        급성 경막하 혈종 : 224 수술례 분석 An Analysis of 244 Operated Cases

        This study is a retrospective clinical analysis of two hundred fourty-four consecutive cases of acute subdural hematoma which were confirmed by operation during the last five years at the Department of Neurosurgery of the Dong-guk University Hospital. The authors have attempted, through this analysis to identify the factors affecting the outcome of acute subdural hematoma. A favorable outcome occurred in 40%, and an unfavorable outcome in 21% of patients at the time of discharge whilea death ocurred in 39% of the cases. We have concluded that initial GCS. pupillary status, systolic arterial blood pressure, FDP vaules, presence or absence of skull fracture. lICP findings on brain CT, hematoma amount and associated intracranial lesions are ail prognostic factors, although the timing of operation did not reflect a significant difference in the outcome, however the sooner the intracranial hematoma lesion is evacuated, the better the outcome will be.

        해수전처리를 위한 침지식 정밀여과 멤브레인 시스템에서 Alginate 파울링의 이해

        Organic fouling observed in submerged membrane filtration as a pretreatment for seawater desalination increases energy consumption for membrane operation because of requiring frequent chemical cleaning and membrane replacement. In membrane pretreatment for seawater facing with algae blooms, membrane fouling was observed in submerged microfiltration using sodium alginate model compound which is one of the main components of extracellular polymeric substances. Without aeration, aglinate fouling increased with its concentration while aeration reduced the alginate fouling effectively regardless of its concentration tested. In the absence of aeration, alingate fouling tended to be decreased with increasing calcium concentration. However, this effectiveness was reduced by increasing sodium chloride concentration. At high concentration of sodium chloride and calcium similar to the seawater conditions, aeration reduced initial fouling. However, as time progressed, the effect of increased airflow rate on fouling reduction was not significant, implying that optimum airflow rate to control alginate fouling in submerged microfiltration can exist. 저압 침지식 멤브레인을 이용한 해수 담수화 전처리에 있어서 유기 파울링은 막간 압력 증가로 인한 화학 세정 횟수의 증가 및 에너지 소비 증가 등 멤브레인 운전시 문제점들을 야기한다. 조류대응 해수전처리에서 조류가 배출하는 extracellular polymeric substances의 대표물질인 sodium alginate를 이용하여 침지식 여과에서 파울링 현상을 관찰하였다. 공기폭기가 적용되지 않은 경우 순수한 aglinate 파울링은 농도가 증가하면서 증가하였다. 그러나 공기 폭기를 적용해 준 경우 alginate 파울링 감소는 매우 효과적이었다. 공기 폭기가 없는 경우 칼슘 농도의 증가에 따라 alginate 파울링은 감소하였다. 동일 조건에서 공기 폭기 시 높은 alginate 파울링 감소효과를 나타내었으나 NaCl 농도를 증가시킨 경우 칼슘 농도의 증가에 따라 파울링 제어를 위한 공기 폭기 효과는 감소하였다. 해수와 유사한 높은 NaCl과 칼슘 농도에서 고농도 sodium alginate의경우 공기 폭기량 증가를 통해 초기 파울링을 감소시킬 수 있었으나 시간의 경과에 따라 상대적으로 낮은 폭기량에서의 파울링 감소 효과와 큰 차이는 없었다.

        Influence of sodium reduction and storage temperature on the growth of total microbes and Bacillus cereus in naturally contaminated hamburger patty and loaf bread

        This study aimed to investigate the influence ofsodium reduction and storage temperature on the growth oftotal microbes and Bacillus cereus in naturally contaminatedhamburger patty and loaf bread, respectively. Thesodium reduction rate of hamburger patty and loaf breadwas 20% and 30%, respectively, and experimental sampleswere kept at 4 C, 25 C, and 40 C for 60 h. Themicrobiological analysis included the colony count of totalmicrobes and B. cereus. The water activity (Aw), titratableacidity (TA), and pH were assessed as factors thatinhibit microbial growth. In this study, Aw, TA, and pH ofall samples were affected by the growth of total microbesand B. cereus during the storage period. Hence, theseresults suggested that sodium reduction in processed foodsshould be preferentially applied as a potent inhibitionstrategy after accurate assessment of inhibitors for differentfood types.

        Judo has not been ready to accept the ideology like a spiritual leap, which can be only understood as strategic interconnection concerning a specific technique or the issues in a technical training. It is the reason why Judo has been just developed, on a bass of accepting the notion and culture of modernly westernized sports and it hasnt significantly recognized a real ideology and value on techniques of Eastern traditions deeply related to Judo trainings. Therefore, we should find out a specific value of trainings as first recognizing consstency with methodology and objective logics that are able to attain by an inevitable relativity during trainings, while working to seek values immanent in martial arts for an advisable development. It is consequently considered to activate Judo based on educational and philosophic training cultures of martial arts through this recognition.

        Cetuximab 주입관련반응의 발생 빈도 및 관련 인자 분석

        Monoclonal antibody treatments can result in severe infusion reactions. Among these, cetuximab, mouse-human chimeric monoclonal antibody reported 15~20% infusion reactions (severe infusion reactions 3~5%). However, there are no actual reported incidences in Korea, and little information regarding the prevention, management or outcomes of infusion reactions. This study represents one of the first attempts to describe the clinical consequences regarding the incidence rates of infusion reactions, effect of premedication and test dose. A retrospective chart review was conducted in Asan Medical Center. Eligible patients were treated with cetuximab as either single agents or in combination with additional chemotherapy. Case searches were conducted by electronic medical records from June 2006 to October 2010. Source documents were evaluated for infusion reactions based on the Common Terminology Criteria for Adverse Events version3.0 (CTCAE v3.0). A total of 336 patients (215 men and 121 women) with the mean age of 56 years (27-76 years) were enrolled in this study. 11 patients were identified with infusion reactions. Mild and moderate infusion reactions are 9 (≤grade2, 3.3%) and severe infusion reactions are 2 (grade4, 0.6%). In premedication, 7 of 285 patients (2.5%) who received antihistamine were identified with a infusion reaction, while 4 of 51 patients (15.2%) who received antihistamine and corticosteroid (p=0.63). 4 of 84 patients (4.8%) treated with monotherapy and 7 of 252 patients (2.8%) treated with combination therapy suffered from infusion reaction (p=0.712). Patients with the administration of test dose (n = 49) experienced infusion reactions were 5 people. Further, 3 of these 5 patients (60%) were identified with infusion reactions at the main dose, not test dose. In addition, 6 patients of the past 2 mg/ml formulation with the unfiltered group (n = 288) (2.1%) and 5 patients of the present 5 mg/ml of the formulation was filtered (n = 48) (10%) were identified with a infusion reactions (p=0.915). It was difficult to find significant factors relating to the infusion reactions because incidences of cetuximab infusion reactions were lower than the previously reported. However, we found that antihistamine with corticosteroids as a premedication tended to reduce infusion reactions and the test dose is not valid.
