Vaccination and Complementary and Alternative Medicine in Patients with Inflammatory Bowel Disease

Background/Aims: Vaccinations in patients with inflammatory bowel disease (IBD) are recommended to prevent infectiousdiseases. However, there are few reports of vaccination in IBD patients in Korea. The frequency of complementary and alternativemedicine (CAM) use is high despite its uncertain effectiveness. This study aimed to identify the rates of vaccination anduse of CAM in patients with IBD. Methods: A total of 219 patients attended an education session for IBD patients held at SeveranceHospital on March 23, 2013. We conducted a survey on vaccination and CAM use in IBD patients; 120 patients completedthe questionnaire. Results: The influenza vaccination rate was 44.2% and pneumococcal vaccination rate was 4.2%. Thirtyone(66%) patients were aware of the importance of vaccination. The vaccination rate was higher in patients who were awareof the importance of vaccination compared with that in patients who were unaware of the importance of vaccination (70.1%vs. 41.7%, P =0.004). The rate of CAM use was 30.0%. The most commonly used CAMs were oral products: vitamins (33.3%),red ginseng (25.0%), and probiotics (19.4%). Conclusions: Awareness of the importance of vaccination and actual vaccinationrates were low in IBD patients. Despite insufficient evidence on the effectiveness of CAMs in IBD patients, many patients usedCAMs. We believe that repeated education and promotion of vaccination are important. Further large-scale studies to investigatethe efficacy and safety of CAMs are warranted in patients with IBD. (Intest Res 2014;12:124-130)
Carpal Tunnel Syndrome Caused by Space Occupying Lesions

Purpose: To evaluate the diagnosis and treatment of the carpal tunnel syndrome (CTS) due to space occupying lesions (SOL). Materials and Methods: Eleven patients and 12 cases that underwent surgery for CTS due to SOL were studied retrospectively. We excluded SOL caused by bony lesions, such as malunion of distal radius fracture, volar lunate dislocation, etc. The average age was 51 years. There were 3 men and 8 women. Follow-up period was 12 to 40 months with an average of 18 months. The diagnosis of CTS was made clinically and electrophysiologically. In patients with swelling or tenderness on the area of wrist flexion creases, magnetic resonance imaging (MRI) and/or computed tomogram (CT) were additionally taken as well as the carpal tunnel view. We performed conventional open transverse carpal ligament release and removal of SOL. Results: The types of lesion confirmed by pathologic examination were; tuberculosis tenosynovitis in 3 cases, nonspecific tenosynovitis in 2 cases, and gout in one case. Other SOLs were tumorous condition in five cases, and abnormal palmaris longus hypertrophy in 1 case. Tumorous conditions were due to calcifying mass in 4 cases and ganglion in 1 case. Following surgery, all cases showed alleviation of symptom without recurrence or complications. Conclusion: In cases with swelling or tenderness on the area of wrist flexion creases, it is important to obtain a carpal tunnel view, and MRI and/or CT should be supplemented in order to rule out SOLs around the carpal tunnel, if necessary.

Purification and General Properties of Pea Cytoplasmic Fructose - 1 , 6 - bisphosphatase

Cytoplasmic fructose-l,6-bisphosphatase (FBPase) from pea leaves was purified and characterized. The purified enzyme appeared to be homogeneous with a monomeric molecular weight of 37,000 as determined by SDS polyacrylamide gel electrophoresis. The purified enzyme was active at neutrnl pH (pH 6.0 to pH 7.0) and insensitive to dithiothreitol, as are other gluconeogenic FBPases from mammals and yeast. It was relatively stable against heat. The activation energy (Ea) and the Arrhenius frequency factor of the enzyme catalyzed reaction were 8.75 ㎉/㏖ and 1.8×10⁴/s, respectively.
