        공중보건 위기 상황에서의 의료인의 돌봄의 의무

        The possibility of a public health emergency is increasing gradually due to new infectious diseases, biological terrorism and natural disasters. In the event of a public health emergency, healthcare professionals who take care of patients on the front line will be given a duty of care as professionals while at the same time taking individual risks, which leads to conflicts of the ethical values. Therefore, the discussion of the reciprocal obligation of the society for the healthcare professionals’ protection as well as the legal obligation to emphasize their responsibility should be preceded in order to perform their duties of care voluntarily. Through a literature review, this article analyzed social expectations and issues about the duty of care, which is considered as the mission of the healthcare professionals in the event of a public health emergency, and suggestions to realize duty of care were given from various aspects. Social efforts to establish a system of social support and protection for the healthcare professionals in the event of a public health emergency could reduce ethical conflicts between the professional and personal values and contribute to the maturity of professional values that aims to voluntarily perform their duties of care. 최근 들어 신종 감염병, 생물학적 테러, 자연재난 등으로 인한 공중보건 위기의 가능성에 대한 관심이 증대되고 있다. 공중보건 위기가 실제 발생할 경우, 최전선에서 환자를 돌보는 의료인은 전문직으로서의 돌봄의 의무를 부여받는 동시에 개인의 위험을 감수하게 되어 의료인의 윤리적 가치 사이의 갈등이 초래될 수 있다. 따라서 돌봄의 의무를 자발적으로 구현하기 위해서는 의료인의 책임을 강조하는 법적인 의무뿐만 아니라 의료인의 보호를 위한 사회의 상호호혜적 의무에 대한 논의가 선행되어야 할 것이다. 본 원고에서는 문헌고찰을 통해 공중보건 위기 발생 시 의료인의 사명으로 고려되는 돌봄의 의무에 대한 사회적 기대와 쟁점을 분석하고, 진정한 돌봄의 의무를 구현하기 위한 방안을 다각적 측면에서 살펴보았다. 공중보건 위기 발생 시 의료인에 대한 지원과 사회적 보호 체계를 마련하고자 하는 사회적 노력은 의료인의 전문적 가치와 개인적 가치 사이에 윤리적 갈등을 감소시키며, 돌봄의 의무를 자발적으로 구현하고자 하는 의료인의 전문직관을 성숙시키는 데 기여할 수 있을 것으로 기대한다.

        사단 의무대의 재이용 의도에 영향을 미치는 요인

        Purpose: The purpose of this study was to identify the factors affecting the revisit intention to medical battalion. Methods: This descriptive study was conducted to 297 Republic of Korea Army soldiers who visited 10 medical battalions for outpatient service. Data were collected from September to October 2019 using the structured questionnaires. Results: The multivariate linear regression analyses showed that favorable attitude of peer or commander for visiting medical battalion; having no drug adverse effect; easy access to safety equipment such as fire extinguisher; longer doctor’s consultation time; facilities and environmental services; and satisfaction for overall outpatient services were highly positively correlated with revisit intention to medical battalion. These factors accounted for 76% of revisit intention to medical battalion. Conclusion: This study identified the importance of patient's experience on revisit intention to medical battalion. The policy to improve the quality of military health care should be established to meet the needs and expectations of the soldiers.

        국군간호사관생도의 스트레스 자각, 대인관계 유능성, 적응적 인지정서조절이 정신건강에 미치는 영향

        Purpose: This study aimed to examine the level of perceived stress, interpersonal competence, adaptive cognitive emotional regulation, and mental health and identify factors that influence mental health of Korea Armed Forces Nursing Academy (KAFNA) cadets. Methods: A descriptive predictive design was used. Data were collected using questionnaires from a convenience sample of 169 KAFNA cadets in 2021. The data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple linear regression. Results: The mean scores of the perceived stress, interpersonal competence, adaptive cognitive emotional regulation, and mental health were 1.99, 3.42, 3.78 and 0.28 respectively. Mental health differed significantly according to religion (F=-2.04, p=.039). Mental health correlated with perceived stress (r=.70, p<.001), interpersonal competence (r=-.34, p<.001), and adaptive cognitive emotional regulation (r=-.37, p<.001). Multiple regression analysis showed that mental health accounted for 57.0% of the variance by perceived stress, interpersonal competence, adaptive cognitive emotional regulation, and religion (F=56.44, p<.001). Conclusion: The results of the study show that perceived stress, interpersonal competence, and adaptive cognitive emotional regulation are important determinants of mental health in KAFNA cadets. These findings suggest that strategies focused on positive cognitive evaluation in stressful situations, and interpersonal relationship should be developed for KAFNA cadets to promote their mental health.

