      • Pharmacokinetics, toxicities, and tissue concentrations of belotecan sprayed by rotational intraperitoneal pressurized aerosol chemotherapy in a pig model

        Objective: We evaluated the pharmacokinetics, tissue concentrations, and toxicities of belotecan during rotational intraperitoneal pressurized aerosol chemotherapy (RIPAC) in pigs. Methods: We sprayed belotecan in 10% and 30% of doses for intravenous chemotherapy in six pigs (cohort 1, n=3, 0.50 mg/m2; cohort 2, n=3, 1.5 mg/m2). We evaluated thetime-dependent plasma concentrations of belotecan before RIPAC to 120 hours for the pharmacokinetics, tissue concentrations in twelve peritoneal regions, and hepatic and renal functions before RIPAC to 120 hours in the 2 cohorts. Results: Mean values of the peak plasma concentration (Cmax), the time to Cmax, the time taken for Cmax to drop in half, and the area under the curve from time zero to the time of last quantifiable concentration were 905 and 3,700 ng/mL, 1.42 and 1.50 hours, 3.64 and 5.60 hours, and 2,260 and 17,900 pg·hr/mL in cohorts 1 and 2, respectively. Mean values of tissue concentrations were 1.5 to 15.3 times higher in cohort 1 than in cohort 2 despite the similar ratio of tissue to plasma concentration, and tissue concentrations in the two cohorts were higher in the parietal peritoneum than in the visceral peritoneum. However, hepatic and renal functions were not different before RIPAC to 120 hours in the two cohorts. Conclusion: RIPAC using belotecan of 0.5 mg/m and 1.5 mg/m may be feasible with fewer hepatic and renal toxicities in pigs. Thus, belotecan of 1.5 mg/m2 may be considered as the starting dose for RIPAC in a phase 1 trial.

        신개발지 일반주거지역의 용도쇠퇴 현황 및 특성에 관한 연구 - 대전 둔산 신시가지를 중심으로 -

        In general, the decay of the residential district has been considered a phenomenon of old towns. But now it has become a problem of new development areas too, which have been a cause of the decay of old town. This study aims to investigate the decay of the residential district in the new development area. The survey site was Dunsan, a town-intown of Daejeon metropolitan city. The findings were as follows (:) 1) even in the new development area, a high level of vacancy ratio was evident. ; 2) among the buildings above ground, 1st floor showed the highest vacancy ratio, which means the 1st floor was allotted to too many commercial facilities relative to the buying power size in this district. ; 3) there were many conversions in response to the decay of the district, from residential to commercial, and commercial to residential. ; and 4) the allocation of each type of conversion showed a spatial pattern.

        여성 노인에서 출산력과 고관절 골절 발생간의 관련성 : 코호트 연구

        Objectives : The reproductive history of women has been suggested to have a possible influence on the risk of osteoporotic fractures. The purpose of this study was to assess the association between reproductive history and hip fractures in the elderly women. Methods : The study subjects were drawn from women members of the Korean Elderly Pharmacoepidemiologic Cohort (KEPEC), aged 65 years or over, whose reproductive histories were available, and who were beneficiaries of the Korea Medical Insurance Corporation (KMIC) in 1993 and lived in Busan city, Korea. The information on reproductive histories, and possible confounders, were collected from mailed questionnaires. Potential hip fracture cases were collected from the claims data obtained between 1993 and 1998, with a hospital survey conducted to confirm the final diagnoses. Rate ratios and their 95% confidence intervals, were calculated using a Cox's proportional hazard model. Results : Following up 5,215 women for 6 years, 51 cases were confirmed with hip fractures. When adjusted for age, weight and physical activity, the rate ratio of hip fractures in women who had given birth three or more times was 0.56 (95% CI: 0.25-1.25), compared with those who had given birth two or less times. When adjusted for age, number of births, weight and physical activity, the rate ratio in women who first gave birth when younger than 22 years was 0.60 (95% CI: 0.34-1.08) compared with those who had giving birth at 22 years or older. Conclusions : According to these findings, an early age when first giving birth might decrease the risk of hip fractures in elderly Korean women.

        초등학교 1학년과 유치원의 교육과정 연계성 분석

        The aim of this study is to ascertain the goodness on the educational contents` organization in the first grade curriculum in light of the curriculum articulation between the kindergarten and the elementary school. The method of analysis is to check the continuity, sequence and integration between them. The result of this study is like these. First, the continuity of the basic organizations between the integrated Subject Matters in the elementary school and the kindergarten curriculum is very weak. Second, the level of educational contents between the We are the First Graders and Wise Life and the kindergarten curriculum are almost same, and the level of the educational contents of the integrated Subject Matters are virtually lower than those of the kindergarten curriculum. Third, there`s a huge overlap in the educational contents between the We are the First Graders and the other first grade`s subjects and between Right Life and Wise life. One reason of these problems seems that the academic system in the organization of the elementary school curriculum has been ignored in the process of reorganizing and making theme-centered curriculum. To solve these mis-connections, the text books and the field applications of the curriculum must be studied, the educational needs of the first grade students must be reflected, and also the specific characters of each subject matters must be valued.

