        Novel Findings of Polypoidal Choroidal Vasculopathy via Optical Coherence Tomography Angiography

        Purpose: To introduce novel findings of polypoidal choroidal vasculopathy (PCV) via optical coherence tomographyangiography (OCTA)Methods: This study is a retrospective chart review of 16 patients (16 eyes) with PCV. OCTA (Avanti RTVue XR)findings were evaluated and selected for analysis after agreement by two retina specialists . Results: Twenty one polyps in 16 eyes (16 patients) with PCV were included in this study. The mean patientage was 67 years (13 men and three women). The shape of polypoidal lesions on OCTA at initial were halo(five polyps), rosette (seven polyps), and vascular network (nine polyps). Eight months after anti-vascular endothelialgrowth factor treatment, in a total of four eyes, seven polyps could be followed up completely, thetwo halo type polypoidal lesions changed to rosette and vascular network type. The lesions of three rosetteand two vascular network type lesions did not change in shape. In addition, the size of the polypoidal lesions(one among two halo types, two among three rosette types, and two among two vascular network types) decreased,but one halo type did not change and one rosette type increased in size on OCTA. Conclusions: En-face OCTA enabled us to categorize novel types of PCV with polypoidal lesions.

        Non-Invasive Measurements of Thickness of Superconductor Films by Using Two-Resonant-Mode Rutile Resonator

        <P>The film thickness should be known for extracting the intrinsic surface resistance from the effective surface resistance as measured by using the dielectric resonator method. Thicknesses of 70 nm to 360 nm are measured for YBa<SUB>2</SUB>Cu<SUB>3</SUB>O<SUB>7−δ</SUB> films in a non-invasive way by using the two-resonant-mode dielectric resonator (TDR) method. A rutile resonator with the respective resonant frequencies of 15.25–15.61 GHz and 15.10–15.37 GHz for the TE<SUB>021</SUB> and the TE<SUB>012</SUB> modes is used for this purpose. Differences between the values as measured by using the TDR technique and those measured by using a step profilometer appear to be less than 3%, which is smaller than the previous value of 5% as measured by using a 8.6 GHz single-resonance mode rutile resonator [Lee et al. J. Korean Phys. Soc. 54(2009)1619]. Merits of using the TDR method are discussed.</P>

        Histologic assessment of the biological effects after speedy surgical orthodontics in a beagle animal model

        Objective: Speedy surgical orthodontics (SSO), an innovative orthodontic treatment, involves the application of orthopedic forces against temporary skeletal anchorage devices following perisegmental corticotomy to induce movement of specific dental segments. Herein, we report the biological effects of SSO on the teeth and periodontal structures. Methods: Five beagle dogs were divided into 2 groups and their 6 maxillary incisors were retracted en masse by applying 500 g orthopedic force against a single palatal mini-plate. Retraction was performed without and with perisegmental corticotomy in groups I and II, respectively. All animals were killed on the 70th day, and their periodontal structures were processed for histologic analyses and scanning electronic microscopy (SEM). The linear distance between the third maxillary incisor and canine was used as a benchmark to quantify the retraction amount. Results: Retraction was markedly faster and retraction amount greater in group II than in Group I. Surprisingly, Group II did not show any root resorption despite extensive retraction, while Group I showed prominent root surface irregularities. Similarly, SEM showed multiple resorption lacunae in Group I, but not in Group II. Conclusions: SSO is an effective and favorable orthodontic approach for major en masse retraction of the maxillary anterior teeth.

        신경초종은 신경초에 발생하는 드문 양성 종양이나, 말초신경 기원의 종양으로는 많은 부분을 차지한다. 본 연구의 경우, 주 증상으로는 종괴를 우연히 발견하여 내원한 경우가 가장 많았고, 동통, Tinel sign, 운동 혹은 감각 이상, 방사통, 저린감으로 다양하게 나타났다. 평균연령 55.4세, 남녀비 7:10으로 나타났다. 호발 신경은 척골신경, 좌골신경, 후경골신경 순이었다. 세침 흡인 검사상 방추형 세포가 나타나 진단적 가치가 높았다. 수술은 대개 적출술을 시행하여 수술 후 증상의 소실을 보였고, 6예에서 합병증을 가졌으나 추시중 합병증의 소실을 보였다. Neurilemoma which cccurres in nerve sheath, is a rare benign tumor, but it largely occupies in the origin of peripheral nerve. In the case of this study, mass is accidentally discovered in the main symptom, and pain, tined sign. motor weakness, sensory change, radiating pain and tingling sensation were appeared as symptom in variety. The average age is 55.4 and the ratio of man to woman is 7:10. Origin nerve is ulna nerve in first, sciatic nerve is second. And posterior tibial nerve is third. In the test of aspiration cytology, spindle cell is appeared and it is highly valued in the aspect of diagnosis. The operation is mostly carried into effect as enucleation. After that, the symptom fades away. The complication is developed in six cases, but it disappeared in follow up

