불안정성 쇄골 원위부 골절에 대한 Hook 금속판 고정술의 치료 결과

목적: 불안정성 쇄골 원위부 골절에 대해 AO Hook 금속판을 이용한 고정술을 시행하고 임상 및 방사선학적 결과를 분석 및 평가하였다. 대상 및 방법: 2009년 3월부터 2010년 10월까지 불안정성 쇄골 원위부 골절에 대해 AO Hook 금속판을 이용하여 수술한 16예를 대상으로 하였다. 임상적 평가로 UCLA 점수, KSS 점수을 이용하였으며, 방사선학적 결과는 최종 추시 단순 방사선을 이용하였다. 결과: 모든 환자에서 골유합을 얻었으며, 평균 골유합 시기는 13.9주 (9~20주)였다. 임상적 평가에서 UCLA 점수는 9예에서 우수, 7예에서 양호이었으며, KSS 점수는 평균 95.5점이었다. 최종 추시상 11예 (68.7%)에서 견봉하 골미란이 발생하였으며 그 중 3예에서 견봉 부위의 통증을 호소하였다. 다른 합병증으로는 금속판 근위부 골절 1예와 견봉하 충돌증후군과 관련된 관절 강직 2예, 절개창 주위의 감각저하 1예가 발생하였다. 결론: 불안정성 쇄골 원위부 골절에 대한 Hook 금속판 고정술은 견고한 고정과 함께 높은 골유합율을 얻을 수 있어 만족할 만한 결과를 보였지만, 금속판의 형태학적 특성으로 인한 합병증의 발생은 반드시 고려되어야 할 부분이라고 생각된다. Purpose: This study evaluated the clinical and radiological outcomes of unstable distal clavicular fractures treated with an AO Hook plate. Materials and Methods: From March 2009 to October 2010, sixteen patients with distal clavicular fractures underwent open plating using an AO Hook plate. The clinical outcomes were assessed by measuring the UCLA scores and KSS sores, and the radiological outcomes were evaluated using simple radiographs at the final follow-up. Results: Fracture union was obtained in all patients at an average of 13.9 weeks (range, 9~20 weeks). The UCLA scoring system showed excellent results in 9 cases and good results in 7. The average KSS scores of distal clavicular fractures were 95.5. At the final follow-up, subacromial osteolysis developed in 11 cases (68.7%) of whom 3 suffered from pain around the acromion. Other complications occurred in 4 patients: one had a fracture adjacent to the plate proximally, two had a stiff shoulder with subacromial impingement, and one had hypoesthesia around the surgical wound. Conclusion: Unstable distal clavicular fractures treated with a Hook plate provided rigid fixation and satisfactory outcomes considering the high union rate. Nevertheless, potential postoperative complications related to morphometric properties of the plate should be considered.
