N/A Melanosis coli associated with ingestion of anthraquinone compounds in laxatives is due to accumulation of macrophages containing brown lipofuscin-like pigments in the lamina propria. We analyzed the clinical, endoscopic, and pathologic findings and colonic motor functions of melanosis coli and performed electronmicroscopic study to know the ultrastructural changes of neural elements. We studied 13 cases (mean age 48.1 +- 18.5 years, male to female cases 6:7). The vegetable laxatives containing anthraquinone derivatives, aloe or bisacodyl were used mainly and the mean duration of laxative abuse was l.5 +- 1.2 years. Endoscopically, dark brownish mucosal disoloration showed polyhedral forms divided by fine striae of lighter color. Melanosis was more intense in the more oral part of the bowel. Pathologically, no relationship was found between grade of melanosis and kind of drugs, daily dose, or durat.ion of addiction. Ultrastructural features on nerve structure were ballooning of axon. reduction of nerve specific cell organelles, and increased lysosomal activity. The alterations of these neural elements are assumed to he associated with impaired colonic motility. In conclusion, vegetable laxatives such as aloe, seem to be an important causes of melanosis coli in Korea. melanosis coli have a tendency to damage nerve fibers. We should avoid laxative abuse in order to prevent pragressing to cathartic colon.

Background/Aims: Conventional triple therapy (CT) for Helicobacter pylori infection fails in up to one-third of patients. Sequential therapy (ST) seem be more effective than CT in other countries. However, there is no systemic literature review that directly compares CT and ST in Korea. The aim of this study was to compare ST with CT for H. pylori infection in Korea. Methods: Six randomized, prospective controlled trials were used to compare 10-day ST and 7- to 14-day CT in treatment-naive patients with documented H. pylori infection in Korea. Pooled eradication rates and OR with 95% CI were calculated. Results: The intention-to-treat eradication rates of H. pylori involving 1,529 patients were 79.7% (95% CI, 76.8-82.5%) for ST (n=754) and 68.1% (95% CI, 64.8-71.4%) for CT (n=775) (OR, 1.838; p<0.001). The per-protocol eradication rate of H. pylori involving 1,366 patients was 86.4% (95% CI, 83.3-88.5%) for ST (n=682) and 76.0% (95% CI, 72.8-79.2%) for CT (n=684) (OR, 1.974; p<0.001). Conclusions: Ten-day ST was superior to CT in terms of eradicating H. pylori infection. Therefore, ST should be considered as a first-line therapy in Korea. However, ST did not achieve a sufficient eradication rate. More effective therapy should be developed. (Korean J Gastroenterol 2013;62:267-271)

EPON is proposed to be used in an infrastructure of the next generation wireless network. EPON can reduce fiber deployment cost and easily decrease the occurrence of layer-3 handover by introducing group cell concept. An EPON-based layer-2 handover algorithm is suggested for fast handover. The time-delay of the control signals are examined using simulation. 차세대 무선망의 인프라 구조로써 현재 광가입자망에 적용되고 있는 EPON의 사용을 제안하였다. 제안된 구조는 광파이버 설치비용을 크게 줄일 수 있고, 그룹 셀 개념을 도입하여 핸드오버 지연시간의 주 발생원인 3계층 핸드오버의 발생 횟수를 대폭 줄일 수 있다. 본 연구에서는 EPON의 특성을 활용한 2계층 Fast 핸드오버 알고리즘을 제안하였고, 이 때 핸드오버에 큰 영향을 미치는 제어신호의 지연시간을 시뮬레이션을 통해 평가하였다.

