        The prognostic impact of reduced variant burden in elderly patients with acute myeloid leukemia treated with decitabine

        Background/Aims: We evaluated the role of next-generation sequencing (NGS)-based disease monitoring for elderly patients diagnosed with acute myeloid leukemia (AML) who received decitabine therapy. Methods: A total of 123 patients aged > 65 years with AML who received decitabine were eligible. We analyzed the dynamics of variant allele frequency (VAF) in 49 available follow-up samples after the fourth cycle of decitabine. The 58.6% VAF clearance (Δ, [VAF at diagnosis − VAF at follow-up] × 100 / VAF at diagnosis) was the optimal cut-off for predicting overall survival (OS). Results: The overall response rate was 34.1% (eight patients with complete remission [CR], six of CR with incomplete hematologic recovery, 22 with partial responses, and six with morphologic leukemia-free status). Responders (n = 42) had significantly better OS compared with non-responders (n = 42) (median, 15.3 months vs. 6.5 months; p < 0.001). Of the 49 patients available for follow-up targeted NGS analysis, 44 had trackable gene mutations. The median OS of patients with ΔVAF ≥ 58.6% (n=24) was significantly better than that of patients with ΔVAF < 58.6% (n = 19) (20.5 months vs. 9.8 months, p = 0.010). Moreover, responders with ΔVAF ≥ 58.6% (n = 20) had a significantly longer median OS compared with responders with VAF < 58.6% (n = 11) (22.5 months vs. 9.8 months, p = 0.004). Conclusions: This study suggested that combining ΔVAF ≥ 58.6%, a molecular response, with morphologic and hematologic responses can more accurately predict OS in elderly AML patients after decitabine therapy.

      • Possible Role of Phosphoinositide-3-Kinase in Mx1 Protein Translation and Antiviral Activity of Interferon-Omega-Stimulated HeLa Cells

        <P>Interferon ω (IFN-ω), a cytokine released during innate immune activation, is well known for promoting direct antiviral responses; however, the possible signal pathways that are initiated by IFN-ω binding to the type I IFN receptors have not been fully studied. Here, we provide evidence that activation of phosphoinositide-3-kinase/protein kinase B (PI3K/Akt) signaling plays a pivotal role in the generation of IFN-ω-mediated biological responses. We found that LY294002 (PI3K inhibitor)-attenuated antiviral activities are induced by IFN-ω treatment. Although such effects of LY294002 are unrelated to regulatory activities on IFN-ω-dependent Mx1 (myxovirus resistance 1) or Mx2 gene transcriptional regulation, translation of Mx1 protein, which was known as a key mediator of cell-autonomous antiviral resistance, was significantly reduced by PI3K inhibition. Further studies showed that PI3K inhibition using LY294002 leads to a decrease in PI3K substrate Akt and mitogen-activated protein kinase extracellular signal-regulated kinase and p38 phosphorylation/activation. In addition, although LY294002 was not able to reduce STAT1 activation, we found that the mammalian target of rapamycin (mTOR)/p70 S6 kinase pathway was significantly attenuated by inhibition of the PI3K/Akt signaling pathway. These results indicate that the PI3K/Akt pathway is a common and central integrator for antiviral responses in IFN-ω signaling via its regulatory effects on mTOR that are required for initiation of mRNA translation of Mx genes.</P><P>Copyright © 2011 S.         <P>We present a single crystalline Ge nanowire anode material sheathed with carbon prepared by a solid–liquid solution method. The composite electrode composed of Ge nanowires shows impressive electrochemical properties, exhibiting a very high reversible charge capacity (after lithium removal) of 963 mA h g<SUP>−1</SUP> with a coulombic efficiency of 91%.</P> <P>Graphic Abstract</P><P>We present a highly pure Ge nanowire anode material sheathed with carbon prepared by solid–liquid solution method.         Venetoclax with Azacitidine Induced Tumor Lysis Syndrome in an Elderly Patient with Acute Myeloid Leukemia: A Case Report

        Combination treatment with hypomethylating agents (HMAs) and venetoclax is being used increasingly in elderly patients with acute myeloid leukemia (AML). Venetoclax with HMAs has been reported to be associated with tumor lysis syndrome (TLS) in AML patients with high leukemic burden. We present a case of an elderly AML patient with low leukemic burden who developed TLS while receiving venetoclax and azacitidine (AZA). A 74-year-old man with newly diagnosed AML with NPM1 mutation received combination therapy with venetoclax and AZA in an outpatient clinic. Within 12 hours after starting venetoclax and AZA, the patient was admitted to the emergency room with fever, general weakness, and laboratory findings consistent with TLS. Based on our results, we recommend monitoring at the start of the treatment with venetoclax and HMAs to prevent and control TLS regardless of the leukemic burden and favorable genetic risk.

