Patellar fractures result from direct and/or indirect forces. The majority of the patellar fractures result from direct injuries and a fall on the knee or a direct blow sustained in the vehicular trauma is one of the common causes. Patellar fractures from indirect forces occur when the intrinsic strength of the patella is exceeded by the pull of the musculotendinous and ligamentous units attaching to it and typically occurs in the act of stumbling or partially falling. Posterior cruciate ligament disruption is generally caused by a posteriorly directed force on the proximal tibia, and hyperextension with abduction or external rotation movements have been also described in the production of posterior cruciate ligament injuries. Posterior cruciate ligament injuries are apt to be negelected frequently, and ipsilateral femoral shaft fracture or posterior dislocation of the hip also may be seen with direct injuries to the patella, but patellar fractures are rarely associated with posterior cruciate ligament injuries. So we are reporting four cases of the posterior cruciate ligament injuries as one of the combined entity, which had been neglected and associated with the patellar fracture after a review of the literatures.

Objective: We describe various clinical presentations of female urethral meatal stricture, which may mimic other pelvic floor disorders and result in diagnostic delay. Diagnosis, management, and outcome results are reported Materials and Methods: We reviewed retrospectively 26 consecutive cases of female urethral meatal stricture. Patient characteristics, history, clinical presentation, symptom score, maximal flow rate, residual urine, diagnosis, management and short-term follow up are reported. Results: Mean patient age plus or minus standard deviation was 54 years. Most 21 (81%) cases were referred as diagnostic dilemmas with symptoms present for 1 month to 30 years. Mean interval between onset of symptoms to diagnosis was 5.8 years. The most common symptoms were residual urine sensation (77% of cases), frequency (54%), pelvic pain (46%), dysuria (46%), weak stream urine (27%), urgency (15%) and 17 patients (65%) have combined symptoms. 16 patients (62%) had been treated with oral and/or vaginal medication and/or psychotherapy. The female urethral meatal stricture was visible on physical examination in 21 patients (81%) and cystoscopy in 5 patients (19%). Of these visible urethral meatal stricture, 11 patients (45%) contained recurrent urinary tract infection, 9 patients (35%) contained urethral mucosal prolapse, 7 patients (27%) contained urethral diverticulum, and 3 patients (12%) contained stress urinary incontinence. 18 patients (69%) who has been treated with oral and/or vaginal medication or psychotherapy was not successfully improved. Treatment consist of urethral meatal excision and/or everted urethral mucosal excision and/or urethral diverticulum excision and/or anti-incontinence surgery when indicated. Subsequently 24 patients (92%) had improved symptom score, but 2 patient was not changed. In 9 patients new symptom developed. Conclusions: Female urethral meatal stricture have several urinary symptoms and may mimic other pelvic floor disorders. This condition should be considered in women with frequency, residual urine sensation, pelvic pain, dysuria and etc. Medical treatment is not sufficient but surgical treatment is usually simple, safe and effective in alleviation associated symptoms.

The effect of non-point source pollution in rural area has much influence on the stream water quality. It is difficult to improve the stream water quality, because of complex pollutant loads technically and because of treatment-cost of the deteriorated water economically. The best way to reduce the non-point source pollution is to check the water quality in the inlet of irrigation channel and control the water quality in the outlet of drainage channel. Stream water quality in the rural area is closely related with the fertilizers, pesticides, and livestock wastewater. Basically the rate of treatment and retention time were estimated for BOD, COD, SS, NH₄-N, T-N, NO₃-N, T-P, PO₄-P using materials such as gravel, crushed stone, useless tire, geotextiles, and concrete block, respectively and also using the combined materials. The rate of treatment for BOD, COD, SS is high, but that for NH₄-N, T-N, NO₃-N, T-P, PO₄-P is low. Optimal retention time in the natural contact channel was 90 minutes.

From March 1991 to August 1995, twenty-eight cases of intraarticular comminuted distal radius fractures were treated by percutaneous pinning with closed reduction under fluoroscopy. From all cases, twenty two fractures(79%) were healed satisfactorily with no special problems. But in cases of fractures showing severe comminution or osteoporosis, the result was not good. We conclude that percutaneous pin fixation is an excellent technique for an unstable fractures in combination with external immobilization, provided the distal radius is not severly comminuted or osteoporotic.
