        계수공제영상 방사선 측정법을 이용한 치주판막술 후 치조골 변화의 평가

        Since $Gr{\"{o}}nadhl$ et al introduced a digital subtraction radiography into periodontal dignosis in 1983, many reports using this system has aimed to assess the peroiodontal disease activity and the alveolar bone changes after periodontal treatment. The present study was performed to evaluate the usefulness of digital subtraction radiography for asessing an alveolar bone changes in 3 months after periodontal flap surgery. Serial intraral raiographs were taken from 5 normal subjects and 6 periodontally diseased patients using customized bite blocks attached to film holder in fxation device and digitized by image processing system(consisting of IBM 386, digital frame grabber, CCD camera, Image-pro II software). And the reference parameters were measured by pixel unit and compared with respective radiographs. The serial radiographs showed a little and statistically insignificant difference in reference paramenters. The conventional intraoral radiographs, the subtraction images and the clor enhanced subtraction images were reviewed by 4 examiners and the examiner's agreement rates were compared. The subtraction images and its color enhanced images showed higher examiner's agreement rate than the conventional radiographs. And the propotions of sites diagnosed with bone loss or gain after periodontal surgery were highter in the subtraction images and its color enhanced images than in the conventional radiographs. Especially, in color enhanced images, the unber of bone agin sites tended to increase according to post-surgery periods. These results indicate that projection geometry could be standardized with the divice used in this study, and the subtraction radiography may be useful to assess an alveolar bone changes after periodntal flap surgery.

        결합조직 이식술을 이용한 노출치근면의 치은피개

        Patients, who have gingival recession and complain of root sensitivity, or esthetic concerns, are candidates for root coverage. When free gingival grafting is used for complete root corverage, the results may not be entirely predictible unless the recession is shallow and narrow because a free gingival graft depends on collateral circulation from the lateral and apical parts of the recipient bed to survive over the avascular root. Various pedicle graft techniques can produce more esthetic results, but these procedures are only indicated when adequate donor tissues are available adjacent to the defect. This case report presents three cases for root coverage using the various connective tissue graft techniques. In the first case(Class III & IV), subepithelial connective tissue grafting was done and resulted in gingival coverage on the two-thirds of exposed root surface and blended with the adjacent tissue in color and texture. In the second case(Class I), connective tissue and partial thickness double pedicle graft resulted in complete coverage of denuded root surface. In the third case(Class I), recession was treated by supraperiosteal envelope technique. The root surface was covered completely and esthetically. Finally, the esthetics in both colors and tissue contours were acceptable to patients in all cases by the connective tissue grafting. However, in the case of the reduced interdental bone, the denuded root surfaces were hardly covered completely.

        급속진행형치주염에서 치석제거술과 국소 약물송달제재의 병용효과

        Rapidly progressive periodontitis is known to be usually associated with systemic problems and improved with antibiotic therapy. Recent experiments in which bioresorbable polycaprolactone was polymerized with minocycline has shown that the system released effective antibiotic concentration during the 7 days' period. This clinical trial was to compare the efficacy of a minocycline film(poly-caprolactone+polyglycol+10% minocycline) insertion plus supragingival scaling(MS) or subgingival scaling & root planing(MSRP) with the scaling(S) or subgingival scaling & root planing alone(SRP), at improving the periodontal condition in RPP. Fifteen patients were examined for plaque accumulation, gingival inflammation, probing depth and attachment loss at baseline, then 1, 2, 4 and 8 weeks after 4 treatment regimens were randomly undergone in 4 comparable sites(PD>5mm, LA>3mm) in each subject. Results revealed statistically significant treatement effect with a reduction in a probing depth in SRP(2.0mm), MS(1.8mm), and MSRP(2.1mm). There was no significant reduction in the supragingival scaling alone group(0.6mm). Similarly, attachment levels were significantly improved in the SRP(1.5mm), MS(2.0mm) and MSRP(2.0mm) groups. Net % BOP reduction at 8 week compared to baseline was 6.7% (S), 26.7% (MS), 26.7% (SRP), and 33.3% (MSRP). MSRP produced the greatest improvement in BOP at 8 week. This data suggests that a subgingival minocycline delivery system as a adjunct to scaling alone or scaling & root planing may produce significant clinical benefits over scaling alone in rapidly progressive periodontitis patients.

        상악전치부에서 치아정출술을 이용한 치관연장의 증례보고

        This case report presents two maxillary anterior cases for clinical crown lengthening by forced eruption. In the first case, clinical crown of maxillary right lateral incisor was almost lost by fracture. Forced eruption using intracoronal splint and elastic thread accomplished vertical root movement successfully. Then, post & core was inserted and final restoration was harmonious with adjacent teeth. In the second case, the crown portion of maxillary right central incisor was almost mutilated by secondary caries. Forced eruption using removable Hawley appliance and elastic accomplished vertical root movement successfully. Then, post & core was inserted and final restoration was placed. In conclusion, clinical crown lengthening by vertical root movement can be accomplished by a simple appliance without any sacrifice of periodontal support in selected patients. A clinical crown so created can be restored to adequate function and arch integrity without compromising adjacent teeth. Therefore, forced eruption is preferred in the anterior region of the dentition where esthetics is of major concern.