        Peritoneal tuberculosis is a rare presentation of tuberculosis. The presenting signs and symptoms, imaging examinations and CA-125 status in peritoneal tuberculosis sometimes resemble that of ovarian cancer. Thus, the possibility of peritoneal tuberculosis should be considered in the differential diagnosis of ovarian carcinoma. We report two cases of peritoneal tuberculosis initially suspected as ovarian carcinoma but diagnosed as peritoneal tuberculosis with laparoscopy. Using diagnostic laparoscopy unnecessary laparotomy was avoided. We suggest that diagnostic laparoscopy can be helpful in differential diagnosis between peritoneal tuberculosis and ovarian carcinoma.

        세계화 시대의 교육과정 총론 개선 방향

        The aim of this study is to inquire into the restructuring of national curricular frameworks and standards for school curricula preparing for the global era which has changed the phases national politics and economics and also personal life. The result of this study has three parts: First, the direction of educational innovation for global era has to be based on the sound nationalism, friendly multiculturalism and leading globalism, because the global era, which we go through currently, has various aspects of globalism such as nationalism, regionalism, internationalism and multiculturalism. Second, school curriculum has to satisfy the following three conditions. 1) Educational policies has to be built on the core need of curriculum not on the rough discourse of politics or economics. 2) The aim of global education is to educate the Koreans cherished with global citizenship as well as Korean citizenship. 3) Selecting and organizing educational contents based on the various levels of globalism; national level, regional level, and global level. Third, general guidelines for elementary and secondary school curriculum has to be reorganized and filtered through on the globalism which is based on the above-mentioned three conditions. In particular three factors of national curriculum framework and standards such as the aims of school curriculum, the formation and time table of subject matters, and the contents, and scope, quantity, and academic standard of subject matters, need to be restructured in the global perspective.

      • Metabolic Dysfunction-Associated Fatty Liver Disease and Subsequent Development of Adverse Pregnancy Outcomes

        BACKGROUND & AIMS: Recently, metabolic dysfunction-associated fatty liver disease (MAFLD), rather than nonalcoholic fatty liver disease (NAFLD), was proposed to better describe liver disease associated with metabolic dysfunction (MD). In this study, we attempted to investigate the impact of MAFLD on pregnancy complications. METHODS: The current study is a secondary analysis of a multicenter prospective cohort designed to examine the risk of NAFLD during pregnancy. In the first trimester, enrolled pregnant women were evaluated for hepatic steatosis by liver ultrasonography, and blood samples were collected for biochemical measurements. The study population was divided into 3 groups: no NAFLD, hepatic steatosis but without metabolic dysfunction (non-MD NAFLD), and MAFLD. The primary outcome was the subsequent development of adverse pregnancy outcomes, including gestational diabetes mellitus, pregnancy-associated hypertension, preterm birth, and fetal growth abnormalities. RESULTS: The study population consisted of 1744 pregnant women, including 1523 with no NAFLD, 43 with non-MD NAFLD, and 178 with MAFLD. The risk of subsequent development of adverse pregnancy outcomes was higher in MAFLD than in non-MD NAFLD (adjusted odds ratio, 4.03; 95% CI, 1.68-9.67), whereas the risk was not significantly different between no NAFLD and non-MD NAFLD. 3) Among women with no NAFLD, the presence of MD increased the risk of adverse pregnancy outcomes. However, women with MAFLD were at higher risk for adverse pregnancy outcomes than women with no NAFLD without MD or those with no NAFLD with MD. CONCLUSIONS: In pregnant women, MAFLD may be associated with an increased risk of subsequent adverse pregnancy outcomes.

        독일 헤센 주의 역량 기반 교육과정 편성 사례 연구

        The aim of this study is to investigate the purposes, designs, and implementation methods of competency-based curriculum through the case of curriculum development in Hessen. The methods of this study are the literature study and interview study. The result of this study is like these. First, Hessen made the competency-based curriculum to overcome the low result of PISA and TIMSS and to improve the quality of education. Second, The basic structures of Hessen`s new curriculum are composed of common competencies, subject-competencies, the basic concepts and content-areas of the subject, Third, Hessen`s curriculum can be realized in the classroom, when the school-based-curriculum is made on the basis of the basic concepts and content-areas of the subject, According to this analysis, this study suggested that we should make the aims of curriculum revision definite, unravel the conflict between general and subject curriculum, and make the implementation method of competency-based curriculum as a main structure of it.

        2015 개정 교육과정에서 핵심역량 도입에 대한 교육 현장과 전문가의 인식 분석

        The purpose of this study is to analyze the regulations of core competencies in the 2015 revised curriculum and to analyze the actual application of core competencies in the education field to explore the direction of improvement of core competencies in the national curriculum. For this purpose, this study used the following research methods: First, the core competency related regulations presented in the 2015 revised curriculum were analyzed. Second, a survey was conducted on teachers and supervisors about the actual use of core competencies and the direction of improvement, and Delphi survey was conducted on curriculum experts. The results of this study are as follows. First, the 2015 revised curriculum development report specifically suggests the curriculum design plan based on the utilization plan of core competencies, but the results are not clearly visible in the 2015 revised curriculum document. Second, in order to utilize core competencies for the organization and operation of school curriculum, the clarity of core competencies related regulations should be preceded in the national curriculum documents. Based on the above research results, this study discussed the clarity of curriculum documents in relation to the introduction of new curriculum policies. And as a way to implement this, two policy proposals and one follow-up study were proposed.
