Histiocytoid cardiomyopathy, an extremely rare heart disease in infants and children, usually occurring in girls under the age of 2 years, is characterized by cardiomegaly, ventricular arrhythmia, and sudden death. In the present study, we report a case of a 2-day-old female neonate who suddenly showed cyanosis and cardiac arrest in the neonatal unit and died without resuscitation. Autopsy revealed multifocal nodular lesions in the left ventricle wall and papillary muscles. Microscopically, these lesions were composed of discohesive round or polygonal cells with eosinophilic cytoplasm and were positive for desmin and negative for CD68. Electron microscopy findings displayed abnormal swollen mitochondria with disorganized cristae, dense granules, and diminished myofibrils in the periphery of the cytoplasm.

In the following report, we describe the case of a rapidly skeletonized corpse discoveredin late summer. On the first day of September, a 57-year-old man was foundalmost completely skeletonized on a hill behind his house. His body lay prone, accompaniedby a brush cutter at the entrance to the graveyard where his friend’s parentswere buried. His cap, clothes, and rubber boots were all intact and undamaged. Uponautopsy, no definite abnormalities were identified except for an old linear fracture onthe frontoparietal bone. According to his friends and neighbors, he had likely died 8 or9 days prior to the discovery of his body.

Overlaying refers to mechanical asphyxia caused by an adult sleeping on an infant due to deep sleep or external intoxication. We analyzed seven cases (6.6%) of overlaying death out of 106 sleep-related infantile death autopsy cases from January 2018 to March 2021. They were all boys, with an average age of 132.3 (63-244) days, and were sleeping with their inebriated parents. In autopsy, only non-specific findings such as facial congestion, pressure marks, scratches, and froth in the nose and mouth were noted. In the sleep-related infantile death investigation, thorough investigation of the death scene and the previous history are essential for diagnosis, because autopsy findings are minimal or non-specific in most cases. Victims of overlaying and sudden infantile death syndrome show similar clinical and autopsy findings. Therefore, special attention are required for the differential diagnosis.

선천성 결합조직형성대뇌아교모세포종 - 1예 보고 -

Desmoplastic cerebral glioblastoma has been described recently and is a very rare histologic variant of glioblastoma. We report a case of congenital cerebral glioblastoma associated with intense desmoplastic stromal reaction. A male infant was born at 36 gestational weeks by Cesarian section. He had a brain tumor, which was detected by fetal ultrasonography. The tumor was partially resected 2 months after the day of the boy’s birth and totally resected when he was one year old. The microscopic features of the tumor were those of glioblastoma, including high cellularity, frequent mitotic figures, vascular endothelial proliferation, and geographic palisading necrosis. The tumor showed an area of intense desmoplasia where tumor cells were surrounded by dense reticulin fibers. The desmoplastic cerebral tumors in children may be a distinct group of brain tumor, and it is important to understand the entity of these tumors which generally seem to be associated with more favorable prognosis compared to other high grade brain tumors.

헬륨가스와 관련된 타살-자살 예: 부검 1예 보고

A 39-year-old man, his 35-year-old wife, and their 13-year-old daughter were founddead in their home. The man and his daughter were found lying in her bedroom, withhis head wrapped in a plastic bag connected to four helium gas cylinders by greenpolypropylene tubes. Fragments of the burnt ignition briquette were found in the bucketlocated at her feet. His wife was found in a decomposed state lying on her right sidein her bedroom. Autopsy findings for the man were unremarkable except for cherrypink lividity; the blood carboxyhemoglobin concentration was 73%. Autopsy findingswere unremarkable for the woman as well, except for a few petechial hemorrhagesand conjunctival congestion. The daughter showed no definite abnormalities; however,her blood and lung contained helium gas. The deceased man’s suicide note, evidencegathered at the scene, and postmortem examination revealed that this was acase of dyadic death.

100 kVp Low-Tube Voltage Abdominal CT in Adults: Radiation Dose Reduction and Image Quality Comparison of 120 kVp Abdominal CT

Purpose: To compare image quality and the effect of radiation dose reduction after decreasing tube voltage from 120 kVp to 100 kVp for abdominal CT in adults. Materials and Methods: A total of 200 patients who underwent abdominal CT at 120 kVp (n = 100) or 100 kVp (n = 100) were enrolled. Automatic tube current modulation was applied with other scan parameters being constant. Radiation dose was calculated based on CT dosimetry index. The image quality of abdominal organs and image noise were assessed quantitatively and qualitatively. Results: A radiation dose reduction of 13.3% was found in the 100 kVp group. On quantitative analysis, image noise was increased up to 47% in the 100 kVp group. CT numbers of liver, pancreas, renal cortex, aorta, portal vein, and psoas muscle in the 100 kVp group were significantly (p < 0.05) higher than those in the 120 kVp group. Signal-to-noise ratio was significantly higher (p < 0.05) in the 120 kVp group. Contrast-to-noise ratio (CNR) of the liver was higher in the 120 kVp group. However, no significant (p > 0.05) difference was observed in the CNR of other organs between the two groups. On qualitative analysis, noise texture of abdominal organs, artifact, and diagnostic acceptability were not significantly (p > 0.05) different. Conclusion: 100 kVp abdominal CT reduced radiation dose by 13.3% without sacrificing image quality compared to 120 kVp abdominal CT.

Esophageal foreign body impaction may be fatal secondary to occlusion of the glottis or external compression of the membranous portion of the trachea. A 14-year-old adolescent female was admitted to the emergency department for evaluation of sudden dyspnea and cyanosis after she swallowed a food bolus (a part of bachelor radish kimchi) without chewing. Unfortunately, the patient died despite cardiopulmonary resuscitation. Autopsy findings showed an ovoid, firm foreign body (3.5×2.5×2 cm) impacted in the proximal esophagus, with focal narrowing of the tracheal lumen secondary to external compression by esophageal foreign body impaction. Death was attributed to choking caused by an esophageal foreign body.

피클링솔트에 의한 아질산염중독: 4예 보고

Pickling salt, also known as curing salt, is a mixture of sodium chloride and sodium nitrite, which is used for color agent and a means to facilitate food preservation. Recently, online purchase of pure sodium nitrite has been restricted, and pickling salts have been used as replacements in cases of suicidal nitrite intoxication. From November 2020 to December 2021, there were four autopsy cases of nitrite poisoning caused by pickling salt, and 10 autopsy cases of nitrite poisoning by pure sodium nitrite. Due to the low nitrite concentration in pickling salts, serum nitrite and nitrate concentration, and methemoglobin levels were relatively low in pickling salts cases. Especially, low methemoglobin levels may cause confusion in the postmortem diagnosis of fatal nitrite intoxication, so caution is required.

A renal angiomyolipoma (AML) is an unusual benign tumor that is composed of adipose tissue, smooth muscle, and abnormal blood vessels. AMLs can occur sporadically or can be associated with tuberous sclerosis. A ruptured renal AML may manifest as lift-threatening hypovolemic shock or sudden death. Herein, the authors present an autopsy case of traumatic rupture of a renal AML in a patient with tuberous sclerosis.