        Diagnostic Accuracy and Prognostic Relevance of Immunoglobulin Heavy Chain Rearrangement and 18F-FDG-PET/CT Compared With Unilateral Bone Marrow Trephination for Detecting Bone Marrow Involvement in Patients With Diffuse Large B-Cell Lymphoma

        Background: In diffuse large B-cell lymphoma (DLBCL), bone marrow involvement (BMI) has an important clinical implication as a component of staging and International Prognostic Index. This study aimed to determine whether molecular analysis of immunoglobulin heavy chain (IgH) genes and positron emission tomography-computed tomography (PET/CT) could overcome the limitation of defining morphologic BMI by trephination biopsy and could increase the diagnostic accuracy or prognostic prediction. Methods: A total of 94 de novo patients with DLBCL underwent PET/CT, polymerase chain reaction (PCR) test for detection of IgH gene rearrangement, and unilateral bone marrow (BM) trephination at diagnosis. Results: A total of 9 patients (9.6%) were confirmed to present morphologic BMI (mBMI) based on trephination biopsy. On the other hand, 21 patients (22.3%) were confirmed to have IgH clonality (IgH BMI), while 16 (17.0%) were classified with BMI based on the assessment of PET/CT (PET BMI). Each IgH rearrangement PCR and PET/CT showed the high negative predictive value of detecting the BMI. However, the combined assessment of IgH rearrangement and PET/CT could increase the diagnostic accuracy and specificity with 87.2% and 97.0%, respectively. The survival outcome of patients with double positive PET BMI and IgH BMI was significantly worse than that with either single positive PET BMI or IgH BMI, and even less than patients with neither PET BMI nor IgH BMI (3-year PFS: 50.0% vs. 75.4% vs. 97.9%, P = 0.007, 3-year OS: 50.0% vs. 75.6% vs. 80.1%, P = 0.035, respectively). Conclusion: This study suggests that the combined evaluation of PET/CT and IgH rearrangement could give additional information for predicting therapeutic outcomes in patients with negative morphologic BMI as an important part of the prognosis.

        내국인용 관광패스 개발에 관한 연구: 서울지역을 중심으로

        본 연구는 서울시가 보유한 관광자원과 다양한 즐길거리를 하나의 투어패스 시스템으로 엮어서, 서울시민 및 서울을 찾는 내국인들이 편리하고 저렴하게 서울시 관광을 진행할 수 있는 관광패스 서비스를 기획하고자 하였다. 이를 위해 국내·외 관광패스 사례 및 내국인 관광객의 서울 관광행태, 서울관광객 의견 등을 분석하여 시민의 여가선용을 위한 효율적이고 합리적인 패스를 개발하여 제시하였다. 관광패스의 사용자 요구분석을 위해 설문의 모집단을 서울시민을 대상으로 하여 서울역, 용산역, 청량리역 등 역사 안 열차 대기 승객을 중심으로 2018년 12월 7일부터 12월 12일까지 약 6일 간 설문을 진행하였다. 수집된 241부를 대상으로 서울시 관광객 방문행태를 알아보기 위한 빈도분석, 서울에 대한 관광목적지 속성별 중요도, 내국인용 서울관광패스 도입 관련 이용의사 등을 알아보기 위한 기술통계를 실시하였다. 이와 같은 선행연구를 바탕으로 5단계 모델 수립 절차에 의해 관광패스 사업모델을 개발하였다. 관광패스 권 종은 당일여행객을 위한 1일 권, 숙박여행객을 위한 2일 권, 1,2일권보다 상대적으로 여유 있는 기간 동안 본인이 방문하고자 하는 곳을 직접 선정하여 구성하는 Big 3권, Big 5 권 종 등 4가지로 발행하는 것을 제안하였다. 본 연구는 향후 내국인을 위한 관광패스 개발 시 의미 있는 정보를 제공할 수 있다는 실무적 측면과 다양한 국내외 관광패스 사례 및 관광객 의견 분석 등 체계적인 과정을 통해 관광패스를 개발하는 과정을 제시한 학문적 측면에서 의미가 있다고 할 수 있다. This study attempts to design a tour pass service that allows citizens of Seoul and Koreans visiting Seoul to travel conveniently and cheaply by combining the Seoul Metropolitan Government s tourism resources and various entertainment items. To attain this end, efficient and reasonable passes were developed and presented for citizens leisurely use by analyzing cases of domestic and foreign tourist passes, tourist behavior in Seoul by Korean tourists, and opinions by Seoul. To analyze the user s demand for the tour pass, the survey was conducted for about six days from Dec. 7 to Dec. 12, 2018, focusing on passengers waiting for trains at platforms including Seoul Station, Yongsan Station and Cheongnyangni Station. The total 241 survey data were collected and analyzed. With the quantitative data, this research provided the frequencies of visit, the importance of each tourist destination property in Seoul, and the intention of using the Seoul tourist pass for Koreans. Based on such prior research, a tourism pass business model was developed according to the five stage model establishment procedure. Tourist pass proposed four types of model based on the duration of travel(One-Day Pass, Two-Day Pass) and number of destination(Big-Three, Big-Five), and provide working-level implications. This study is meaningful in terms of practical aspects that meaningful information can be provided in the development of tourist passes for Koreans in the future, and academic aspects that suggest the process of developing tourist passes through systematic processes such as various domestic and international tourism pass cases and tourist opinion analysis.

