        전 복강경적 간절제술

        Purpose : The aim of this study was to determine whether liver resection treated by using laparoscopy is equally safe for patients who have benign liver mass, hepatolithiasis, hepatocellular carcinoma (HCC), of metastatic liver cancer. Methods : We performed our study in 32 patients with the following conditions: 17 patients with IHD stones, 7 with HCC, 1 with cholangiocarcinoma, and 7 patients who had colon cancer with liver metastasis. On a retrospective bases, these patients were investigated for clinical history, type of operation, time for operation, hospital stay, and post operative complications. Results : The target age range was from 37 to 80 years and the mean age was 61.2 years old. The percentage of patients over 60 years old was 68.7% According to their past history, 8 patients had experienced an upper abdominal operation. In our study there were 9 cases of left hepatectomy(28.1%), 9 cases of left lateral segmentectomy (28.1%), 2 cases of sectionectomy, and 12 cases of wedgeresection. The average operation time was 364±148 mins, the average bleeding amount was 417cc, and the average hospital stay was 13.2 days. There were 8 cases(25%) of postoperative complications developed (2 bile leak, 1 intrabdominal abscess, 1 acute renal failure, 2 wound infection). All cases were improved with conservative management. And there was one case of remnant IHD stone, which had surgical treat and one patient had peritoneal carcinomatosis, who had laparoscopic liver resection for HCC. Conclusion : Our study showed that laparoscopic liver resection has advantages; it is less painful, causes a small operative scar, a the short period of hospital stay and has cosmetic benefits. In the future, laparoscopic liver resection could be an important option for the treatment of hepatolithiasis and HCC through improved surgical instrument and skill of operation.

        119 구급대의 교통사고 예방 및 피해 경감 방안

        In 2021, the National Fire Agency recorded 141 traffic accidents involving 119 ambulances. A common scenario involves collisions at intersections, where ambulances often running into a red light intersect with vehicles proceeding with a green light. These accidents entail special attention because of the potential threats they pose not only to emergency medical service (EMS) personnel but also to the safety of patients and other vehicles on the road. Many studies have emphasized that it is necessary to reinforce the priority rights of ambulance and the duty of other vehicles to yield. However, research addressing challenges faced by ambulance drivers has remained insufficient. This study conducted a survey of EMS personnel about their psychological status while driving an ambulance and the use of warning lights and sirens. Prevention of traffic accidents related to 119 ambulances entails data collection and statistical analysis, reinforcement of professionals, expansion of emergency vehicle priority signaling systems, and development of simulator-based training programs. Policies that do not reflect ambulance drivers’ psychological state and driving patterns might turn out to be ineffective. 소방청 통계에 의하면 119 구급차(이하 “구급차”라 한다)와 관련된 교통사고는 2021년에 141건이 발생하였다. 대표적인 유형은 교차로에서 적색등화에 진입한 구급차와 녹색등화에 진행한 상대방 차량이 충돌하는 것이다. 구급차의 교통사고는 구급대원뿐만 아니라 환자와 보호자, 그리고 상대방 차량에 승차한 인원의 생명과 안전을 위협한다. 「도로교통법」상 긴급자동차인 구급차의 우선 통행 및 특례를 강화하여야 한다는 선행연구는 다수 있었다. 하지만 구급차 운전자를 대상으로 한 연구는 부족하였다. 본 연구는 구급대원을 대상으로 구급차 운전 중의 심리상태, 경광 등 및 사이렌 사용실태 등에 대한 설문조사를 실시하였다. 구급차와 관련된 교통사고를 예방하고 피해를 경감시키기 위해서는 데이터의 수집 및 통계분석, 전문인력의 보강, 긴급자동차 우선 신호 시스템의 확대, 시뮬레이터에 기반한 교육훈련 개발, 구급차의 안전에 관계되는 옵션 강화가 필요하다. 특히 이에 대한 정책개발 과정에 구급차 운전자의 심리상태와 운전패턴을 반영하여야 한다.

