        노인장기요양보험 재가급여유형이 노인의 우울감에 미치는 영향

        본 연구는 노인장기요양 재가급여유형이 노인의 우울에 미치는 영향을 확인하고 합리적 장기요양서비스 이용모형을 위한 근거자료를 제시하기 위한 것이다. 주요 연구결과는 다음과 같다. 첫째, 주간보호서비스를 중심으로 이용하는 노인 일수록 방문요양 중심 이용노인 보다 우울감이 낮았다. 둘째, 학력이 낮을수록, 동거가족수가 적을수록, 주택 자가소유가 아닐수록, 경제상태가 좋지 않을수록 노인의 우울감이 높았다. 또한, 건강상태가 나쁠수록, 만성질환이 있을수록 우울감이 높았다. 요양서비스는 이용기간이 길수록 높았다. 셋째, 방문요양서비스와 주간보호서비스 중심 이용 노인의 우울에 미치는 영향 요인에 따른 차이를 분석한 결과, 사회적 지지의 경우 방문요양서비스 이용 노인들에 있어서 유의미한 영향을 미치지 않는 반면, 주간보호서비스 이용 노인들에게는 통계적으로 유의미한 영향을 미치고 있었다. 즉, 주간보호서비스 이용 노인들에 대한 사회적지지 수준은 정신건강에 중요한 요인으로 영향을 미치는 반면에 방문요양 이용 노인들에게는 영향을 미치지 않았다. 따라서 재가급여 이용노인의 우울 감소를 위하여 방문요양급여와 주간보호서비스간의 연계와 통합적인 서비스를 위한 노력이 필요하다. This study presents references that the utilized rate of long-term care services reasonably and identifies that having an effect on depression of elderly by long-term care insurance. With regards to this prior research, the included results are as follows. First of all, the elderly who use the day care service mainly showed lower depression than those who visited care centers. Second of all, depression was higher with lower educational background, the number of family members, non-owned house and the economic condition. Not only that, the depression was higher as the state of health worsened, the more chronic disease. According to the period of use of the home-care service, moreover, depression was higher as the use of home-care service was longer. Third of all, the factors affecting the depression of the elderly who use home-care services and day care services are the social support did not have a significant effect on the elderly who use the visiting care services, but it had a statistically significant effect on the elderly who use the day care services. In other words, the level of social support for the elderly who use day care services is an import factor for mental health, but does not affect the elderly who use visiting home-care. As a result, the implements for reducing depression of elderly people using long-term care re-supply benefits were discussed.

        간호사와 환자가 인지하는 환자존중 비교 연구

        A study was designed to compare patients` and nurses` views on the expectations and performance related to ``respect for patients.`` The subjects of this study, which was approved by an Institutional Review Board, were 276 nurses working in general wards or intensive care units and 172 patients admitted to the general ward of a university hospital in South Korea.?The instrument used in the study was based on the concept of Respect for Patients developed by Yoo Myoung-Ran. The collected data was analyzed using SPSS WIN 12.0 with a paired t-test, independent t-test, and a one way ANOVA. The results were as follows. First, there was a statistically significant difference between nurses` views of the expectations and performance levels related to respect for patients (p=.000). Second, there was a statistically significant difference between patients` views of expectations and performance?levels related to respect for patients (p=.000). Third, the level of expectation concerning respect for patients as viewed by nurses and patients was 4.46 and 4.22 respectively. This was a statistically significant difference (p=.000). The level of performance related to respect for patients as viewed by nurses and patients was 3.48 and 3.82 respectively. This too was a statistically significant difference (p=.000). Fourth, there was a statistically significant difference in the level of expectations concerning respect for patients relative to the level of the nurses` education (p=.015) and a statistically significant difference in the level of performance related to respect for patients relative to the nurses` working place (p=.001). There was a statistically significant difference in the level of expectations concerning respect for patients relative to patients` income levels (p=.009) and a statistically significant difference in the level of performance concerning respect for patients relative to patients` income levels (p=.005) and religion (p=.037).

        신규간호사의 조직사회화 측정도구 개발

        Purpose: This study was conducted to develop an organizational instrument on socialization for new graduate nurses in the workplace, and to test its reliability and validity. Method: A methodological design was used to develop the instrument. Data were collected from new clinical nurses in 188 hospitals nationwide in Korea. In the first stage, 159 items were developed through a literature review. In the second stage, seven experts conducted a content validity test and the number of items was reduced to 155. In the third stage, factor analysis was performed to analyze the data collected from 198 nurses in 8 hospitals using the modified instrument. A total of 65 items were included and a five-point Likert scale was used. In the fourth stage, the final evaluation instrument was finalized as comprising 11 factors in 7 domains, using 39 items developed from analysis of the data collected from 1,410 nurses using the 40 items. It was comprised of 11 factors in 7 domains. The SPSS 11.0 windows program was used. Results: Accumulated variation for the 11 factor-39 items nurse organizational instrument on socialization was 67.22%. The results of the reliability test indicated that Cronbach's coefficient for the total 39 items was over 0.97. Conclusion: The instrument developed in this study has a high degree of reliability and validity.
