In clinical setting, treatment-refractoriness, medication induced tardive dyskinesia and amenorrhea in chronic schizophrenia are frequently problematic. However, there are few guideline solving these problem available to clinicians. The goal of this study was collecting clinical data on clinical effectiveness and predictors of response of switching to olanzapine. We attempted to switch to olanzapine from risperidone and clozapine in chronic 31(risperidone 17, clozapine 14) schizophrenia and schizoaffective disorder patients suffering from sustained symptoms, weekly blood monitoring, medication induced tardive dyskinesia and amenorrhea. Previous antipsychotics dosage was gradually decreased for 2 or 3weeks, at the same time olanzapine dosage was gradually increased. At baseline, after 1 week, after 2 weeks and after 4 weeks we checked Brief Psychiatric Rating Scale, Clinical Global Impression Scale, Sympson-Angus Rating Scale, Barnes Akathisia Rating Scale and followed up after 12 months. Successful switch after 4 weeks was achieved in 25 patients(clozapine 9(64.2%), risperidone 16(94.1%)). Overall, mean BPRS and CGI scores increased significantly. Successful maintenance after 12 months was achieved in 17 patients(clozapine 5(35.7%), risperidone 12(70.5%)). Overall, mean BPRS and CGI scores increased significantly too. Switching to olanzapine from other atypical antipsychotics is recommendable in chronic schizophrenia with treatment refractoriness and drug induced side effect.

In shipbuilding industry, welding position are usually flat and vertical position at the erection stage. Application of SAW and EGW for these positions makes it possible to achieve enhanced productivity and high quality. But owing to their large size and weight it is difficult to apply these techniques in short and narrow regions. To overcome this problem, our company developed light weight and compact size 4-axis welding carriage which perform 3D weaving. The purpose of this study is to explain the development and application of intelligent welding carriage using 3D weaving pattern that can fill a large amount of welds and thereby making it possible to achieve high quality of welding. This study shows 3D weaving pattern, development of weaving database, and skill of adaptive control response for the variable gap. Also, it shows the results of procedure qualification test for the AH-grade steel when applied to the intelligent welding carriage.

Nefazodone, a newer antidepressant is a phenylpiperazine derivative that inhibits the reuptake of both norepinephrine and serotonin, and antagonizes $5-HT_{2A}$ and ${\alpha}_1$ adrenergic receptors. Compared with SSRIs, nefazodone caused the fewer activating symptoms, adverse gastrointestinal effects(nausea, diarrhea, anorexia) and adverse effects of sexual function, but is associated with the more dizziness, dry mouth, constipation, visual disturbances and confusion. We report on 4 cases of visual disturbances and hallucinations in patients taking nefazodone. It is not certain what mechanisms mediated these side effects, but three mechanisms are possible. 1) Nefazodone, as a 5-HT2 antagonist, might induce visual disturbances. 2) mCPP, metabolite of nefazodone might contribute to the hallucination through action on 5-HT receptor. 3) Dopaminergic enhancing activity of nefazodone might cause hallucination. These case report raises the possibility that dose-related perceptual disturbances may exist with nefazodone. The fact emphasizes the need to pay close attention to all possible drug interactions, particularly in patients treated with multiple psychoactive agents, older patients, and patients with decreased hepatic function.

A 18 year-old male patient visited hospital due to psychotic symptoms. He had been sniffing volatile inhalants since 3 years ago, but he had quit sniffing 10 months ago. However he has reexperienced visual and auditory hallucinations without sniffing inhalant since 8 months ago. He had no family history of schizophrenia. The psychiatric examination revealed that he had positive symptoms of schizophrenia, such as visual hallucination, auditory hallucinations, delusion of being controlled, delusional perception, and somatic hallucination/delusion, but did not show negative symptoms. It was difficult to diagnose this case as schizophrenia or inhalant-induced psychotic disorder. This case may suggest that the positive symptoms are important manifestations of psychotic disorder due to volatile inhalant abuse. We also suggest that the psychotic symptoms of chronic inhalant abuse are serious and potentially irreversible.
A Study on the Structural Integrity of an Auxiliary Feed Water Pump in a Nuclear Power Plant