      • Ilizarov 술식을 이용한 골절 치료

        골절의 고정방법에는 여러 가지가 있으며 이중 Ilizarov 외고정 기구는 강력한 골절 부위 고정력과 압박 신연 운동이 가능하며 사지의 단축이나 각변형 및 회전변형의 고정이 용이하다는 장점이 있어 그 사용이 증가되는 추세이다. 타 기구를 이용한 고정이 어려운 조건에서 적절한 적응증을 선별하여 시행한 Ilizarov 외고정 방법은 여러 형태의 난치성 골절에서 만족할만한 방사선학적, 임상적 유합을 기대할 수 있다. There are some complicated fractures of the long bones that cannot be treated by conventional methods. Nineteen patients with dificult long bone fractures were treated by Ilizarov ring external fixators between the periods of January 1993 to January 1997. 1.There were 4 cases of severe comminuted fracture which had difficulty in reduction or reduction maintainance. 3 cases of segmental and comminuted fracture, 3 cases of short periarticular fracture, 2 cases of open fracture with soft tissue defect, 2 cases of open fracture with bone loss, 3 cases of Jehovah's witness and 1 case of physeal injury. 2.The bone union was obtained in all cases and the duration of Ilizarov ext. fixator application was minimum 6 weeks unto 22 weeks. With an average of 14 weeks and full weight bearing was possible in all cases after removal of the Ilizarov ext. fixator. So, we conclude that the Ilizarov external fixation technique is one of effective methods for the treatment of difficult fracture of the long bone.

        Korean Medication Algorithm for Depressive Disorder 2021, Fourth Revision: An Executive Summary

        Objective: In the 19 years since the Korean College of Neuropsychopharmacology and the Korean Society for Affective Disorders developed the Korean Medication Algorithm Project for Depressive Disorder (KMAP-DD) in 2002, four revisions have been conducted. Methods: To increase survey efficiency in this revision, to cover the general clinical practice, and to compare the results with previous KMAP-DD series, the overall structure of the questionnaire was maintained. The six sections of the questionnaire were as follows: 1) pharmacological treatment strategies for major depressive disorder (MDD) with/without psychotic features; 2) pharmacological treatment strategies for persistent depressive disorder and other depressive disorder subtypes; 3) consensus for treatment-resistant depression; 4) the choice of an antidepressant in the context of safety, adverse effects, and comorbid physical illnesses; 5) treatment strategies for special populations (children/adolescents, elderly, and women); and 6) non-pharmacological biological therapies. Recommended first-, second-, and third-line strategies were derived statistically. Results: There has been little change in the four years since KMAP-DD 2017 due to the lack of newly introduced drug or treatment strategies. However, shortened waiting time between the initial and subsequent treatments, increased preference for atypical antipsychotics (AAPs), especially aripiprazole, and combination strategies with AAPs yield an active and somewhat aggressive treatment trend in Korea. Conclusion: We expect KMAP-DD to provide clinicians with useful information about the specific strategies and medications appropriate for treating patients with MDD by bridging the gap between clinical real practice and the evidence- based world.

        Large-Scale Plasma Patterning of Transparent Graphene Electrode on Flexible Substrates

        <P>Graphene, a two-dimensional carbon material, has attracted significant interest for applications in flexible electronics as an alternative transparent electrode to indium tin oxide. However, it still remains a challenge to develop a simple, reproducible, and controllable fabrication technique for producing homogeneous large-scale graphene films and creating uniform patterns with desired shapes at defined positions. Here, we present a simple route to scalable fabrication of flexible transparent graphene electrodes using an oxygen plasma etching technique in a capacitively coupled plasma (CCP) system. Ascorbic acid-assisted chemical reduction enables the large-scale production of graphene with solution-based processability. Oxygen plasma in the CCP system facilitates the reproducible patterning of graphene electrodes, which allows controllable feature sizes and shapes on flexible plastic substrates. The resulting graphene electrode exhibits a high conductivity of 80 S cm<SUP>–1</SUP> and a transparency of 76% and retains excellent flexibility upon hard bending at an angle of ±175° and after repeated bending cycles.         The Korean Medication Algorithm Project for Depressive Disorder 2021: Comparisons with Other Treatment Guidelines

        The Korean Medication Algorithm Project for Depressive Disorder (KMAP-DD) first was published in 2002, and has been revised four times, in 2006, 2012, 2017, and 2021. In this review, we compared recommendations from the recently revised KMAP-DD 2021 to four global clinical practice guidelines (CPGs) for depression published after 2010. The recommendations from the KMAP-DD 2021 were similar to those from other CPGs, although there were some differences. The KMAP-DD 2021 reflected social culture and the healthcare system in Korea and recent evidence about pharmacotherapy for depression, as did other recently published evidence-based guidelines. Despite some intrinsic limitations as an expert consensus-based guideline, the KMAP-DD 2021 can be helpful for Korean psychiatrists making decisions in clinical settings by complementing previously published evidence-based guidelines, especially for some clinical situations lacking evidence from rigorously designed clinical trials.
