        하이드록시아파타이트 지지체의 표면 처리 방법에 따른 골아세포의 성장

        Purpose: Recently, bioceramics have become popular as a substitute graft material for reconstruction of bony defect after trauma or tumor surgery. Among the bioceramic materials, hydroxyapatite (HA) is favored due to its biocompatibility. HA scaffold is composed of the interconnected reticular framework, macropores and micropores. Macropores play an important role in cell migration, nutrients supply and vascular ingrowth. On the other hand, a number of micropores less than $10{\mu}m$ form an irregular surface on HA scaffolds, which prevents the osteoblast from adhering and proliferating on the surface of HA scaffold. Methods: In this study, three different groups were designed for comparison. In the first group (group A), conventional method was used, in which HA pellet was applied without surface pretreatment. The second group (group B) was given a HA pellet that has been coated with crystalline HA solution prior to application. In the third group (group C), the same method was used as the second group, where the pretreated HA pellet was heated ($1250^{\circ}C$, 1 hour) before application. Osteoblast-like cells ($2{\times}10^4$/mL) were scattered onto every pellet, then they were incubated in 5% $CO_2$ incubator at $37^{\circ}C$ for twelve days. During the first three days, osteoblast cells were counted using the hemocytometer daily. ALP activity was measured on the 3, 6, 9 and 12 culture days using the spectrophotometer. Results: Under SEM, group A showed a surface with numerous micropores, and group B revealed more rough crystal surface. Group C revealed a fused crystal appearance and flattened smooth surface. In proliferation and ALP activity of osteoblast cells, group C showed better results compared to group B. Group A which lacks pretreatment of the surface showed less osteoblast proliferation and ALP activity than group C, but showed better results than group B. Conclusion: We found that crystallized HA with heat treatment method enhances the osteoblasts proliferation and differentiation on the surface of HA pellets.

        보툴리눔 독소를 이용한 Frey 증후군 환자의 치료

        Purpose: Frey's syndrome is a common complication after parotid surgery and characterized by gustatory sweating and flushing in the periauricular area during meals. Although a variety of methods were proposed to prevent this postoperative problem, they were unsatisfactory. We used botulinum toxin A to treat 9 patients with Frey's syndrome and investigated the duration effect after the injection. Methods: Nine patients became the object of study about the effect of botulinum toxins as treatment of Frey's syndrome. Age of patients ranged from 25 to 78 years(mean, 43.7 years). Six of nine patients had both symptoms of gustatory sweating and flushing. And the others had only gustatory sweating symptom. Using Minor starch iodine test, the affected skin area was detected, and it was marked by 1cm2 sized grid appearance. After application of EMLA cream on the gustatory sweating area, botulinum toxin A was injected intracutaneously into the affected skin area(2.5U/㎠). Patients were followed up from six to fifteen months(mean, about 12 months) and asked about improvement of their symptoms. Results: The treatment with botulinum toxin A had an effect within 2 days after the injection. Six months after injection, gustatory sweating disappeared completely in all patients, and 5 of six patients who had gustatory flushing improved in their symptom. At the last follow-up, no patients complained of recurrent gustatory sweating and flushing except one. One patient, seven months after initial injection, was retreated with botulinum toxin A because of recurrence, and the result was successful. The duration of the effect after botulinum toxin A treatment was ranged from seven to thirteen months. One patient in our series experienced the upper eyelid weakness as adverse effect, but it improved spontaneously. Conclusion: Local injection of botulinum toxin A is an effective, safe and long-lasting method for treatment of Frey's syndrome. Hereafter, however, additional study is required to evaluate the duration effect of botulinum toxin A as to frequency in use and dosage.

