        인터넷 취업알선은 디지털시대로의 변화에 따라 취업의 새로운 패러다임으로 대두되고 있다. 인터넷 취업알선의 대두를 거래비용이론의 접근에 따르면 탐색비용을 비롯한 거래비용은 줄어드는 대산 도덕적 해이와 역선택 비용은 증가하는 것으로 분석된다. 따라서 인터넷 취업알선이 확산되기 위해서는 두가지 방안이 제시될 수 있다. 단기적으로는 도덕적 해이와 역선택을 축소하기 위하여 개인정보 보호 및 보안 등을 비롯한 시장질서를 정립하는 것이며, 장기적으로는 새로운 비용의 증가분을 담보하는 신뢰의 구축을 위하여 채용부터 인사노무관리 및 해고에 이르기까지 전과정을 수행할 수 있는 인적자원 종합 기업을 육성하고 양성하는 것이다. The internet employment service becomes new paradigm of recruiting in the digital ages. Transaction cost approach explains that emergence of internet employment service results from the reduction of selection and recruiting cost. Because transaction cost is reduced utilizing internet tool, internet employment service becomes very popular. But that service increase the cost of moral hazard and adverse selection. The effective strategy of internet employment service enlargement make, shortly, a rule of internet employment market including solving the security and safety problems. The long term strategy i s developing the human resource firms with the integrated HRM functions in order to basically solve the moral hazard and adverse selection problem by the formation of trust.

        Mini-Review : Comparing the Areas of Interest in the Field of Functional Gastrointestinal Disorder and Neurogastroenterology and Motility Between the East and the West

        There is a paucity of studies that compare the differences in published articles submitted from the East and the West in the area of neurogastroenterology and motility (NM). To compare the article topics from the East and the West which have been published, 5 Western (Gastroenterology, Gut, American Journal of Gastroenterology, American Journal of Physiology-Gastrointestinal and Liver Physiology, and Neurogastroenterology and Motility) and 3 Eastern gastrointestinal journals (Journal of Gastroenterology, Journal of Gastroenterology and Hepatology, and Journal of Neurogastroenterology and Motility) were selected based on the impact factor. Published papers were classified into 12 categories and 60 subcategories. The titles and abstracts of review articles, original articles, and meta-analyses from these journals were reviewed for the last 2 years (2013-2014). In case of Journal of Neurogastroenterology and Motility which is published quarterly, this search was performed for 5 years (2010-2014).Of the total 2656 reviewed articles, 842 (260 from the East and 582 from the West) were classified into the category of NM. The most frequently published papers from the Western researchers were categorized as brain-gut interaction, visceral hypersensitivity, and irritable bowel syndrome, whereas those from the Eastern researchers were categorized as gastroesophageal reflux disease, functional dyspepsia, and irritable bowel syndrome. This difference between the East and the West is not just due to the journal itself, but it also depends on the author’s affiliation and their ability to perform high quality research in the area of the NM. These data provide evidence for the research trend and give valuable information to the researchers for determining subjects for the study and for selecting proper journals for publishing their studies.         Min-Distance Hop Count based Multi-Hop Clustering In Non-uniform Wireless Sensor Networks

        In wireless sensor networks, an energy efficient data gathering scheme is one of core technologies to process a query. The cluster-based data gathering methods minimize the energy consumption of sensor nodes by maximizing the efficiency of data aggregation. However, since the existing clustering methods consider only uniform network environments, they are not suitable for the real world applications that sensor nodes can be distributed unevenly. To solve such a problem, we propose a balanced multi-hop clustering scheme in non-uniform wireless sensor networks. The proposed scheme constructs a cluster based on the logical distance to the cluster head using a min-distance hop count. To show the superiority of our proposed scheme, we compare it with the existing clustering schemes in sensor networks. Our experimental results show that our proposed scheme prolongs about 48% lifetime over the existing methods on average.

