        비전정에 발생한 혈관평활근종 1예

        Angioleiomyoma is an uncommon benign tumor composed of smooth muscle cells and vascular endotheliums. It is common in the uterus and the gastrointestinal tract but rare in the head and neck. This neoplasm is extremely rare in the nasal cavity, accounting for less than 1% of all vascular leiomyomas. The tumor has a benign nature and the recurrence of tumor is rare and complete removal is the treatment of choice. We present a case of angioleiomyoma which involves the nasal vestibule and was excised by a sublabial approach.         상악동에서 발생한 골성 혈관종 1예

        Osseous hemangiomas typicallyoccur in the vertebral column and skull bones. Osseous hemangioma in the nasal cavity is extremely rare; only eight cases originating in theturbinates and maxillar bone have been reported in the English and Korean literature. Recently, we observed a case of osseous hemangioma occupying the right maxillary sinus. After preoperative embolization, the mass was completely removed by endoscopic inferomedial maxillectomy. There wasno evidence of recurrence at24 months postoperatively. To our knowledge, this is the first reported case of osseous hemangioma occupying the maxillary sinus.

        비용종 환자에서 포도상구균 외독소에 대한 말초혈액 단핵구의 반응

        Background and Objectives:Superantigens such as Staphylococcus aureus exotoxin (SE) have been implicated in the pathogenesis of chronic rhinosinusitis with nasal polyposis (NP). The aim of this study was to determine the immunologic response of peripheral blood mononuclear cells (PBMCs) to staphylococcal exotoxin B (SEB) in patients with NP. Methods:The interleukin (IL)-4, IL-5, and interferon-γ (IFN-γ) responses of PBMCs to nonspecific mitogens such as phylohemagglutin (PHA) and SEB were examined in 24 NP patients and 16 control subjects. The presence or absence of atopy and asthma was determined to evaluate the correlation of these conditions with the levels of cytokines. Results:PBMCs from the NP patients were more likely to produce IL-4 and IL-5 in response to SEB than those from controls. There was no difference in the mitogen-induced cytokine responses between NP patients and controls. SEB-induced IL-5 and IL-4 levels were higher in patients with NP with asthma than in patients with NP without asthma. Conclusion:Patients with NP show an exaggerated Th2 cytokine response of PBMCs to SEB.

        What is the Relationship between the Localization of Maxillary Fungal Balls and Intranasal Anatomic Variations?

        Objectives. Although the mechanisms underlying the initiation and maintenance of inflammation in unilateral maxillary fungal balls (FBs) are poorly understood, the relationship between intranasal anatomy and maxillary FB is thought to play an important role. The aim of this study was to investigate the relationship between anatomic variations and FB. Methods. We enrolled 140 patients who were composed of 56 patients with FB, 56 patients with unilateral chronic rhinosinusitis (CRS), and 28 patients with no sinus disease. Computed tomography scans were retrospectively analyzed to identify and compare the associated nasal anatomic abnormalities. To measure the volume of the nasal cavity and middle meatus, computed tomography scans were reconstructed into three-dimensional images. Results. The relatively larger volume of the middle meatus was associated with the localization of the FB in contrast with the CRS. However, the nasal-cavity volume, nasal valve area, and nasal septal deviation were not significantly associated with localization of FB. The mean volumetric and areal measurements such as nasal cavity, middle meatus, and nasal valve in FB-ipsilateral sides were not significantly different from those in contralateral sides as well as other groups. Conclusion. The middle meatus bears the major part of the inspiratory nasal airflow, and its volume may influence the occurrence of FB. Objectives.         수면호흡장애가 있는 소아의 타액 알파 아밀라아제수준과 심박동률 변이도 간 상관관계

        Background and Objectives: The aim of this study was to evaluate the relationship between salivary alpha-amylase (sAA) and heart rate variability (HRV) indices in SDB children based on objective parameters of polysomnography (PSG). Materials and Method: This prospective study enrolled 67 children who underwent a physical examination and full-attended in-lab PSG with continuous electrocardiographic signal. The sAA were measured at night before PSG and in the early morning after PSG. Results: The subjects were divided into control [n=26, apnea-hypopnea index (AHI)≤1] and obstructive sleep apnea syndrome (OSAS, n=41, AHI>1) groups; the OSAS group was subdivided into mild (1<AHI≤5), moderate (5<AHI≤10), and severe (10<AHI) groups. The severe OSAS group was significantly different from the control and other OSAS subgroups in terms of the ratio between the low- and high-frequency components (LF/HF ratio) among the HRV indices. The LF/HF ratio was positively correlated with the sAA ratio and sAA subtraction (r=0.271; r=0.347). Conclusion: TAlthough both HRV and sAA were useful methods of predicting severe OSAS in children, a weak correlation between HRV and sAA was shown in pediatric OSAS subjects. Therefore, HRV and sAA may be independent parameters revealing different aspects of pediatric OSAS.

        Practical Review of Olfactory Training and COVID-19

        Olfactory disorders one of the most frequent distinctive symptoms of COVID-19 infection. COVID-19-induced olfactory disorder can be classified as post-infectious olfactory dysfunction (PIOD). The effect of drugs on olfactory disorder following upper respiratory infection, including PIOD, has not been clearly established, which adds to the difficulty with treating the disorder. However, the effect of olfactory training on PIOD has been confirmed by numerous studies. As such, olfactory training is gaining attention, and has taken on greater importance, as the sole treatment for COVID-19–induced olfactory disorder in this pandemic age. This review describes the effect of olfactory training for COVID-19–induced olfactory disorder by analyzing the relevant literature.

      • 사석지반에 대한 쌍곡선 구성모델의 적용성 연구

        Until recently the other attempts except linear elastic analysis using assumed elastic modulus had not been made in order to evaluate the settlement of the rock fill materials in Korea. Especially, it was almost impossible to predict the precise settlement of the breakwater structure made with dumped rubble stone. In this study, 3 sets of large scaled triaxial compression tests for porous basaltic quarry rocks were carried out and numerical simulation of those triaxial compression tests were performed applying non linear elastic model. Two stress-strain behaviors were compared to study the adaptability of hyperbolic constitutive model for the rubble stone. The results showed quite good agreements between the two stress-strain behaviors. Thus, the hyperbolic constitutive model is thought to be alternative approach evaluate the settlements of the loose rock-fill material. 최근까지 우리나라의 암성토의 침하량을 산정하기 위해 가정된 탄성계수를 이용한 선형탄성해석 외에 다른 방법들이 시도되지 않고 있다. 특히 해상투기된 사석으로 구성된 방파제 구조물의 침하량을 정확히 산정하는 것은 거의 불가능하였다. 이 연구에서는 북제주의 채석장에서 채취한 3조의 다공질의 북제주의 화산암시료에 대해 대형삼축압축시험을 수행하였고 비선형모델을 이용하여 삼축압축시험을 수치해석으로 모사하였다. 실험과 해석에 의한 응력-변형률 거동이 비교되어 사석에 대한 하이퍼볼릭 구성모델의 적용성을 조사하였다. 비교 결과 두 응력-변형률 거동이 잘 일치하므로 하이퍼볼릭 모델은 느슨한 암성토체의 침하량을 산정하는데 적절한 것으로 판단되어진다.
