right vertebral artery on computed tomography angiography. The patient was immediately admitted and underwent diagnostic angiography. She subsequently underwent stent-assisted coil embolization, which gradually resolved her dizziness and tinnitus.

Copper(II) acetate spectrophotometry method (CASM) was used for the rapid and convenient determination of cation exchange capacity (CEC) in soils. This method is composed of a single-step exchange reaction that adsorbs copper and is measured through spectrophotometry. The CEC of 16 Korean soils were measured using 1M ammonium acetate method (AAM) and the CASM. The CEC values determined by CASM and AAM were not significantly different, and were highly correlated (r = 0.966**). Due to the convenience, cost effectiveness, and time saving analysis of CASM, this method is recommended for most soil laboratories to measure CEC in Korean soils. However, CASM may not be applicable for soils that have a much higher CEC (greater than 20 cmolc kg<SUP>-1</SUP>).

Objective: The aims of the clinical study was to reveal the desensitization effect for the dentifrice with calcium glycerophosphate. Methods: A total of 90 subjects were allocated into three groups: 30 for the experimental group, 30 for the comparison group, and 30 for the controlled group. Each group used dentifrices for 2 weeks. The calculus index, papillary marginal attached gingival index, gingival index, patient hygiene performance index, plaque index, halitosis by B&B checker, hyperesthesia by Airblow and electrical pulp test were evaluated 4 times. Results: Hyperesthesia by Airblow decreased in the experimental group (from 6.20 to 5.53 at week 1, 4.73 at week 2, 4.23 at week 4), comparison group (from 6.17 to 5.83 at week 1, 5.10 at week 2, 5.03 at week 4), and controlled group (from 5.97 to 5.83 at week 1, 5.67 at week 2, 5.53 at week 4). For the experimental group, statistically significant efficacy was presented from week 1, whereas it was presented from week 2 for the comparison group. Efficacy presented more rapidly in the experimental group, relieving hyperesthesia and was statistically significant at weeks 2 and 4. Conclusion: According to those results, glycerophosphate calcium included in dentifrice may lead to more effective and rapid effects in relieving hyperesthesia, compared with previous dentifrices.

Ultrasonographic measurement of optic nerve sheath diameter (ONSD) has been validated to detect and monitor the increased intracranial pressure (IICP) in neurosurgical field. Especially, the ONSD has been known to reflect well the ICP in case of intracranial hemorrhage (ICH) occurring in the anterior circulation of intracranial vascular system, however it has not been well elucidated the role of ONSD in the posterior circulating vascular problems. A 43-year-old man presented with a subarachnoid hemorrhage with intraventricular hemorrhage due to rupture of dissecting aneurysm at right vertebral artery, and immediately performed the stent-assisted coil embolization. Two day after coil embolization, he became nearly alert without definite neurologic deficits. The ONSD was reduced from 5.8 mm to 5.5 mm. The 10th postoperative days, the patient suddenly changed into stuporous mentality due to rebleeding of aneurysm. The ONSD was enlarged to 6.7 mm. We report a case of intimate relationship between ONSD and IICP after rupture of dissecting aneurysm located in posterior circulation.