        악성 한선 극세포종 증례보고

        Purpose: Hidroacanthoma simplex (HAS) is a rare benign neoplasm of the skin and known as intraepidermal poroma. Malignant transformation of HAS has been reported in the literature, but it is very rare. We experienced a case of malignant hidroacanthoma simplex (MHA) and performed the surgery of the neoplasm. Methods: A 73-year-old patient visited our department for evaluation and treatment of the skin lesion ($3.2{\times}3.0\;cm^2$) that occurred two years before on the right ankle area. The lesion was characterized by dark-brown, hyperkeratotic and protruding patch. The patient had no symptom, and any palpable lymph node was not found on physical examination. The result of laboratory was also nonspecific. The lesion was excised completely with a resection margin of 5 mm including subcutaneous tissue partially, followed by full-thickness skin graft for reconstruction. Results: The result of biopsy was reported as malignant hidroacanthoma simplex. Histologically, the epidermis showed discrete aggregates of tumor cells which had round, hyperchromatic nuclei and pale or clear abundant cytoplasm. Cystic spaces were found within the tumor and mitosis were seen, frequently. No recurrence was found one month after surgery. Conclusion: The case of malignant hidroacanthoma simplex was treated with full-thickness skin graft following wide excision of the tumor.

        희귀동위원소 핵반응에 따른 엑스선폭발 광도곡선 변화

        엑스선폭발 (X-ray Bursts)은 중성자별 표면에서 발생하는 열핵반응폭주에 의해 발생하는 것으로 알려져 있다. 이러한 열핵반응폭주는 중성자별 표면으로 유입되는 물질의 유입율 및 금속함량과 열핵반응폭주에 참여하는 원자핵의 핵반응에 따라 달라진다. 이번 연구에서는 세가지 특정한 반응인 삼중알파, $^{18}\mbox{Ne}(\alpha,p)^{21}\mbox{Na}$, $^{15}\mbox{O}(\alpha,\gamma)^{19}\mbox{Ne}$ 핵반응율에 따라 엑스선폭발의 광도 곡선 모양이 어떻게 변화하는지를 조사하였다. 연구의 결과로 광도 곡선의 형태가 같은 반응의 서로 다른 반응율에 따라 변화하는 것을 확인하였지만 54개 동위원소를 포함한 반응망을 가지고 계산한 광도 곡선 모양은 반응율에 따른 변화를 고려하더라도 여전히 실제 관측된 광도 곡선과 큰 차이를 보였다. 이전 연구에서처럼 광도 곡선의 모양은 열핵반응폭주에 참여하는 원자핵의 갯수에 더 큰 영향을 받는다는 것을 확인하였다. X-ray bursts (XRBs) have been thought to occur via thermonuclear runaway reactions on the surface of a neutron star. The runaway reactions are affected by both the properties of the accreted material, such as the accretion rate and the composition, and the reaction rates of participating nuclei. We investigate how the shape of the XRB light curve changes with specific reaction rates. As a case study, we choose three reaction rates, triple alpha, $^{18}\mbox{Ne}(\alpha,p)^{21}\mbox{Na}$, and $^{15}\mbox{O}(\alpha,\gamma)^{19}\mbox{Ne}$. We find that the shape of the light curve does change with different reaction rates for the same reaction, but the predicted light curves with 54 isotopes do not match the observed ones even when the variations within the different reaction rates are considered. We confirm that the shape of the light curve is more sensitive to the number of isotopes that participate in the runaway reactions.

        Finite-Difference Time-Domain Analysis on Grating Coupled Plasmonic System

        In this work, we used a finite-difference time-domain (FDTD) characterization to demonstrate that the e-field near metal nanoparticles (NPs) can be enhanced by dielectric grating structure. An array of Au NPs (ϕ = 60 nm) with 4 nm gaps is set as a plasmonic system. The Au NP arrays on a flat polydimethylsiloxane (PDMS) substrate and a thin Au film-coated substrate are considered as control samples. Coupling with a PDMS line grating (5 m 3 m 1 m = width gap height) is carried out by placing the Au NP array on the PDMS grating and by putting the PDMS grating on the Au NP array on a thin Au-film-coated substrate. The dependences of the Wavelength and the angle of incidence of a plane-wave source on the e-field intensity are investigated. For the sample systems in the result, the maximum e-field of the PDMS-grating-coupled samples is enhanced by more than a factor of two compared to that in the control samples. This finding will be helpful to develop e-field-enhancing templates for surface-enhanced raman spectroscopy (SERS) applications.

