        Neostigmine 분할투여가 Vecuronium 차단 가역에 미치는 영향

        The hypothesis that administration of neostigmine in divided doses might accelerate the antagonism of nuromuscular blockade was investigated. Neostigmine 0.05mg/kg was adminsitered either in a single bolus dose (Group I, n=10) or in an initial dose of 0.01 mg/kg 1 minute later (Group II, n=10), 2 minutes later (Group III, n=10) and 3 minutes later (Group IV, n=10) for antagonism of vecuronium- induced blockade. Reversal was attepted at 10 percent spontaneous recovery of twitch height. The mean time (±SE) from the first injection of the drug until the train-of-four(TOF) ratio value had reached 0.75 was signifincantly longer in Group II and IV (594.8±63.9 seconds and 555.6±22.2 seconds respectively) than Group I and III (380.6+36.0 seconds and 357.8±44.2 seconds respectively). It is concluded that adminstration of neostigmine in divided doses with 0. 01 mg/kg and 0.04 mg/kg did not produce a significantly faster reversal of residual vecuronium-induced neuromuscular blockade as compared to a single bolus adstration.

        진행성 근육 발육 이상 환자의 마취 경험

        A 43 years old female patient who had known FSH form(Fascioscapulohumoral type) muscular dystrophy was scheduled for total abdominal hysterectomy due to uterine myoma. Induction of ansthesia was performed without muscle relaxant and maintained with fentanyl, oxygen and nitrous oxide. Muscle relaxation was excellent without neuromuscular blocking agent by the meuromuscular twitch monitoring(too week response to neuromuscular twitch at 70mA). After the end of operation, the patient was awaken without any problem and was transferred to recovery room.

        Reactive Orange 16 Dye를 이용한 청색광차단 기능의 하이드로겔 콘택트렌즈 제조

        Purpose : Blue-light blocking dyes were chemically conjugated to the hydrogel contact lenses. Blue-light blocking properties of the resultant contact were investigated, and the amounts of attached dyes were quantified. Methods : Vinyl sulfone-based Reactive Orange 16 Dyes were covalently conjugated to hydrogel contact lenses containing a considerable amount of alcohol groups. The investigation for blue-light blocking properties of the hydrogel lenses and the quantification for the amounts of attached dyes were performed using UV-vis spectrophotometer. Results : The blue-light blocking Reactive Orange 16 Dyes were successfully conjugated to the hydrogel contact lenses. The resultant contact lenses exhibited good blue-light blocking properties according to the results of the UV-vis spectra investigation. Reactive Orange 16 Dye conjugated to contact lenses was quantified using Beer-Lambert's law, and the blue-light blocking rate of contact lenses could be controlled by adjusting the concentration of reactive dye. Conclusion : In this study, a reactive dye with a blue-light blocking function was fixed to hydrogel contact lenses through chemical bonding, and their blue-light blocking function was analyzed. It was confirmed that as the concentration of the blue-light blocking dye increased in the chemical reaction, the blocking intensity in the blue light wavelength region between 380 and 500 nm and the amount of the conjugated dye also increased. The development of blue-light blocking contact lenses that can be mass-produced is expected to play an important role in the development of functional ophthalmic optical medical devices. 목적 : 반응성 청색광차단 염료를 하이드로겔 콘택트렌즈에 화학적으로 고정시켰고, 제조된 콘택트렌즈의 청색광차단 기능의 분석 및 첨가된 염료를 정량하고자 한다. 방법 : Vinyl sulfone-기반의 반응성 염료인 Reactive Orange 16 Dye를 다량의 알콜 작용기를 함유하는 하이드로겔 콘택트렌즈에 화학적으로 결합시켰다. 콘택트렌즈의 청색광차단 특성 및 염료의 정량은 UV-vis spectrophotometer를 이용하여 확인하였다. 결과 : 청색광차단기능의 Reactive Orange 16 Dye가 성공적으로 하이드로겔 콘택트렌즈에 결합되었다. UV-vis spectra 분석을 통해 염료를 함유한 콘택트렌즈들이 우수한 청색광차단 기능을 보임을 확인하였다. Beer-Lambert의 법칙을 이용하여, 콘택트렌즈에 첨가된 Reactive Orange 16 Dye를 정량하였으며, 반응 염료의 농도 조절을 통해, 콘택트렌즈의 청색광차단율을 조절할 수 있었다. 결론 : 본 연구에서는 청색광차단 기능의 반응성 염료를 하이드로겔 콘택트렌즈에 화학적 결합을 통해 고정시키고, 청색광차단 기능을 분석하였다. 화학적 반응에서 청색광차단 염료의 농도가 증가할수록, 380~500 nm 사이의 청색광 파장 영역에서의 차단 세기와 첨가된 염료의 양이 같이 증가함을 확인하였다. 대량생산이 가능한 청색광차단 콘택트렌즈의 개발은 기능성 안광학의료기기 개발에 큰 역할을 할 것으로 기대된다.

