        Impact of the oxygen functional group of nitric acid‑treated activated carbon on KOH activation reaction

        To prepare activated carbon with a high specific surface area, oxygen functional groups (OFGs) that can serve as useful electron donors during KOH activation were treated with nitric acid and incorporated into activated carbon. OFGs are incorporated differently according to the surface characteristics of starting materials. Up to 22.46% OFGs are incorporated into wood-based activated carbons (WACs), the C=O, COOH contents was 1.90, 17.05%, respectively. Whereas up to 12.82% OFGs are incorporated into coconut shell-based activated carbons, the C=O, COOH contents was 4.12, 6.15%, respectively. The OFGs used for increasing the specific surface area are the carbonyl group, and as the content of the functional group increases, the carbonyl group spreads to the carboxyl group. The specific surface area of activated carbons increased by 10–68% with an increase in the carbonyl group up to 6% (maximum point of carbonyl group). On the other hand, the specific surface area for WACs increased when the carboxyl group was 10% or below, but decreased by 6–15% when it increased to 10% or excess.

        Tuning the Microstructure and Mechanical Properties of Cold Sprayed Equiatomic CoCrFeMnNi High-Entropy Alloy Coating Layer

        An equiatomic CoCrFeMnNi high entropy alloy (HEA) coating layer was prepared via a cold spray (CS) process. In orderto control the microstructure and nano-indentation properties of the CS HEA, heat treatments were employed. CS HEAcoating layer showed a heterogeneous microstructure where ultra-fine grains were formed at the particle interfaces, whilecoarse grains were formed inside the particles. Furthermore, deformation twins (DTs) were also formed inside the particlesdue to severe plastic deformation (SPD) in each particle generated during the CS deposition. For the 550 °C heat treatment(HT), fine Cr-rich precipitates were additionally formed at the grain boundary and particle boundary. By contrast, recrystallizationoccurred during 850 °C HT, while the size of the Cr-rich precipitate increased. The nano-indentation hardness ofthe CS HEA coating layer was 10.9 GPa, which was ~ 3 times higher than that of the conventional cast HEA. The superiorhardness of the CS HEA might has been enabled due to the combination of the high dislocation density, DTs, and ultra-finegrains. Based on the results above, the strategy to control the microstructure and mechanical properties through HT of theequiatomic CoCrFeMnNi HEA coating layer prepared via the CS process has been discussed.

        A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

        Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum.         울증(鬱證)환자에게 용서프로그램을 활용한 오지상승요법(五志相勝療法)을 시행하여 호전된 1례

        Yuzheng(鬱證) comes from obstruction of qi by stress. The patient has depressed mood, irritable sign, chest discomfort, costal pain, angry state or some strange feeling on the throat. Oh-Ji-Sang-Seung(五志相勝) therapy is base on the theory of interrelation in five elements in oriental medicine. The contents of Oh-Ji-Sang-Seung(五志相勝) therapy include five subjugations of five emotions. Anxiety subjugates fear(思勝恐), fear subjugates joy(恐勝喜), joy subjugates pity(喜勝悲), pity subjugates anger(悲勝怒), and anger subjugates anxiety(怒勝思). Forgiveness program is a kind of psychological therapies to decrease the degree of anger and it is included in Oh-Ji-Sang-Seung(五志相勝) therapy. In this case, a female patient, 50 years old, who suffered from Yuzheng(鬱證) with chest discomfort, irritable sign, easily angry state, depressed mood, hot flush, insomnia. We used Oh-Ji-Sang-Seung(五志相勝) therapy besides herbal medication, acupuncture to her condition got improved. Therefore we reported it for the treatment.

      • Diagnosis of serous ovarian cancer malignancy using miR-214-3p expression measurement in serum exosome

        Objective: To evaluate expressions of target gene expression and target gene mRNA level expression in ovarian tissues and serum exosome and investigated the roles of exosome for early detection of ovarian cancer and improvement of prognosis. Methods: We selected five samples of each ovarian tumors (benign, borderline, low grade serous carcinoma, and high grade serous carcinoma) collected in Seoul national university hospital between June 2012 and September 2017. We analyzed mRNA expression and target genes in exosome membrane protein and ovarian cancer tissue and checked the targeted genes mRNA level which had tumor suppressor function based on the references and miRDB (http://mirdb.org/). We investigated the expression of miR-214-3p from the serum-derived exosomes of benign (n=5), borderline (n=4), low grade serous carcinoma (LGSC, n=5), high grade serous carcinoma (>12 months (n=5), 6-12 months (n=5), <6 months (n=5) depending on recurrence period after chemotherapy). Results: MicroRNA-214-3p (miR-214-3p) has various target genes and is reported to be involved in the proliferation, survival, angiogenesis and invasiveness of cancer cells. MiR-214-3p expression in exosomes derived from serum was reported to increase with the malignancy of ovarian tumors (p-value <0.001). MiR-214-3p was overexpressed in the exosome of serous carcinoma group compared to benign group. Expression of LIM homeobox domain 6 (LHX6) gene, which is known as a direct target of miR-214-3p, was low in the serous carcinoma group compared to benign or borderline group (p-value <0.001). We verified the decrease in expression of miR-214-3p in exosomes from ovarian cancer cells (OVCAR3, OV90, and ES2) after treatment with GW4869, which inhibited exosome production (p-value <0.001). Conclusion: We suggested that miR-214-3p could be used as a novel biomarker for the diagnosis of serous ovarian carcinoma by measuring within the exosome from serum.

