      • 거대적아구성빈혈의 임상적 고찰

        Megaloblastic anemias are disorders caused by impaired DNA synthesis and chara-cterized by the presence of megaloblastic cells. Clinical data on these disorders in Korea are limited. The aim of this study was to analyze clinical characteristics of megaloblastic anemias caused by vitamin B12 deficiency. We identified 40 cases who were diagnosed as megaloblastic anemia from January 1993 to December 2002 in a retrospective study on the basis of chart review. We investigated the annual distribution of diagnosed patients, clinical manifestations and response to treatment. 19 cases were complicated with a previous total gastrectomy(gastrectomy group) and 21 cases had no history of surgical operation(non-gastrectomy group). The most common presenting complaint was generalized weakness. The median age at diagnosis was 7(range: 38-79) years in non-gastrectomy group. The mean pos-toperative duration was 7(range: 4-25)years in group of total gastrectomy and mean level of serum vitamin B12 was lower than 100 pg/mL in both group. Peripheral blood revealed thrombocy-topenia, macrocytic anemia and hypersegmented neutrophil in all cases. After treatment of pareliteral vitamin B12, Their clinical symptoms and hematologic findings improved except two patients who had bone marrow with lower cellularity. Cobalamin replacement after total gastrectomy is still missing. Diagnoses are made too late in a population of patients, resulting in irreversible neurological sequale, and we suggest that Pernicious anemia seems not to be very rare in Korea.

      • WWW 기반의 원격교육시스템 설계 및 구현

        The purpose of this paper is to develope a distance education that would be a courseware for a specific object, which can be available for student to learn the necessary knowledge or function, with no restriction of time or place, using WWW and Oracle DBMS. The distance education was designed in this paper consists of bulletin board, subject information, lecturing schedule, online instructions, questions and answers, submit and confirmation of reports, evaluation of lecturing, reference sites, chatting and discussion room, and data bank. The distance education coupled by WWW and database will change the educational form from one-direction to bi-direction between a teacher and the students. This can also provide a multimedia information to the students through WWW environment with convenient user interface. Those benefits made it possible to offer animated audio-visual materials and search for the information concerned through a hypertext technique. Besides, various kinds of informations were stored at database to increase an effectiveness in managing data. Since both teacher and students, are easy to add new information using the process of instruction to the database, the amount of the stored information in the distance education can be expanded effectively.

        친환경농업 시범마을에 대한 환경영향평가

        1999년부터 2001년까지 3년간 친환경농업 시범마을에 적용되고 있는 INM (Integrated Nutrient Management) and IPM (Integrated Pest Management) 실천에 따른 추진효과를 분석하기 위하여 시범마을로 지정된 옥천과 양평지역의 농가 포장에서 시비량, 양분유실 및 생태계 변화 등을 조사 분석하였다. 3요소 시비량은 토양검정시비로 인하여 농가관행시비와 비교하여 28.6~39.4% 절감할 수 있었으며, 쌀 수량도 시비량이 비교적 적은 토양검정 시비구에서 3~10% 증수하였다. 영농기간동안 토양 침투수중 NO₃-N 농도는 농가관행 시비구 2.9 ㎎/L에 비하여 토양검정 시비구는 1.5 ㎎/L로 감소하였으며, 시범마을 논으로 유입되는 관개수의 COD, NH₄-N, NO₃-N 농도는 각각 15.0, 0.67, 1.39 ㎎/L, 시범마을 논에서 배출된 용수는 각각 12.4, 0.29, 2.42 ㎎g/L로 감소되는 경향이었다. 처리별 수서곤충, 환형동물, 패류 및 감각류 등의 개체수는 농가관행구 100에 대하여 토양검정 시비구 293, 토양검정과 개량제 혼용구 276, 무비구 90으로 토양검정 시비구에서 많았으며 수서 무척추 동물은 관행농업을 수행하고 있는 지역을 100으로 볼 때 친환경농업마을은 242와 443으로 INM과 IPM을 실천하는 시범마을에서 생물종이 다양하고 그 개체수가 많은 것으로 나타났다. 친환경농업 시범마을 조성 후 작물 추천시비량 준수와 적기 예찰을 통하여 필요이상 사용되는 비료와 농약을 절감하여 생태계의 개선과 토양이 건전화된 것으로 평가되었다. In order to preserve the soundness of ago-ecosystem and products safety in agriculture, this study was evaluated environmental impact in relation to the INM (Integrated Nutrient Management) and IPM (Integrated Pest Management) at paddy fields at Ok-Chun and Yang-Pyung region. By introduction of INM with the application of BB(bulk-blanded) fertilizer based on soil analysis and EM, the application rate of fertilizer was reduced to about 28.6-39.4% and the yield of brown rice was increased to about 3-10% compared to conventional practices. The concentrations of COD, NH4-N, and NO3-N in irrigation water flowed to the environmental-friendly agriculture practices were 15.0, 0.67, and 1.39 ㎎/L, respectively. The concentrations of COD, NH₄-N, and NO₃-N from paddy fields in drainage water were 12.4, 0.29, and 2.42 ㎎/L, respectively. The total number of the freshwater invertebrates was higher in field treated with fektilhtion by prescription with soil testing. Also, the population density of aquatic insects was higher than the other fields at both demonstration villages. In conclusion, it was possible to reduce the amount of fertilizer and agricultural chemicals application, and increase the yield of rice by application of the environmental-friendly agriculture practices.

