COVID-19-associated Pulmonary Aspergillosis: A New Entity

In March 2020, the World Health Organization declared the COVID-19 outbreak a pandemic. This resulted in the discovery of a new clinical Aspergillus disease phenotype, COVID-19-associated pulmonary aspergillosis. This review aimed to collect and share clinical experiences from this new disease.

원주종(cylindroma)은 땀샘에서 기원한 드문 피부 부속기 종양이다. 주로 단발성의 구진, 결절 형태로 두피에 호발하며, 여성이 남성보다 약 2배 가량 흔하게 나타난다1,2. 30대 이하에서 나타나는 경우가 많으며, 드물게 악성 전환을 할 수 있는 것으로 알려져 있다. 55세 여자 환자가 수 개월 전 발견한 두피, 이마, 우측정강이와 앞가슴의 구진에 대해 검사와 제거를 위해 내원하였다. 과거력 상 특이 병력은 없었으나 가족력 상 환자의 아버지도 환자와 비슷한 병변이 두피에 여러 개 있다고하였다. 피부 소견에서 두피와 앞가슴, 오른쪽 정강이 부위에 적갈색 구진이 관찰되었고, 오른쪽 이마에도 살색 구진을 보였다(Fig. 1). 이학적 검사 상 통증과 압통 등의 특이적 증상은 보이지 않았다. 환자는 상기 병변 모두에 대해 제거를 원하였고, 임상적으로 피부 섬유종을 의심하여 네병변 모두에서 수술적으로 절제생검을 시행하였다. 병리조직학적 검사에서 네 병변 모두에서 같은 종류의 종양이 관찰되었다(Fig. 2). 종양은 진피 내에 국한되어 있었고 주위조직과 경계가 잘 구분된 호염기성 기저양 세포로 이루어진 다수의 소엽들이 모여서 섬을 이루고 있었다. 기저양세포의 소엽들은 크기와 모양이 다양하고 서로 맞물려 있는 듯한 양상을 보여, 특징적인 ‘jigsaw puzzle’과 같은 모습을 보였다. 소엽은 두 종류의 기저양 세포로 구성되었으며, 소엽의 중심부는 크고 창백한 소포성 핵을 가진 세포들로 구성되어 있고, 변연부에는 작고 진하게 염색되며 책상배열되어 있는 세포들로 이루어져 있었다. 이상의 임상소견과 특징적인 조직병리학적 소견을 바탕으로 다발성원주종으로 진단하고 새로운 병변의 발생 혹은 기존 병변의 재발 시 내원할 것을 설명한 후 환자는 추적관찰이 되지 않았다. 원주종은 1859년에 Billroth3가 원주종으로 처음 명명하였다. 해외에는 드물지 않게 보고되었으나 국내에는 1994년 윤 등4이 보고한 이마에 발생한 1예와 조 등5이 보고한 상완의 1예의 단발성 원주종 2예, 변 등2이 보고한 두피의 다발성 원주종 1예가 보고되어 있다. 원주종은 병리조직학적으로 호염기성 세포로 주로 구성된 에크린 땀샘종, 털상피종 등과의 감별이 필요하며 특징적인 ``jigsaw puzzle`` 소견으로 쉽게 감별할 수 있다. 원주종의 악성 변화는 드물지만 악성 변화의 가능성을 고려하여 수술적 절제가 추천되며, 특히 가족성 원주종증 또는 Brooke-Spiegler 증후군과 같이 다발성인 경우, 통증, 궤양, 감염 등을 동반한 경우에는 수술적으로 제거하는 것이 좋다2. 이에 중년에 발병하였고, 임상적으로 비전형적인 분포를 보였으며, 육안으로 피부섬유종과 유사하게 보인 다발성 원주종 1예를 경험하고 국내 문헌 상 드문 증례로 생각하여 보고하는 바이다.
Dermatophytosis: Clinical Manifestation, Diagnosis, and Considerations for Antifungal use

Dermatophytosis is a fungal infection of the keratinized tissues of the skin, hair, and nails caused by dermatophytes or other fungi. Epidermophyton, Microsporum, and Trichophyton are the three genera of dermatophytes that cause skin infections. In Korea, Trichophyton rubrum is the most common dermatophytes. Dermatophytosis can be classified into anthropophilic, zoophilic, and geophilic, and most fungal infections encountered by dermatologists are tinea, dermatophytosis. This study attempted to provide a brief discussion about the clinical manifestation, diagnosis, and considerations for antifungal use in the treatment of dermatophytosis.

Background: The contagious skin infestation scabies, is caused by the mite Sarcoptes scabiei var. hominis. These microscopic organisms navigate under the skin surface to lay eggs, creating burrows that cause notable discomfort and itching and enable the maturation and subsequent proliferation of the mites. Spread to other hosts can occur through direct, often prolonged, skin-to-skin contact or indirectly through shared items such as clothing, bedding, or towels. Current Concepts: Management of a scabies outbreak encompasses several pivotal steps: accurate diagnosis, comprehensive treatment with topical scabicides to kill the mites, rigorous environmental control (including washing all possibly infested materials in hot water), meticulous contact tracing of all close contacts, administration of symptomatic relief through antihistamines and corticosteroids, and meticulous follow-up to verify successful treatment outcomes. Discussion and Conclusion: The prevention of further infestation and recurrent outbreaks is crucial, necessitating a holistic, integrative public health management approach. This approach involves consistent communication and public education about preventive measures and prompt treatment strategies to curtail widespread infestation, thereby safeguarding community health.