        A well-ordered flower-like gold nanostructure for integrated sensors via surface-enhanced Raman scattering

        <P>A controllable flower-like Au nanostructure array for surface-enhanced Raman scattering (SERS) was fabricated using the combined technique of the top-down approach of conventional photolithography and the bottom-up approach of electrodeposition. Au nanostructures with a mean roughness ranging from 5.1 to 49.6 nm were obtained by adjusting electrodeposition time from 2 to 60 min. The rougher Au nanostructure provides higher SERS enhancement, while the highest SERS intensity obtained with the Au nanostructure is 29 times stronger than the lowest intensity. The SERS spectra of brilliant cresyl blue (BCB), benzenethiol (BT), adenine and DNA were observed from the Au nanostructure. </P>

        <P>Aspirin-exacerbated respiratory diseases (AERD) are associated with the metabolism of arachidonic acid. FPR2 (formyl peptide receptor2) is a high-affinity ligand receptor for potent anti-inflammatory lipid metabolites: lipoxins. Thus, functional alterations of the FPR2 may contribute to AERD. We investigated the relationship between single-nucleotide polymorphisms (SNPs) in the FPR2 and AERD. Asthmatics were categorized into AERD <15% decreases in forced expiratory volume in one second (FEV(1)), and/or naso-ocular reactions after oral aspirin challenge (n=170) and aspirin-tolerant asthma (ATA, n=268). In all, 11 SNPs were genotyped. FPR2 protein expressions on CD14-positive monocytes in peripheral blood were measured using flow cytometric analysis. We performed RT-PCR of the FPR2 mRNA expressed by peripheral blood mononuclear cells. Logistic regression analysis showed that the minor allele frequency of FPR2 -4209T>G (rs1769490) in intron 2 was significantly lower in the AERD group (n=170) than in the ATA group (n=268) (P=0.006, P(corr)=0.04, recessive model). The decline of FEV(1) after aspirin challenge was significantly lower in the subjects with GG homozygotes of FPR2 -4209T>G than those with the other genotypes (P=0.0002). Asthmatic homozygotes for FPR2 -4209T>G minor allele exhibited significantly higher FPR2 protein expression in CD14-positive monocytes than did those with the common allele of FPR2 -4209T>G allele (P=0.01). There was no difference in the expression of the wild form and the exon 2 deleted variant form of FPR2 gene according to the genotypes of FPR2 -4209T>G. The minor allele at FPR2 -4209T>G may have a protective role against the development of AERD, via increase of FPR2 protein expression in inflammatory cells.</P>

        Hypotrichosis simplex is characterized by scanty or absent hair, which is not associated with ectodermal defects or systemic abnormalities. It is classified according to hereditary transmission, extent of affected area, and degrees of absence of hair. We report a case of an 18-year-old man presenting decreased hair density and woolly hair on the scalp. He recognized decreased hair density when he was 7 years old. There was no hair on the arms, legs, and axillary area. There were scanty hair on the eyebrow and pubic area. There was no family history of hereditary disorders affecting hair. Serum analysis revealed the increased level of dehydroepiandrosterone sulfate and progesterone. Low dose dexamethasone suppression test was negative and abdominopelvic computed tomography showed no adrenal incidentaloma or hyperplasia. The histologic examination of the scalp showed decreased number of hair follicles and mild perifollicular lymphohistiocytic and eosinophilic infiltration. Herein, we report a rare case of non-familial generalized hypotrichosis simplex.

      • A case of symmetric lipomatosis of the tongue

        Symmetric lipomatosis of the tongue (SLT) characterizes the symmetric and nonencapsulated lipomas infiltrating muscle fibers. It is an extremely rare condition and considered as different disease from multiple symmetric lipomatosis in affected age, body region, and ethnic group and in the relevance to alcoholism. We report a case of a 56-year-old man presenting multiple symmetric yellowish nodules on both lateral borders of the tongue. There was no pain or discomfort on the tongue. He was a chronic alcoholic and afflicted with pellagra, hypertension, diabetes, and anemia. The histologic examination of the lesion showed mature adipose tissue infiltrating lingual muscles consistent with SLE. He did not want the surgical resection of the lesions. Therefore, lipomas were treated by four times of intralesional injection of triamcinolone (8mg/cc). However, there was no significant change in the size of the lipomas. Herein, we report a rare case of SLT comorbid with pellagra.