        고조선 ‘본풀이’의 역사인식과 본풀이사관의 수립

        The purpose of the study was to make classification of the times new by recognizing early describing style of history as ‘Bonpuri’ and establishing Bonpuri concept of history through disclosure of historical recognition about Bonpuri, starting from meta history questioning about history of historical description. The study reanalyzed ‘The birth myth of Korea’ handed down as an oral history by ‘Bonpuri’, a Korean language of myth, by re securing the position of the prehistoric age called until now in the historic world as the oral historic age and reviewing values of oral history and living history. The study newly captured identification and history of Sinsikoguk before Kojoseon by dividing ‘the birth myth of Kojoseon’ called as ‘Dangun Myth’ into Sinsi myth of Hwanung and Kojoseon myth of Dangun according to descriptive methods of myth. Double structure of Bonpuri was exposed to the birth myth of Koguryeo and Silla and escaped from the existing linear analysis of myth. Therefore, the traditional theory of Nordic myth can also be overcome critically by analyzing a diachronic flow of continuous and accumulative history as the logic of Bonpuri. Bonpuri concept of history is a cosmic and global history and includes the past history, the present history and the future history as a general history. More key point is to set up a desirable history of the future and to make the present history move toward the direction. To be so, we shall spread the timely classification of history to the future, differing from now, and propose value to have to pursue. Such historic recognition is reflected in Gut oral?.narrating Bonpuri. Therefore, Bonpuri concept of history can make classification of the times new with incantation, art, revolution and symbiosis which are characteristics of Gut. We can sort the times into ‘Incantation age’ recognizing and settling realistic problems in ways of religion and incantation, ‘Art age’ depending on literary recognition and countermeasure about the world to overcome limitation in settling problems in a way of incantation, ‘Revolution age’ settling problems in social scientific recognition and countermeasure about the world to overcome limitation in artistic expression and ‘Symbiosis age’ recognizing the world and settling problems in a biological way. If the modern is revolution age, the future is symbiosis age. Therefore, it goes on making the future of mankind progressive to make eco environmental culture according to the value of symbiosis. The Bonpuri concept of history is historical recognition by Bonpuri describing history and a historical view depending on historic functions. First, it highly appreciates the value of oral history and overcomes a historical recognition of the times called as the prehistoric age through the times of oral history. Second, to raise understanding, handing down power and reliability of historical knowledge, we describe history with interest in sacred stories. Third, we secure broad diachronic recognition and cosmic spatial recognition from history of the beginning of the world to historical view of the future. Fourth, we always find clues to settle problems in history by describing history till the viewpoint of the present and describing history again where realistic problems take place. Fifth, we propose the future history as an alternative plan to settle structural contradiction of the reality and to lead the future of community to a prosperous way. At this time, it is possible to establish a productive history going on making history.

        민족주의적 스포츠에 관한 재조명

        Sports has been the unifier of people` s energy that brought together a country by creating a feeling of solidarity. Also, it was this country that brought forth the Olympics, World Cup, etc. In other words, sports was able to settlement nationalism and develop through nationalism Sports helped enhance patriotism, and helped in bringing together a country as one. However, within sports, a exclusivism exists. This exclusivism is the unwillingness to understand and acknowledge other countries, and to consider other countries` as below one` s own. As pointed out above, sports can bring a nation together, but it can also lead the nation to view other countries with exclusion. Amongst these things, what meaning the pureness of sports has for us is being focused on. It is true that `we` and not `they` has been an important part in our values, and still is so in our reality. A nation is an unusual area inhabited by one ethnic group with one language and long history and tradition. Therefore, the pride in a unitary people by the Koreans have been one of the forces which helped the people survive through the rough history. In other words, the characteristic of the Koreans is that its is the solution to danger. This doesn`t mean that the Korean nationalism has a rebellious side to it. It means that nationalism was incorporated by the government when the nation was in danger. Therefore, the goal of this research is to re-viewed the nationalism in sports, and also look not only at the structural factor but also create a new healthy perception about nationalism Our problem is not getting rid of nationalism itself, but to create a citizen`s national identity. The fact that a correct understanding of nationalism is needed to save our sports is a reason why we need to take another look at nationalism It is foolish to attempt to achieve this without knowing, first what role sports has played in our society throughout history.