      • 간호실무 : 미혼남녀의 피임경험에 관한 질적 연구

        Purpose: The aim of this study was to identify contraception behaviors in unmarried men and women. Methods: A descriptive qualitative study design was used. Individual in-depth interviews with fifteen participants were conducted. Content analysis was utilized to determine the main outcomes. Results: For men, three themes were consistently seen as contraception behaviors: ``men desired sexual intercourse greatly``, ``men would not use a contraceptive device, even though they knew it was necessary``, and ``men gradually considered contraceptive use to live their lives without interruption``. For women, the three themes were as follow: ``women had sex half willingly while half did not have sexual inter course for the first time``, ``women did not use contraceptive devices because of ignorance of contraceptive methods``, and ``women did not carefully consider contraceptive use``. Conclusion: To prevent unmarried men and women from having unplanned pregnancies, tailored sex education programs should be established throughout Korea. All things considered, the effect of multimedia sex education should be considered to maximize the use of contraceptive devices.

        미혼남녀의 피임실천행위와 관련 요인

        Purpose: This study aimed to identify differences in contraception behavior and related factors between unmarried female and male. Methods: A cross-sectional survey design with a convenient sampling was used. From Seoul, 107 women and 96 men were recruited. A structured questionnaire was self-administered from July 14, 2008 to September 2, 2008. Results: The average ages of women and men were 26.9 and 27.8 years old, respectively. Fifty-six percent of women and 85.4% of men have experienced sexual intercourse. There were no differences between female and male in attitude toward contraception, subjective norm, and contraception behavior, while female had higher score of self-efficacy for contraception (p=.02) and intention of contraception (p=.02). There was positive correlation with subjective norm (r=.22, p<.05), self-efficacy (r=.53, p<.01) in male and attitude (r=.32, p<.05), self-efficacy (r=49, p<.01) in female. According to the result of multiple regression, the significant factor of contraception behavior was self-efficacy for both female (β=.49, p=.00) and male (β=.53, p=.00). Conclusion: To improve contraception behavior for unmarried female and male, it is necessary to develop proper sex education program to enhance self-efficacy for contraception. Adjusted education program by this result will contribute to increase sexual health for female and male.

        COVID-19 대응 군 간부의 소진, 이차적 외상성 스트레스 및 공감만족

        Purpose: The purpose of this study was to identify individual and occupational factors influencing burnout, secondary traumatic stress, and compassion satisfaction in military officers who experienced supporting civilians in responding to COVID-19. Methods: This descriptive study was conducted on 140 special forces’ military officers who experienced responding to disasters within 3 months. Data were collected through an online survey from September to November 2021. Results: The mean scores for burnout, secondary traumatic stress, and compassion satisfaction were 21.67±6.03, 20.54±8.21, and 39.72±8.12 out of 50, respectively. The significant factors that influenced burnout and secondary traumatic stress were ‘higher passive stress copying styles’ (B=0.17, p<.001; B=0.31, p=<.001, respectively) and ‘lower social support (B=-0.11, p=.031; B=-0.10, p=.001, respectively). The compassion satisfaction was more significantly associated with ‘higher self-efficacy’ (B=0.37, p=<.001), ‘higher active stress copying styles’ (B=0.19, p=.006), and ‘education responding to disasters’ (B=2.04, p=.029). Conclusion: The results suggest that the strategies to increase social support, self-efficacy, and active stress coping styles should be considered in developing educational programs for military officers responding to disasters to minimize burnout, secondary traumatic stress and to improve compassion satisfaction.

        간호․보건계열 전문직 간 교육을 위한 시뮬레이션 모듈 개발 및 적용

        Purpose : This study aims to develop an interprofessional education (IPE) simulation module and explore the effect of IPE on nursing and health science students. Method : In this study, simulation on IPE was developed, and IPE practice was adopted for students of nursing and health science (clinical pathology, health administration, occupational therapy, physical therapy, radiology). In December 2019, the satisfaction, self-confidence, and learning flow of 119 students were analyzed as a descriptive study by applying the convergent design of a mixed study. Students’ learning experience was explored through group debriefing and individual self-reflection journals. Results : Flow was positively correlated with satisfaction (r=.66) and self-confidence (r=.81), whereas positive correlation was found between satisfaction and self-confidence (r=.70). The significant predictors of the flow in learning were the satisfaction (β=.15) and self-confidence (β=.70). The theme was derived from three core themes and eight sub-themes, with “Be a healthcare professional approached as a team, who collaborate and communicate for the patient’s health with mutual respect” proving to be the one that students assimilated. Conclusion : When designing an IPE simulation module, realistic IPE-simulated practice modules promoting inter-professional interaction should be developed, and their effectiveness should be continuously evaluated.