The auxiliary-feed-water pump (AFWP) used to supply water during a station black out situation at nuclear power plants should meet the seismic qualification regulations stipulated in IEEE Std 323 and 344, so as to withstand earthquakes or dangerous situations. Here, we establish a model for the estimation of the structural integrity of this type of pump. If the natural frequency that results from a modal analysis is less than 33 Hz, we adopt a dynamic analysis, instead of a static analysis. A dynamic analysis was carried out taking into consideration seismic conditions such as the floor response spectra (FRS), an operation-base earthquake (OBE), and a safe-shutdown earthquake (SSE). Finally, an analytical estimation of the structural integrity of an AFWP is made through a comparison of calculated values and allowable values. If the result is less than the allowable stress, the pump is deemed to have good structural integrity. In addition, future studies will involve a stability check for rotor accidents that may occur during the operation of the pump.

The current study of commoner's house was exposed to the limiation because of inter preting the whole to the subject of the only special building at a specific region and selecting the representative value from random sample without common factor. I consider extracting the results from the same cultural area or the same zone of life as soluting method to the fundamental problem in the history of Korean commoner's house.
장기간 Risperidone 투여후 지연성으로 발생한 Neuroleptic Malignant Syndrome 1례

Risperidone은 비교적 최근에 개발된 항정신병약물이며, 그것이 가진 도파민 D2 수용체와 세로토닌2 수용체를 차단하는 작용기전, 추체외로 부작용과 지연성 운동장애가 적다는 점, 정신분열병의 음성증상에도 효과가 있다는 점에서 흔히 비전형적 약물로 불리운다. 그러나 이러한 비전형성에도 불구하고, 이미 전세계적으로는 15개 정도의 risperidone에 의한 신경이완제 악성증후군(neuroleptic malignant syndrome : NMS)의 보고가 있으며. 국내에서도 최근 하나의 증례가 보고되었다. 저자들은 정신분열병으로 장기간 risperidone으로 치료받던 젊은 남자 환자에서 치료 1년 9개월만에 발생된 NMS 1례를 경험하였기에 관련문헌의 고찰과 더불어 증례보고하고자 한다. Risperidone is a relatively new antipsychotic agent. It is often referred to as 'atypical', because it has a mechanism of action that blocks post synaptic dopamine-2 and serotonin-2 receptors, it is associated with fewer extrapyramidal side effects, it is not yet associated with tardive dyskinesia, and it may have some efficacy on negative symptoms of schizophrenia. In despite of its 'atypical' nature, there are already more than 15 reports of risperidone-induced neuroleptic malignant syndrome. Only one case of risperidone-induced NMS was reported recently in Korea. We report one case of delayed risperidone-induced neuroleptic malignant syndrome in young male patient and review the related articles.
SMT사용공장의 바닥 및 천장 방음시공에 따른 바닥충격음 저감 효과에 관한 연구

This study aims to solve the problem of noise and vibration transmitted to adjacent room especially to the downstairs when using SMT(Surface Mount Technology) equipments To achieve this, a kind of system for preventing noise and vibration was applied, and the effects of sound insulation performance for those alternatives were tested and analyzed To reduce floor impact noise, floating floor system using a plastic pallet and rubber was applied The applied plastic pallet is the size of 30cm wide ? 60cm deep ?6cm high and designed to array on a concrete slab, above this plastic pallet, a 14-cm thick concrete was constructed After installation of this system the floor impact sound was reduced about 7dB(A) For more reduction, a ceiling with a 5-cm thick glass wool and twofold of 0.9-cm thick plywood was constructed at the directly below downstairs According to this, 4dB(A) was additionally reduced Based on these results, the design guidelines were suggested to reduce the impact noise generated by using SMT equipment