        민속예술 비교연구의 준거와 비교모형 설정

        A comparative study on folk are is classified into six according to purpose, target and relation. For the purpose, there is a comparative study on A)diffusion theory and B)corresponding theory, for the target there is a comparative study on C)culture theory and D)work theory, and according to the researcher and relation there is a comparative study on E)ethnocentrism and F)cultural relativism. An advisable comparative study should be proceeded from A) to B), and E) to F), but a comparative study on C) and D) must be done together. A work theory cannot be excluded in the comparative study on folk art. Meanwhile diffusion theory of comparative study did not meet reference and theoretical principle of a diffusion theory, and it even did not consider cultural and ecological difference by burying in tradition theory. In order to overcome this limit, a new comparativism establishes a comparison model that can coherently study from comprehensive comparison to microscopic comparison. This study established a ‘comparison model for nomadic and agricultural culture’ based on experience of the precedent study. Same and different characteristics between the northern culture and our culture can be grasped and the reasons can be explained. Therefore there is a limitation in propagating influence theory or tradition theory with partial homogeneity without considering ecological reference and cultural context. The bigger problem is not comparing the work itself well propagating tradition theory of the folk art. In art, the work itself is as important as cultural context. However for the case of a masque dance, the northern culture tradition theory is propagated as if came from Mongol without comparing. Masks of Mongol are mostly related with animals like deer, yak, and bird, but in comparison Korean masks are mainly about men. It’s very much comparative animal masks and masque dance of Ongod religion which is based on richness of pasturage is in contrast with human masks and masque dance of religions related with agricultural Gods which is based on richness of agriculture. A ‘nomadic and agricultural comparison model’ has a persuasive point. A diffusion theory is on the assumption of order and superiority of culture, but it is hard to apply this theoretical premise to the Mongolian masque dance and the Korean masque dance. Because there is no evidence that the Mongolian masque dance is superior or precedent than the Korean masque dance. Since a ‘nomadic and agricultural comparison model’ is based on cultural and ecological reference, the two cultures should be examined equally and analyzed with cultural relativism. Therefore, the purpose of a comparative study can be attained by setting a comparison model in a new comparative point of view and comparing the two cultures equally.

        종합군수지원(ILS) 측면의 린 메인터넌스 프레임워크, 정비 전략제시 및 린 예방정비(PM) 사례 연구

        최근까지의 종합군수지원 개발은 정비계획, 지원장비, 보급지원, 군수지원교육 등 ILS 지원성요소의 개발, 획득에 주로 초점을 두었다. 이제는 장기적인 관점에서 무기체계의 운용유지와 총수명주기관리체계(TLCSM)와 같이 지속유지성을 강조하는 제도 등을 고려하여 지원성 분야의 효율성 증대, 비용절감 등 린(LEAN) 관점에서 지원성 전략과 정책이 필요할 때이다. 본 논문에서는 ILS 이론, 린 개념, 린 메인터넌스의 이론적 배경을 살펴보고 이를 바탕으로 군수지원 측면의 린 메인 터넌스 프레임워크를 제시하고 정비 전략을 제시하였다. 또한 레이더 체계의 금속형 필터에 대한 린 예방정비(PM) 사례를 정비 시간, 정비 인시, 정비 비용 3가지 측면에서 분석하였다. 마지막으로 무기체계의 지원성요소 측면에서 추진 방향을 제시 하였다. Until recently, Integrated Logistics Support (ILS) focused mainly on the development and acquisition of supportability factors (maintenance planning, maintenance tools, support equipment, supply support, maintenance training, etc.). In the long term, supportability strategies and policies are needed in terms of LEAN-like cost reduction, efficiency in the field of weapon systems. This study first introduces the concept of Lean, theory of Lean Maintenance and suggests Lean maintenance framework, maintenance strategy for ILS. The case studies were analyzed in terms of the maintenance time, maintenance man-hours, maintenance cost for Lean PM in a radar system. The direction of LEAN on supportability in a weapon system is suggested.