      • Localized Darier’s disease of scalp

        Darier’s disease (DD) is a rare autosomal dominant genodermatosis characterized by greasy hyperkeratotic papules in seborrheic regions, nail abnormalities, and mucous membrane changes. Mutations in the gene ATP2A2 cause abnormal keratinocyte-keratinocyte adhesion and aberrant epidermal keratinization and it is the primary histologic features of DD. Localized DD (LDD) is a clinical variant, usually presenting a zosteriform, linear, or segmental distribution and, in 10% of the cases, situated along the Blaschko lines, because of a genetic mosaicism induced by a mutation occurring during embryogenesis. A 73-year-old male presented with pruritic, multiple, erythematous lesions on the scalp for several decades. He had no family history of similar skin lesions. Physical examination revealed linearly distributed, verrucous plaques on the occiput area and crusted erosions on the Rt. frontal area. Histopathology findings included hyperkeratosis, focal acantholysis, dyskeratosis with corps ronds and grains and mild perivascular lymphocytic infiltration, suggesting the diagnosis of LDD. After use of topical retinoids and corticosteroids for 1 month, the lesions were greatly improved. LDD, however rare, should be considered as a differential diagnosis of clinical lesions that follow Blaschko's lines, as well as verrucous epidermal nevus, striated lichen and linear lichen planus. Herein, we report a case of LDD of the scalp as an educational case.

      • Anemia in Chronic Liver Disease

        Aims: The anemia in chronic liver disease is common. The causes can be bleeding, malnutrition, erythrocyte sequestration and underproduction, and medicine’s side effects. Many patients are just regarded as iron deficiency anemia (IDA) and described iron supplements. We investigated the patterns of anemia in chronic liver disease and triage the causes. Methods: The patients invited to Korea university ansan hospital from 2012 to 2016 were investigated retrospectively. 1632 patients were test for anemia lab. The definition of anemia is <13.0g/dl(men) or 8.1g/dl(women) according to WHO’s threshold. Reticulocyte production index (RPI) is calculated by [reticulocyte count (%)*hematocrit/ 45]/reticulocyte maturation time. If RPI ≥2.5, it is classified as hemolysis or hemorrhage. If RPI < 2.5, it was checked red cell morphology. Normocytic (MCV 80-100) is classified as hypo-proliferative disorder, and macrocytic (MCV >100) or microcytic (MCV <80) is classified as maturation disorder. In hypo-proliferative disorder, eGFR (using MDRD fomula) < 30ml/min/1.73 m2 is classified as renal disease. If CRP >3mg/dl, it is classified as inflammatory disorder. In maturation disorder, if transferrin saturation (Fe/TIBC) <15% or ferritin <18 ng/ml, it classified as IDA. In macrocytic morphology, if the patients diagnosed as alcoholic disease, it is classified as folate and/or vitamin B12 deficiency. Others are classified as undetermined. This classification is modified from ‘algorithm of the physiologic classification of anemia’ of Harrison’s Internal Medicine. Results: Total 441 patients were available for analysis. 11 patients were classified as hemolysis/hemorrhage according to RPI ≥2.5. 430 patients were divided as hypo-proliferative (n=275) and maturation disorder (n=155) according to RBC morphology. IDA 72, renal disease 9, inflammatory disease 30, and thiamine folate deficiency 61 patients were classified. 258 patients were undermined. In IDA 72 patients, 38 patients were treated with iron supplement. In other causes 111 patients, 16 patients were described iron. In undermined 258 patients, 75 patients described iron. The proportion who diagnosed and appropriately treated with iron supplement was 8.8%, and 3.7% were miss treated, and 17.5% needed further evaluation. Conclusions: A lot of patients with anemia in chronic liver disease are treated with undetermined diagnosis. Using algorithm, many patients could be classified and treated appropriately.

        Differences of Spinal Curvature, Thoracic Mobility, and Respiratory Strength Between Chronic Neck Pain Patients and People Without Cervical Pain

        Objective To investigate the differences of spinal curvature, thoracic sagittal mobility, and respiratory strength between patients with chronic neck pain (CNP) and people without cervical pain, and to determine the correlation between respiratory strength and thoracic mobility in CNP patients. Methods A total of 78 participants were finally included in this study, of whom 30 had no cervical pain and 48 had CNP. The Neck Disability Index (NDI), cervical lordotic curvature, thoracic kyphotic curvature, thoracic sagittal range of motion (ROM), maximal inspiratory pressure (MIP), and maximal expiratory pressure (MEP) were measured and analyzed. Results In males, thoracic sagittal ROMMEP-MIP and MEP showed a significant difference between the no cervical pain group and the CNP group. In females, thoracic kyphotic curvature, thoracic sagittal ROMMEP-MIP, MIP, and MEP were significantly different between the no cervical pain group and the CNP group. Thoracic kyphotic curvature was significantly correlated with MEP and MIP in all population groups, and significantly correlated with NDI in the female group. Thoracic sagittal ROMMEP-MIP had a significant linear relationship with NDI, MEP, and MIP in all population groups. Conclusion The thoracic mobility during forced respiration was reduced in patients with CNP and was correlated with respiratory strength. Changes in the biomechanics of the cervicothoracic spine and rib cage due to CNP may contribute to impairment of respiratory strength.