      • 동일한 국제예후지표(International Prognostic Index)를 나타내는 미만성 대형 B-세포 림프종에서 Bcl-2와 p16의 임상적 중요성

        In Korea, malignant lymphoma is a common cancer, comprising about 2.7% of all malignant neoplasm. Diffuse large B cell lymphoma is the most common lymphoma, representing about 50% of all Non-Hodgkin's lymphoma. Diffuse large B-cell lymphoma is usually considered as heterogeneous group of neoplasms rather than a single clinicopathological entity. Clinical prognostic systems, including the International Prognostic Index (IPI), although useful to assess overall prognosis, embrace patients with heterogeneous prognoses. But International Prognostic Index scoring system is not sufficiently predict the prognosis. It is likely that the prognostic assessment of patients with diffuse large B-cell lymphomamight be improved by using biological features. Bcl-2 protein and p16 protein expression is recognized as useful biologic markers predicting the prognosis of patients with diffuse large B-cell lymphoma. To determine the clinical significance and prognostic value of bcl-2 and p16 proteins expression patterns, we studied 18 patients with de novo DLBL, whose archival pathology specimen were available for immunohistochemistry studies, atChungnam National University Hospital from September 1992 to December 2000. Archival specimens from each patient were immunostained with respective antibodies for bcl-2, p16. The results are as follows; 1) The median age was 54(rage : 37-69). There were 12 male patients(66.7%) and 6 female patients(33.3%) The 'B' symptom was abscentin all patients. The stages were as follows : Ⅰ, 2 patient(11.1%), Ⅱ, 10 patient(55.6%), Ⅲ, 4patient(22.2%) and Ⅳ, 2patient(11.1%). 3 patients(16.8%) had the elevated LDH level, 14 patients(77.8%) had the normal LDH level and 1 patients(5.6%) was not identified the LDH level. 2 patients(11.1%) had the bulky disease and 16 patients(88.7%) had no bulky diease. The distribution of ECOG status were O, 2 patients(11.1%c), 1, 14patients(77.8%) and 2, 2patients(11.1%). 2) Theimmunohistochemistry results are as follows bcl-2:+,10 patients(55.6%), bcl-2:-, 8patinets(44.4%), p16:+,3 patients(16.7%), p16:-, 15patients(83.3%) 3) After a median follow UP durations of 67 months, the median survival time was 57 months with a rage of 7-100+ months. 5-years overall survival rates was 44% by Kaplan-Meier method. 4) Reduced overall survival was demonstrated in the patients who expressed bcl-2 protein(P=0.0174). 5-year overall survivial rate was 12%(bcl-2 expression) versus 88%(no bcl-2 expression) 5) Among diffuse large B-cell lymphoma patients with IPI score 0-1, reduced overall survival was demonstrated with bcl-2 expression(P=0.023). 5-year overall survival rate was 18%(bcl-2 expression group) versus 100%(no bcl-2 expressiongroup) 6) Median survival durtation of diffuse large B-cell lymphoma patients negative for p16 expression was 57 months whereas p16 postive patients' median survival duration was not reached(P=0.4478). In diffuse large B-cell lymphoma patients with identical IPI scores, bcl-2 expression had additional prognostic value.

      • A Research on Ultra Precision Machining for Ti-6AL-4V Alloy Based Biomedical Applications Using Nanopositioning Mechanism

        <P>In this study, piezoelectric actuator, flexure guide, power transmission element and control method are considered for nanopositioning system apparatus. The main objectives of this thesis were to develop 2-axis nanostage which enables 2-axis control with the aid of piezoelectric actuator, and to improve the precision of the ultra-precision lathe (UP2) which is responsible for the ductile mode machining of the hardened-brittle material where the machining uses a the single-crystal diamond. Through simulation and experiments on ultra-precision positioning, stability and priority of the nanopositioning system with 2-axis nanostage and control algorithm are developed using Matlab/Simulink. Then the system, is applied to analyze surface morphology of the titanium alloy (Ti-6Al-4V)</P>

      • A GSK-3/SHAGGY-Related Protein Kinase is Involved in Phytochrome Signal Transduction Pathway

        Phosphorylation of cellular proteins is a key regulatory mehanism for signal transduction pathway in living cells. Phytochrome, a red/far-red light photoreceptor in plants, is known to employ protein phosphorylation for its light signaling, although its detauked mechanism is still ambiguous. This study is intended to identify the phosphoproteins and protein kinases that are regulated by phytochrome, by employing transgenic rice seedlings that overexpress Arabidopsis phytochrome A. Red light stimulated phsophorylation of a 70 kDa protein and far-red light negated the effect. The red light induced phosphotylation of the 70 kDa protein was strongly activated by heparin and inhibited by poly-L-lysine, suggesting that the 70 kDa protein phosphorylating kinase belongs to GSK-3/SHAGGY protein kinase that has functional roles in establishing cell fate and pattern formation in Drosophila. Taken together with the fact that phytochrome controls plant development, these results may suggest that a GSK-3/SHAGGY-related protein kinase in plant(ASK) is likely to be involved in phytochrome signal transduction.