Dysphagia occurs frequently as an accompanying sign of neurodegenerative disorders. Degeneration of the dorsal vagal motor nucleus may cause esophageal dysphagia. Multiple system atrophy (MSA) is a progressive neurodegenerative disorder of unknown etiology. Dysphagia is one well-recognized complication of MSA and is closely related to the prognosis of patients. However, the swallowing function of patients with MSA has not been explored systematically. We experienced a 73-year-old male who had recurrent syncope with dysphagia. Neurologic examinations revealed a cerebellar dysfunction and autonomic function tests were abnormal. We diagnosed this case as MSA. The esophagogastroscopy was normal and the esophageal manometry revealed low pressure of the upper esophageal sphincter and lack of normal peristalsis. Barium esophagography showed ineffective primary peristalsis and weak tertiary peristalsis. We considered that diagnosis of dysphagia caused by degeneration of autonomic cranial nerves. Although benign gastric ulcers do not develop into gastric cancer, they are similar to early gastric cancer and difficult to distinguish. This may lead to misconsider early gastric cancer as gastric ulcer while diagnosing. Since gastric cancer does not have any special symptoms until discovered, it is important to detect gastric ulcers by early gastroscopy to prevent the gastric cancer. Therefore, we developed a Convolution Neural Network (CNN) model that can be helpful for endoscopy. 3,015 images of gastroscopy of patients undergoing endoscopy at Gachon University Gil Hospital were used in this study. Using ResNet-50, three models were developed to classify normal and gastric ulcers, normal and gastric cancer, and gastric ulcer and gastric cancer. We applied the data augmentation technique to increase the number of training data and examined the effect on accuracy by varying the multiples. The accuracy of each model with the highest performance are as follows. The accuracy of normal and gastric ulcer classification model was 95.11% when the data were increased 15 times, the accuracy of normal and gastric cancer classification model was 98.28% when 15 times increased likewise, and 5 times increased data in gastric ulcer and gastric cancer classification model yielded 87.89%. We will collect additional specific shape of gastric ulcer and cancer data and will apply various image processing techniques for visual enhancement. Models that classify normal and lesion, which showed relatively high accuracy, will be re-learned through optimal parameter search.

Stomach cancer is the most diagnosed cancer in Korea. When gastric cancer is detected early, the 5-year survival rate is as high as 90%. Gastroscopy is a very useful method for early diagnosis. But the false negative rate of gastric cancer in the gastroscopy was 4.6~25.8% due to the subjective judgment of the physician. Recently, the image classification performance of the image recognition field has been advanced by the convolutional neural network. Convolutional neural networks perform well when diverse and sufficient amounts of data are supported. However, medical data is not easy to access and it is difficult to gather enough high-quality data that includes expert annotations. So This paper evaluates the efficacy of transfer learning in gastroscopy classification and diagnosis. We obtained 787 endoscopic images of gastric endoscopy at Gil Medical Center, Gachon University. The number of normal images was 200, and the number of abnormal images was 587. The image size was reconstructed and normalized. In the case of the ResNet50 structure, the classification accuracy before and after applying the transfer learning was improved from 0.9 to 0.947, and the AUC was also improved from 0.94 to 0.98. In the case of the InceptionV3 structure, the classification accuracy before and after applying the transfer learning was improved from 0.862 to 0.924, and the AUC was also improved from 0.89 to 0.97. In the case of the VGG16 structure, the classification accuracy before and after applying the transfer learning was improved from 0.87 to 0.938, and the AUC was also improved from 0.89 to 0.98. The difference in the performance of the CNN model before and after transfer learning was statistically significant when confirmed by T-test (p < 0.05). As a result, transfer learning is judged to be an effective method of medical data that is difficult to collect good quality data.

Background/Aims: There is increasing need for third-line therapy of Helicobacter pylori due to increasing level of antibiotics resistance. The aim of this study was to compare rifabutin and levofloxacin rescue regimens in patients with first- and second-line Helicobacter pylori eradication failures. Methods: Patients, in whom a first treatment with proton pump inhibitor-clarithromycin-amoxicillin and a second trial with proton pump inhibitor-bismuth-tetracycline-metronidazole had failed, received treatment with either rifabutin or levofloxacin, plus amoxicillin (1 g twice daily) and standard dose proton pump inhibitor. Eradication rates were confirmed with 13C-urea breath test or rapid urease test 4 weeks after the cessation of therapy. Results: Eradication rates were 71.4% in the rifabutin group, and 57.1% in the levofloxacin group, respectively. Although there was no significant difference in Helicobacter pylori eradication rates between two groups (p=0.656), rifabutin based regimen showed relatively higher eradication rate. Conclusions: Helicobacter pylori eradication rates of rifabutin- or levofloxacin-based triple therapy could not achieve enough eradication rate. Further studies would be needed on combination of levofloxacin and rifabutin-based regimen or culture based treatment. (Korean J Gastroenterol 2012;59:401-406)
동결건조 갈색거저리 유충의 제조공정 표준화에 따른 자가규격 및 유통기한 설정