        기도유지기 마스크 환기마취를 이용한 비관혈적 비골골절 정복술

        Purpose: The majority of nasal fractures have been treated by closed reduction. And they were manipulated under local anesthesia or general anesthesia. Under the local anesthesia, patients can feel the pain and fear, so general anesthesia through the endotracheal intubation became popularized recently to treat the nasal fracture. But it has still the drawbacks of postanesthetic complication. Therefore, under the mask ventilation anesthesia using oral airway, we tried to manipulate the nasal fracture. Methods: From July 2007 to November 2007, we worked with fifty patients that were manipulated the nasal fracture. Fifty patients were divided into two groups, general anesthesia with the endotracheal intubation group(n=25) and the mask ventilation using oral airway group(n=25). We checked up the anesthesia time, postanesthetic complication, postoperative aesthetic & functional problem of nose in two groups. Results: In total operation time and sore throat frequency among the postoperative anesthetic complications, there was statistically significant difference between the mask ventilation group and the endotracheal intubation group(p<0.05). But there was no difference statistically in nausea frequency(p>0.05). And no patients complained of postoperative nasal complication such as septal deviation, septal perforation, nasal obstruction and hump nose in two groups. Conclusion: Through the mask ventilation using oral airway, we could reach satisfactory results in the anesthetic time and postanesthetic complication.

        유연성 선형 성형작약 개발에 관한 연구

        The shaped charge consists of the cast or pressed explosive and the metal liner. The pressure formed in detonation wave is so high that the liner is collapsed and the jet of high temperature, pressure and velocity is produced. The jet penetrates the target. In this paper, the simulation for optimization of flexible linear shaped charge(FLSC) was carried out by AUTODYNE program. Based on the results of simulation, we made a prototype of FLSC and evaluated penetration performance, flexibility and its application. The test result of prototype was compared with that of simulation.

        1970~1980년대 중국의 핵전력 강화 노력과 핵 억제전략

        China has been threatened by the West several times since the 1950s and its nuclear deterrence capabilities have been recognized as an indispensable tool for its nuclear deterrence strategy. This has demonstrated for China’s successful development of its first nuclear bomb on October 16, 1964. Since then its was a report that China has possessed about at least 50 nuclear warheads by the end of the 1960s. Compared with numbers of nuclear warheads of United States and the Soviet Union China has been regarded as an absolute inferiority disposition of demonstrating its nuclear deterrence capabilities. However, during the period ranging from the 1970s to the 1980s, China has increased its nuclear deterrence capabilities in saying its approximately 230 numbers of nuclear warheads in the year of 1989. In addition China has developed its so-called nuclear triads: Intercontinental Ballistic Missiles (ICBM) deployed land nuclear sites, nuclear-powered submarines equipped with submarine launched ballistic missiles (SLBM), and H-series strategic nuclear bomb-equipped long-range bombers known as H-6 aircraft. This indicates that China has changed its concept of nuclear deterrence strategy from existent deterrence strategy to minimum deterrence strategy. Moreover, China also demonstrated its nuclear deterrence strategy known as concept ambiguity of using its nuclear weapons because of its weakness and limits of facilitating nuclear weapons and capabilities. In conclusions China continues to increase its nuclear deterrence capabilities and its nuclear arsenals regarding to its sufficient nuclear deterrence capabilities and People’s Liberation Army (PLA) wants to qualitative leading as well as quantantative comparison with United States mainly. Chinese nuclear strategy will be more innovative reform to meet its nuclear deterrence capabilities in either qualitative or quantile comparison.