        Characteristics and Risk Factors of Aspiration in Lateral Medullary Infarction

        Objective: To evaluate the characteristics of dysphagia and identify the risk factors of bolus aspiration in patientspresenting with pure lateral medullary infarction (LMI). Methods: Between January 2014 and January 2019, 51 post-stroke patients with LMI who underwent a videofluoroscopicswallowing study (VFSS) were enrolled retrospectively, and their medical records and brain magnetic resonanceimaging results were reviewed. The VFSS results were evaluated to analyze the swallowing function using thepenetration-aspiration scale, functional dysphagia scale, and imaging analysis software. Results: Bolus aspiration was detected in 21 patients (41.2%). The common abnormal VFSS findings were residue invalleculae (74.5%), delayed triggering of pharyngeal swallow (72.5%), residue in pyriform sinuses (62.7%), delayed pharyngealtransit time (56.9%), reduced laryngeal elevation (51.0%), and coating of the pharyngeal wall (49.0%). Theincidence of aspiration was significantly higher in the typical lesions (including the diagonal band-shaped lesions)and the large type lesions extending ventrally or dorsally, as compared to other lesion types (P<0.05). Logistic regressionanalyses revealed that the residue in pyriform sinuses is a significant independent risk factor of aspirationin the puree trial, and prolonged pharyngeal delay time (PDT) and residue in valleculae are significant risk factorsin the thin liquid trial (P<0.05). Conclusion: Considering all clinical factors, lesion locations, and swallowing processes, results of the current studyindicate that residue in pyriform sinuses is an independent risk factor of aspiration in the swallowing puree technique,whereas prolonged PDT and residue in valleculae are independent risk factors of aspiration in the swallowingliquid technique.

      • A case of oral manifestation of cutaneous lupus erythematosus treated by alitretinoin

        Lupus erythematosus is an autoimmune disease with a broad spectrum of skin and multisystemic manifestations. Cutaneous lupus erythematosus (CLE) is referred to the lesions restricted to skin and/or mucous membranes. The prevalence of oral involvement ranges from 9 to 45% in patients with systemic lupus erythematosus and from 3 to 25% in patients with chronic cutaneous involvement. Thus far, antimalarial drugs, dapsone, and retinoids are considered as the first- and second-line treatment medicine in the treatment of CLE. In case of treatment failure or intolerance by the above drugs, vitamin A derivative, alitretinoin was reported as an effective alternative treatment option. We report a case of a 63-year-old woman presenting erythematous eroded plaques on the right buccal mucosa. She was diagnosed as CLE after punch biopsy and received alitretinoin (30 mg daily for 2 days and 10 mg daily for 4 weeks). For 1 month, the patient showed continuous improvement of buccal lesions and finally, the lesions were nearly cleared. She experienced headache and vomiting when she took alitretinoin 30 mg at the first time and those symptoms were disappeared after the dose reduction of alitretinoin. There was no other adverse effect. Herein, we report a case of oral manifestation of CLE treated by alitretinoin and suggest that alitretinoin could be an effective treatment option for CLE.

        Clinical Characteristics and Treatment Outcomes of 3 Subtypes of Achalasia According to the Chicago Classification in a Tertiary Institute in Korea

        Background/Aims Achalasia is classified into 3 types according to the Chicago classification. The aim of this study was to investigate characteristics and treatment outcomes of 3 achalasia subtypes in Korean patients. Methods Fifty-five patients diagnosed with achalasia based on conventional or high-resolution esophageal manometry were consecutively enrolled. Their clinical characteristics, manometric, endoscopic and esophagographic findings and treatment responses were analyzed among the 3 subtypes of achalasia. Results Of 55 patients, 21 (38.2%) patients had type I, 28 (50.9%) patients had type II and 6 (10.9%) patients had type III. The median follow-up period was 22.4 (interquartile range, 3.6-67.4) months. Type III patients were older than type I and II patients (70.0 vs. 46.2 and 47.6 years, P = 0.023). The width of the esophagus in type I patients was wider with more frequent bird`s beak appearance on esophagogram than the other 2 types (P = 0.010 and 0.006, respectively). Of the 50 patients who received the evaluation for treatment response at 3 months, 7 patients (36.8% vs. 26.9%) were treated with pneumatic dilatation and 4 patients (21.1% vs. 15.4%) with laparoscopic Heller`s myotomy in type I and II groups, respectively. The treatment responses of pneumatic dilatation and Heller`s myotomy in type I group were 71.4 and 50.0% and in type II were 85.7 and 75.0%, respectively, and all 5 patients in type III group showed good response to medical therapy. Conclusions Clinical characteristics of 3 achalasia subtypes in Korean patients are consistent with other studies. Treatment outcomes are variable among 3 subtypes.