        누두흉환자에서 Nuss 방법에 의한 수술 중 환기방법에 따른 호기 일회호흡량, 흡기 최대압력 및 활력징후의 변화

        Changes of Vital Signs, Expired Volume and Maximal Pressure by Mode of Ventilation during a Nuss Operation in Patients with Pectus Excavatum Soo Dal Kwak, M.D., Nam Soon Baek, M.D., Su Hyun Cho, M.D. Ki Ryang Ahn, M.D., Sun Hak Lee, M.D., Chun Suk Kim, M.D. Wook Park, M.D., and Hyung Joo Park, M.D. Departments of Anesthesiology, ??Chest Surgery, College of Medicine, Soonchunhyang University, Cheonan, Korea Background: This study was undertaken in order to investigate any cardiopulmonary changes caused by the surgical procedure during a Nuss operation in patients with pectus excavatum. Methods: Thirty patients undergoing the Nuss operation were divided into two groups. The patients were randomly assigned to either Group-V (n = 15) which volume controlled ventilation was applied, or to Group-p (n = 15) where pressure controlled ventilation was applied. Mechanical ventilation provided a tidal volume to be set at approximately 10 ml/kg and a frequency set to keep ETCO_2 in range, between 32 mmHg and 38 mmHg. During the surgical procedure, we recorded expired volume (V_E) and inspired maximal pressure (Pmax) changes according to the ventilator mode, and then also recorded changes in vital signs. Results: Heart rate and blood pressure had no significant changes in either groups. In Group-V, during rotation of the metal bar, there was a significant decrease in expired volume (P < 0.001), changes of inspired maximal pressure gradually increased with the operation (P < 0.001). In Group-P, rotation of the metal bar caused a significant decrease in expired volume (P < 0.001). Conclusions: During the Nuss operation, there should be careful observation of changes in the vital signs, expired volume and maximal pressure during insertion and rotation of the metal bar.       • Marine Benthic Algal Community in the Coastal Waters off Soo Woo Island, Tongyeong

        The species composition and seasonal patterns in marine benthic algal community in the coastal waters off Soo Woo Island, Tongyeong were investigated seasonally throughout 2019. A total of 85 species, 9 Chlorophyta, 16 Phaeophyta, and 60 Rhodophyta, were recorded, and dominant species were Sargassum horneri, Caulacanthus ustulatus, Ceramium sp., Gracilaria textorii, Chondracanthus intermedius, Chondracanthus tenellus, Herpochondria elegans, Lithophyllum okamurae, Lomentaria catenata, Polysiphonia sp., and Pterocladiella capillacea during study periods. The number of species were differed with seasons and stations; Higher number of algal species was 52 species in spring, whereas 33 species was lower in summer. The number of algal species were higher at station 3, 4, while the lower value was at station 1, 6 than other stations. The differences of marine benthic algal composition at each station in MDS ordination due to the physical characteristics and local topography.

        비접촉 초음파 탐상 기법을 이용한 항공기 브레이크 디스크의 신뢰성 평가

        Carbon fiber-reinforced silicon carbide (Cf-SiC) and SiC / SiC composites have high thermal conductivity, and excellent corrosion and wear resistance, a low coefficient for thermal expansion and are lightweight. This is why they are commonly used in parts of the aerospace industry to develop an aircraft thrust deflector, jet vane, combustion chamber, elevens, body flap, and a shingle. So, understanding how this state-of-the-art Cf-SiC affects both internal and external crack detection and determining issues during the manufacturing process of composite materials, should be evaluated according to valuation techniques in the external environment. In this paper, we apply a non-contact air ultrasonic technique of non-destructive testing techniques to perform a study on internal defect detection identification and assessment of carbon-fiber reinforced silicon carbide composites to perform basic research and applied research.

        The air wipping system that is erupted the compressibility of a fluid through microscopic nozzle, it removes the surface of dust and heteromorphy material contact to the surface of manufacture material the spreaded air with high speed and in case of the necessity for surface coating, according to manufacture material that it can plate melted coating material to the surface, constantly. This system was applicating the dry, cooling, clean, platting plant etc. of product, usefully in the whole instruction line. The most useful application of filed is using for product an auto a galvanized steel sheet in continuous fusion zinc galvanizing. For this paper, we performed base research about that the analyzation of flowing characteristics of compressibility fluid in air wipping system for flowing fluid and the flowing visualization of air wipping flowing characteristics for using the method of ultrahigh speed image processing.