      • 청소년기 진로성숙이 취업 준비기 심리요인에 미치는영향: 진로 자기효능감의 매개효과를 중심으로

        중학교 최근 진로발달의 관점에서 청소년들의 올바른 직업의식과 나아가 삶의 목표를 설정하는 데에 많은 관심을 두고 있다. 이는 청소년기의 진로발달이 갖는 중요성이 대두되고 있기 때문이다. 이러한 사회적 관심에 착안하여 본 연구는 청소년기의 진로계획성이 취업 준비기가 된 후에도 어떤 영향을 미치는지 알아보고자 하였다. 특히, 청소년기의 진로계획성이 취업 준비기의 진로 자기효능감과 낮은 자존감 및 불안과 같은 내적 진로장벽에 어떠한 영향을 미치는지 살펴보았다. 먼저, 청소년기 진로계획성이 취업 준비기의 진로 자기효능감과 낮은 자존감, 미래 불안에 미치는 영향을 살펴본 결과, 진로성숙이 낮은 자존감과 미래 불안에는 직접적인 영향을 미치고 있지 않았다. 그러나 청소년기 진로성숙이 취업 준비기 진로 자기효능감을 매개로 하여 낮은 자존감에 간접적인 영향을 미치고 있음을 확인하였다. 또한 취업 준비생의 진로 자기효능감은 낮은 자존감을 통해 미래불안에 간접적인 영향을 미치고 있었다. 이는 진로 자기효능감이 높을수록 자존감은 상승하며 이는 미래에 대한 불안과 스트레스를 감소시킬 수 있음을 나타낸다. 본 연구는 청소년기의 진로성숙에 관한 연구가 부족한 실정에서 청소년기의 진로성숙이 추후 취업 준비기가 되어 나타나는 개인 심리요인에 어떠한 영향을 미치는지 탐색적으로 살펴본 데에 의의가 있다. 하지만 청소년기 진로성숙이 취업준비기의 진로 자기효능감 및 낮은 자존감과 불안에 미치는 영향에서 다른 요인들을 통제하지 못하였다는 한계가 있어 보다 심층적인 추후 연구를 기대해 본다. This study is designed to explore the effect of adolescent’s career maturity on psychological factors for employment preparation, focusing on the mediating effect of career self-efficacy. We used the panel data, Youth Panel 2007 of Korea Employment Information Service. As a result of analysis, career maturity has no direct influence on low self-confidence and future anxiety. However career maturity has indirect influence on low self-confidence, mediated by career self-efficacy. In addition career self-efficacy affects indirectly future anxiety through low self-confidence. This research output will be used as a basic database for developing educational methods and career counseling for adolescent and young adults.

        Palmitate induces RIP1/RIP3-dependent necrosis via MLKL-mediated pore formation in the plasma membrane of RAW 264.7 cells

        <P><B>Abstract</B></P> <P>We previously reported that palmitate induces receptor-interacting protein (RIP)1-dependent necrosis in RAW 264.7 macrophage cells. In response to death receptor stimuli, RIP1 is reported to activate RIP3, which causes the phosphorylation and translocation of mixed-lineage kinase domain-like (MLKL) protein to the plasma membrane, subsequent pore formation in the plasma membrane, and necrotic cell death. In the current study, we investigated the role of MLKL in palmitate-induced, RIP1/RIP3-dependent necrotic cell death in RAW 264.7 cells. The down-regulation of RIP1 or RIP3 by siRNA transfection protected the cells from palmitate-induced cell death. In addition, MLKL was phosphorylated at the serine residue and translocated to the plasma membrane in palmitate-treated cells. In these cells, MLKL was observed as aggregate dots on the plasma membrane. The findings also show that palmitate induced the formation of pores with varied shapes and sizes, and an increase in propidium iodide (PI) uptake and lactate dehydrogenase <B>(</B>LDH) release. Furthermore, the down-regulation of MLKL by siRNA transfection significantly decreased palmitate-induced PI uptake and LDH release, resulting in protection against palmitate-induced necrotic cell death. The findings reported here indicate that palmitate induces RIP1/RIP3-dependent necrosis via MLKL-mediated pore formation of RAW 264.7 cells in the plasma membrane, which could provide a new mechanism to explain the link between elevated levels of free fatty acids (FFAs), palmitate in particular, and macrophage death.</P> <P><B>Highlights</B></P> <P> <UL> <LI> The down-regulation of RIP1 or RIP3 by siRNA transfection protected the cells from palmitate-induced cell death in RAW 264.7 cells. </LI> <LI> MLKL was phosphorylated at the serine residue and translocated to the plasma membrane in palmitate-treated RAW 264.7 cells. </LI> <LI> The down-regulation of MLKL by siRNA transfection significantly inhibited palmitate-induced pore formation in the plasma membrane, resulting in protection against palmitate-induced necrotic cell death in RAW 264.7 cells. </LI> </UL> </P>