본 연구에서는 갈색거저리 유충의 식품등록 후 산업화에 대비하여 대량생산 시설에 적용 가능한 동결건조 갈색거저리 유충곤충의 제조공정을 확립하였다. 갈색거저리 유충을 1 ~ 2일간 절식시킨 후 식용을 목적으로 하므로 위생적인 측면을 고려하여 분변제거 및 세척한 후 고온 · 고압 살균법으로 살균하였다. 살균시 용기 내에 부직포(48 cm × 55 cm)를 깔고 갈색거저리 투입량은 1 kg(3 cm)이하로 하는 것이 세균 및 진균의 제거에 효율적임을 확인하였다. 또한 생산효율을 증가시키기 위하여 다단식 살균기를 사용하여 각 단 사이의 공간을 10 cm 이상 확보하여 115oC, 1 kgf/cm2에서 10분간 고온 · 고압 살균한 경우 미생물이 검출되지 않음을 확인하였다. 살균한 갈색거저리 유충 동결건조를 위해 동결은 -35oC 이하에서 용기[W(63 cm) × L(57 cm) × H(3 cm)]별 4 kg씩(두께 1.5 cm 이하) 투입하여 실시하고, 건조는 -15oC 이하, 0.5 torr 이하에서 최소 30시간 이상 건조하는 것이 효율적인 것으로 판단하였다. 동결건조 갈색거저리 유충의 자가규격으로 수분 5% 이하, 산가 3 mg/g 이하, 과산화물가 30 meq/g 이하, 조단백 45% 이상, 조지방 25% 이상, 올레산 11 ~ 16%를 허용범위로 설정하였다. 마지막으로 이화학적 특성, 관능적 특성, 미생물학적 지표 조사를 통한 유통기한 설정 결과 PET 재질 용기로 포장될 경우 동결건조 갈색거저리 유충의 최종 유통기한은 제조일로부터 12개월로 결정하였다. 따라서 본 연구를 통해 확립된 대량생산 시설에 적용 가능한 동결건조 갈색거저리 유충의 제조공정을 통해 식품등록 후 산업화시 보다 위생적이며 균일한 제품 제조가 가능할 것이고, 자가규격 및 유통기한은 제품 관리에 응용되어 보다 안전성 및 안정성 확보가 가능할 것이라고 사료된다. This study was carried out to establish the self-specification and shelf-life by standardization of manufacturing process for Tenebrio molitor larvae. First, standardization of manufacturing process for T. molitor was set up. Sterilization for larvae placed on a multistage shelf with intervals of about 10 cm was carried out at 115oC, 1 kgf/cm2 for 10 min. After sterilization, T. molitor larvae were frozen at less than ?35oC for more than 12 hrs. And then, they were dried under ?15oC, 0.5 torr vacuum for more than 30 hrs. Second, we decided self-specification for T. molitor larvae. Their moisture, acidity, peroxide, crude protein and crude fat level should be 5% or less, 3 mg/g or less, 30 meq/g or less, 45% or more, and 25% or more, respectively. Also, oleic acid, representative material, level was set up 11 ~ 16%. Third, we decided shelf-life by analysis of the physicochemical characteristic, sensory evaluation and microbial indicators. The final expiry date for lyophilized T. molitor larvae in PET bottle was calculated as 12 months from date of manufacture. We expect that optimal manufacturing process system, self-specification, and shelf-life proposed in this study can be used in industrial production of T. molitor as a novel food.

Mucosa-associated lymphoid tissue (MALT) lymphoma is a typical primary gastrointestinal lymphoma, particularly in the stomach. Although primary rectal lymphoma is rare, it may present as a subepithelial tumor. Several techniques have been proposed for a tissue diagnosis in subepithelial tumor, including endoscopic ultrasonography (EUS)-guided fine needle aspiration (EUS-FNA), EUS-guided trucut biopsy (EUS-TCB), and tacked biopsy. However the diagnostic efficacy of these techniques appears to be limited. The unroofing technique involves removal of the overlying mucosa, thereby exposing the subepithelial lesion. It was originally reported as a method for endoscopic treatment of colorectal lymphangioma. In this case, a subepithelial tumor of the rectum was diagnosed using the endoscopic unroofing technique. This is a useful modality for the diagnosis of subepithelial tumor, because it provides histologic results in a safe and rapid manner. (Korean J Gastroenterol 2012;59:428-432)
