        지방자치단체의 홍수피해 응급복구 과정에서의 기술자원 활용방안

        본 연구에서는 홍수발생시 재해응급복구에 관한 장비조합의 적정성을 판단하고, 장비운용의 전문성을 가지고 있는 건설실무 인력자원을 활용하여 경제적이고 신속한 재해응급복구가 이루어질 수 있는 방안을 제시하였다. 홍수피해복구 과 정에 대한 절차와 방법을 조사하여, 현행 대처과정에 대한 이해를 정리하였고 이에 대한 법적근거를 조사•분석하였다. 지금 까지는 장비자원과 전문건설실무자원에 대한 효과적인 응급복구장비 투입과정에 대한 연구가 부족하여 응급복구시 비용의 과다지출 및 응급복구 지체상황이 발생하였다. 이에 따라 장비자원과 기술자원을 효과적으로 응급복구과정에 투입하여 개 선된 경험적 사례를 정리하고 이론적 근거를 조사, 분석하여 향후 효과적인 홍수피해 응급복구가 지방자치단체에 이루어질 수 있도록 하는 방안을 제시하였다. 이를 위해 복구장비의 응급복구투입에 따른 우선순위를 결정하고, 필요한 장비가 효율 적으로 활용되도록 재해지역의 이동거리와 연관성을 분석하여 활용방안을 제시하였으며, 이러한 전문 인력이 자치단체 산하의 지역자율방재단에서 적절한 기능을 할 수 있는 새로운 방안을 제시하였다. This study analyzed appropriate equipment combination for emergency recovery after flood disasters and proposed an alternative approach for economic and rapid emergency recovery based on the utilization of human resources who have practical experience in construction business and in equipment operation. The current countermeasures and response to flood disaster including emergency procedures and methodologies are investigated and related legal framework was also analyzed. Currently, some emergency recovery works are over payed and delayed due to unclear and inefficient processes on equipment and human resources mobilization during and after flood disasters, particularly in local governments. Best practices in mobilizing equipment and human resources were compiled, theoretical foundation was investigated and analyzed, and improved emergency recovery schemes were proposed for effective flood disaster countermeasures in local government. Prioritization in equipment and human resources mobilization was also proposed considering equipment transportation distance and accessability to disaster sites. This study also proposed improved ways to utilize experts in local areas in supporting community emergency response volunteers for disaster risk management.

        ST 비상승 심전도를 보이는 급성 관상동맥증후군 환자에서의 심근경색 조기 예측인자로서의 D-dimer의 유용성

        Purpose: To determine if D-dimer level is a supportive marker in the prediction of myocardial infarction in patients with acute coronary syndrome and non-ST elevation ECG, who arrive in the emergency room. Methods: We conducted a prospective, observational study of patients with acute chest pain that had occurred within 24 hours and who had non-ST elevation on ECG. They were managed according to the 2005 ACLS guidelines; the following clinical tests were performed: D-dimer, cardiac markers, and CAG. Final diagnoses were divided into 3 groups: myocardial infarction (MI), unstable angina (UA), and others (non-specific or angina). Results: Ninety-three (93) patients were included in this study. The mean D-dimer value was higher in patients diagnosed with MI (1.04±2.12 ug/dl) than in those patients diagnosed with UA (0.48±0.51 ug/dl) (p=0.026). At the 0.38 ug/dl diagnotic threshold estimated by ROC curve analysis, corresponding to the cutoff for diagnosis of myocardial infarction then in this value sensitivity and specificity of D-dimer were 70.6% and 62.7%. In cardiac enzymes, sensitivity and specificity of Tn-I were 47.1% and 98.3, respectively, and for CK-MB were 35.3% and 98.3%, respectively. Conclusion: D-dimer value may be at higher levels in AMI and the sensitivity of D-dimer was higher than that of TnI and CK-MB in patients with acute coronary syndrome and non- ST elevation ECG. However, because of relatively lower sensitivity and specificity of the D-dimer test, this test may useful as supporting marker for predicting myocardial infarction.

        응급실에 내원한 성인 환자들의 몸무게 추정에 있어 종아리 둘레 측정의 유용성

        Purpose: There is no reliable body index to use in estimating adult body weight. The purpose of the study was to develop a body index that can estimate adult body weight and to perform a regression equation of body weight versus the body index Methods: We performed a prospective study. We measured body weight and various body indexes, including arm, wrist, calf, and ankle circumference, in the patients who visited our emergency department. A correlation analysis was performed between these body indexes and body weight, and a simple linear regression analysis was used to examine the relationship between the most correlative body index and body weight. Results: Two hundred five patients were enrolled. The correlation coefficient between body weight and calf, arm, wrist, and ankle circumference were, respectively, 0.919, 0.792, 0.546, and 0.432 for males and 0.913, 0.781, 0.476, and 0.430 for females. The body index with the greatest correlation to body weight was calf circumference. The regression equations were: male body weight = 2.510×calf circumference - 25.673, and female body weight = 2.375× calf circumference - 22.560. Conclusion: The most reliable body index for estimating adult body weight is calf circumference.
