        묘박중 해저 저질에 따른 파주력 특성

        In general, a high tension on the anchor and chain is placed when a ship at anchor is subjected to heavy weather. Mariners have to pay attention to whether dragging anchor occurs to keep the safety of the ship at anchorage since it is difficult to maintain the stable motion of ship and it causes collisions with other ships nearby. In this paper, the ship motion against the external forces was shown to obtain the fundamental data about characteristic of holding power due to nature of seabed at anchor, so practical trials were carried out in rocky area and muddy area using a trial ship around coastal area of South Korea. In muddy seabed, holding power showed reasonable tension values depending on the distance from anchor position of continuing swing motions of a ship corresponding to wind force. Meanwhile in rocky seabed, tension values on the chain appeared very high occasionally regardless of the distance from the anchor position and seemed to exceed its holding power to be the breaking strain of the chain although weather was not in a severe condition. Therefore, some of the cables laid on the seabed were presumed to be caught in a crack on the rock. It is assumed that even a small amount of external force may cause the chain to break in a moment in rocky seabed. Additionally, wind and current forces had a somewhat contradictory effect on holding power of the ship between them. 선박 운항의 효율 및 안정성을 높이고자 제주 주변 및 남해 연안의 묘박지에서 묘박중 해저 저질에 따른 파주력의 변동에 관한 기초자료를 제공할 목적으로, 묘 박을 실시한 몇 지점에 대한 측정결과를 비교·분석하였 다. 저질이 펄 또는 암반 지역의 묘박지에서 투묘에 따른 파주력 특성을 측정한 결과, 초기 투묘시 펄에서의 닻의 파주력은 주묘로 인하여 줄어드는 값과 잘 일치하는 것 으로 판단되며, 묘박지에서 묘박중인 선체의 이동은 풍속 및 풍향과 잘 일치하고 있었다. 특히 선박에서의 풍속 의 크기는 선체의 운동 등의 영향으로 더 크게 나타나며, 이러한 영향 등으로 해저가 암반 지대인 경우, 묘쇄에 순간적인 장력의 급격한 변동이 있음에도 그에 대응하 는 선체의 위치변동으로 나타나지 않아 외력에 의한 묘 쇄의 파단 가능성이 높은 것으로 판단된다. 묘쇄의 현수 부분 길이를 충분히 확보하지 못할 경우, 외력에 의한 급격한 장력 증가를 흡수하지 못해 파단의 위험 발생 가능이 높으며, 특히, 너울 등이 있는 악천후의 경우 암 반 지대 투묘는 절대 금물이다. 묘박중 선체의 swing은 풍속이 절대적이며, swing의 이동 방향의 변화가 묘쇄에 가장 큰 힘을 주어 이에 따라 묘쇄의 장력증가로 파주력이 감소하게 된다. 유속은 대략 1 knot 이상이 되면 일정 부분 선체의 swing을 줄여 묘쇄의 장력을 줄이고, 이로 인해 파주력 감소를 억제하는 역할도 있는 것으로 판단 되므로, 향후 풍속과 유속이 묘박중인 선박에 어떤 형태 로 어느 정도의 영향을 미치는지 더욱 정략적인 분석이 필요할 것으로 판단되었다.

        The Effect of Simultaneous Antigravity Treadmill Training and Electrical Muscle Stimulation After Total Hip Arthroplasty: Short Follow-Up Time

        Objective To assess the effectiveness of our devised hybrid physiotherapy regime using an anti-gravity treadmill and a low-frequency electrical stimulation device, as measured in patients with hip osteoarthritis after total hip arthroplasty (THA). Methods The outcomes of the postoperative rehabilitation in 44 patients who underwent THA for hip osteoarthritis were retrospectively examined. The conventional group (n=22) underwent the postoperative rehabilitation according to our protocol, while the hybrid group (n=22) underwent the same training, along with training on an anti-gravity treadmill and training using a low-frequency therapeutic device. The outcome measures were recorded and reviewed with the Numerical Rating Scale for pain, which rates pain on an 11-point scale from 0 to 10, surgical side knee joint extension force, 10-m walking test, Timed Up and Go test, and the 6-minute walking distance (6MD). The outcome measurement was taken 2 weeks after conducting pre-operation and antigravity treadmill training and electrical muscle stimulation, and compared the respective results. Results At the timeframe of 2 weeks from the surgery after conducting a devised hybrid physiotherapy, the values of knee extension muscle strength and 6MD were not worse in the hybrid group than conventional group. In the evaluation at 2 weeks after surgery, the knee extension muscle strength and 6MD values significantly decreased compared with the preoperative values only in the conventional group. Conclusion Lower limb muscular strength and endurance were maintained in the hybrid group, which suggested that hybrid physiotherapy could maintain physical functions early after THA operation.

        The post-progression survival of patients with recurrent or persistent ovarian clear cell carcinoma: results from a randomized phase III study in JGOG3017/GCIG

        Objective: In this study we sought to investigate the clinical factors that affect post progression survival (PPS) in patients with recurrent or persistent clear cell carcinoma (CCC). We utilized the JGOG3017/Gynecological Cancer InterGroup data to compare paclitaxel pluscarboplatin (TC) and irinotecan plus cisplatin (CPT-P) in the treatment of stages I to IV CCC. Methods: We enrolled 166 patients with recurrent or persistent CCC and assessed the impactof variables, including platinum sensitivity, treatment arm, crossover chemotherapy, primarystage, residual tumor at primary surgery, performance status, ethnicity, and tumor reductionsurgery at recurrence on the median of PPS in patients with recurrent or persistent CCC. Results: A total of 77 patients received TC, and 89 patients received CPT-P. The median PPSfor patients with platinum-resistant disease was 10.9 months, compared with 18.8 monthsfor patients with platinum-sensitive disease (hazard ratio [HR]=1.88; 95% confidence interval[CI]=1.30–2.72; log-rank p<0.001). In the multivariate analysis, the platinum sensitivity(resistant vs. sensitivity; HR=1.60; p=0.027) and primary stage (p=0.009) were identified asindependent predictors of prognosis factors for PPS in recurrent or persistent CCC. Conclusions: Our findings revealed that platinum sensitivity and primary stage are clinicalfactors that significantly affect PPS in patients with recurrent or persistent CCC as well as other histologic subtypes of ovarian cancer. PPS in patients with recurrent CCC shouldestablish the basis for future clinical trials in this population.