        성인용 Gould 및 소아용 Cook 압력전달도관에서 적정 동맥혈증 가스분석치를 얻기위한 추출 혈액량 결정에 관한 연구

        Although heparin solution is widely used as an anticoagulant in blood gas analysis, alterations in blood gas measurements may occur when small sample volumes are diluted by heparin. Cook pressure tubing for infants and children and Gould pressure line for adults were applied to each of 10 adult patients whose cardiovascular function was normal. Arterial blood gases were measured serially after withdrawing 2, 4, 6, 8, 10 and 12 ml of flush-vlood solutions from the Gould pressure connecting tubing, and 1, 2, 3, 4, 5 and 6ml from the Cook pressure line, which were connected to a transducer throuhg a 3-way stopcock. In the Gould pressure tubing, the pH was unchanged from sample 2 and PaCO_2 and base excess were unchanged from sample 3. PaO_2 did not change between the samples. In the Cook pressure line, the pH and base excess were unchanged from sample 2 and PaCO_2, did not change between the samples. Inthe Cook pressure line, the pH and base excess were unchanged from sample 2 and PaCO_2 HCO_2 and PaO_2 from sample 3. We conclude that arterial blood should be withdrawn in bolumes of at least 4 ml by the Gould pressure connecting tybing and 3 ml by the Cook pressure line to obtain accurate results in blood gas analysis.

        후쿠시마 측위하의 미세 혈관 감압술후 발생한 상기도 폐쇄와 상완 신경총 손상

        A 25 years old woman was adrnitted to the department of neurosurgery for a right hemifacial spasm. She was undertaken microvascular decompression in the Fukushima lateral position. Following completion of operation, she began to complain of dyspnea, but the phonation was not changed. Mild weakness and paresthesia on left upper extremity was complained, too. Severe swelling on the left side of face and neck expending to left shoulder was noted simultaneously. Immediate computerized tomography was taken to reveal the cause of respiratory distress, which suggested that venous and/or lymphatic congestion on the left side of neck. As respiratory distress was getting more severe, emergent tracheostomy was taken. The cause of above symptoms was suspected to an extreme rotation and flexion of the head resulting in direct contact of mandibular body to clavicle. A month after operation, there was still remained mild paresthesia on left thumb and index finger.s

        토끼 대동맥환에서 Halothane 과 Sevoflurane 이 내피의존에 의한 혈관 장력에 미치는 영향

        This study was aimed to elucidate the endothelium-dependent vascular effects of halothane and sevoflurane on rabbit aortic rings at two conventional concentrations(high induction and low maintenance concentration in human). Isometric tenslon was recorded in isolated aortic rings. Preparations of rabbit thoracic aorta were suspended in Krebs' buffer and aerated with 95% O_2 and 5% CO_2. One set of the rings had intact endothelium and the other set of the rings had endothelium mechanically denuded. In the first experiments, the rings were precontracted with norepinephrine(NE) of 10^(-7) M. After tension was stabilized in 10~15 minutes following NE, halothane(1, 2%) or sevoflurane(2, 4%) was bubbled with O_2/CO_2 gas mixture at increasing concentrations. In the second experiment, O_2/CO_2 gas mixture only(control rings), halothane 2% or sevoflurane 4 % with O_2/CO_2, gas mixture was bubbled for 10 minutes prior to and during contraction with NE of 10^(-7)M. After tension was stabilized following NE, acetylcholine(10(-8)-10(-6) M) was added cumulatively. In the third experiment, the procedure was as same as the second experiment except for that acetylcholine(10^(-8)-10^(-6) M) was substitued for nitroglycerin (10^(-9)-10^(-6) M) . The present study demonstated that both of halothane and sevoflurane at high concentration caused a vasoconstriction to 110.7±4.2% and 122.4±8.4% in vascular rings with intact endothelium, and 106.1±1.9% and 118.3±3.5% in vascular rings with denuded endothelium, respectively, compared to each control value of 100%. Furthermore, halothane and sevoflurane attenuated the acetylcholine induced relaxing response in NE-precontracted vascular rings with intact endothelium, but did not affect any change of tension in vascular rings with denuded endothelium. Halothane and sevoflurane did not attenuate the nitroglycerin induced relaxing response in NE-precontracted vascular rings with both intact and denuded endothelium. In conclusion, halothane and sevoflurane at high concentration has vasoconstrictory effects ascular smooth muscles in rabbit aortic rings regardless of presence of endothelium and also attenuated the endothelium-dependent relaxation.
