        의인성 쇄골하정맥 파열로 인한 응급 혈관내 스텐트 삽입

        Central venous cannulation is one of the most commonly performed procedures for critically ill patients in the emergency room. Serious complications like a rupture of subclavian artery may occur during this procedure. We report a case of successful stent graft deployment for iatrogenic ruptured subclavian artery after attempted right subclavian vein catheterization in a 31 year-old female patient with hypovolemic shock due to cervical os laceration during vaginal delivery.       • 열식 유량계의 운전온도 범위 확장을 위한 센서부 열전달 계수에 관한 연구

        In order to measure gas flux, there are some techniques based on volume flux and mass flux. The mass flux measurement is more favored to measure correctly gas flux under the extremely - high thermal or extremely - low pressure conditions. The thermal mass flow-meter is the representative technique to measure mass flux. In designing the flow-meter, it is important to calibrate convectional thermal coefficient affected by flow velocities around sensor and thermal changes. To analyze quantitatively the convectional mechanism around cylinders in high thermal external flow , experiments are conducted through installing 40m3/min blower at 100 mm duct for high thermal gases and heater able to bring ambient gas up to 200 ℃. By using control valves at the end point of duct, we controlled fluid velocities. By installing Pt 100 sensor to the heater, fluid temperature is controlled. In this experiment, convectional coefficients around cylinders are proportional to velocity to the power of 0.45 . A and B in the correlation equation coefficients behaved independently at 105 ℃

        노인의 우울증과 뇌파검사를 통하여 심리적 요인에 미치는 효과

        Purpose : This study purposed on the understanding of psychology in the elderly women using depression scale and electroephalography. The subjects were thirty elderly women in geriatric care hospital. All participants were elderly women over 65 years old. The subjects signed up with informed consent and they were divided into a normal elderly group, an exercise elderly group and a hospital elderly group. They were randomly assigned with 10 persons. Methods : The study instruments were Geriatric Depression Scale(GDS) and Poly-G-I. Brain wave activity was measured by "power spectrum analysis" of TeleScan program. Statistic analysis consisted of average, standard deviation, One-way ANOVA and post-hoc Tukey using SPSS 21.0 version. The significance was set at .05. Results : Depression scale results showed that the exercise elderly group were 10.60±5.36 points which was the lowest among the three groups. The Normal elderly group was 16.20±5.59, and the hospitalized elderly group was 16.70±6.76 points. There was no significant difference between the normal elderly group, exercise elderly group and hospitalized elderly group. The hospitalized group showed statistical difference in relative theta power in the area of Fp1, F3, F4, T3, P3 as compared to the normal elderly group and the exercise elderly groups. There was no significant difference between relative beta power and relative gamma power in three groups. Conclusion : Geriatric depressed scale showed no significant difference in each group. This is indicated in the mental problems associated with depression. This indicates a higher level of depression in the hospitalized elderly women, more than in the normal elderly women and exercise elderly women groups.

        보건의료종사자에 대한 폭력 양상과 대처에 대한 연구

        우리나라에서 의사와 간호사를 비롯하여 환자에 대한 치료를 위한 의료행위를 수행하거나 보조하는 각종 보건의료종사자에 대한 의료기관 내부 혹은 외부에서의 폭력으로 인한 사고는 끊임 없이 발생하고 있는 것이 작금의 현실이다. 그리고 이와 같은 현상은 비단 우리나라뿐만 아니라 미국의 경우에도 발생하고 있는 상황이다. 그렇기에 보건의료종사자를 폭력으로부터 보호하기 위한 예방책과 폭력이 발생하였을 때 그에 대한 보상과 치료, 법적 제재 등 사후 처리가 필요하다는 점에 대한 인식은 우리나라와 미국 모두 동일하다. 그러나 우리나라와 미국 사이에 보건의료종사자에 대한 폭력에 대한 예방 및 대처방안에 관한 차이점이 존재한다. 즉, ① 그와 같은 예방 및 대처방안을 마련하기 위하여 현실에 관한 실증을 위해 타 직업군과의 비교나 통계 처리의 존부, ② 정부 주도 하의 대응인지 이익단체 주도 하의 대응인지, ③ 의료기관이라는 장소적 한계에 국한하여 예방 혹은 대응책을 마련하고 있는지 여부, ④ 보건의료종사자의 측면 외에 환자의 측면에 대하여도 예방이나 대응책을 마련하고 있는지 여부 등이 그것이다. 이렇듯 보건의료종사자에 대한 폭력을 예방하고 대처하기 위한 다양한 방안의 제시에 있어서는 궁극적으로 어느 정도의 범위까지 포함할 것인지를 각국의 실정과 사회적 수용성에 의거하여 설정하여야 할 것이다. The current reality is that the violence in and out of medical institutions continues to occur in Korea, including doctors and nurses and other health care workers who perform or assist in medical care for patients. And this is not only happening in Korea but also in the United States. Therefore, both Korea and the U.S. share the same perception that preventive measures to protect health care workers from violence and that compensation, treatment, and legal sanctions that should be taken after the violence occurs. However, there are differences in prevention and countermeasures against violence against health care workers between Korea and the United States. Namely, ① Is there any comparison or statistical processing with other occupational groups to substantiate the reality in order to prepare such prevention and countermeasures? ② Is it government-led or profit-oriented? ③ Whether preventive or counter measures that being prepared are limited only in the limit of the location of the medical institution ④ In addition to the health care personnel’s aspects, whether preventive or countermeasures are being prepared for the patient’s side. So in the presentation of various measures to prevent and combat violence against health care providers, ultimately, to what extent it should be included should be set according to the state of affairs and social acceptability of each country.

        제주도 탈염 용암 해수를 이용하여 수경재배한 새싹인삼 추출물의 In Vitro 신경독성에 대한 신경보호 효과

        본 연구에서는 유용한 미네랄 성분을 다량 함유하고 있어 산업적 가치가 높은 제주 용암 해수에서 탈염 과정을 거쳐 만들어진 탈염 용암 해수를 이용하여 수경재배한 새싹인삼추출물 건조시료를 Q-Orbitrap으로 성분을 규명하고, HPLC로 기준 및 시험방법을 설정하여 표준화했으며 ICP-AES를 이용하여 무기물 함량을 분석하였다. 시험결과에서 주요 ginsenoside Rb1, Rg1 및 Na, Mg, Ca, Sr, V와 같은 미네랄 함량이 증가함을 확인했으며 이에 대한 효능을 입증하기 위해서 제주도 탈염 용암 해수를 이용해 수경재배한 새싹인삼 추출물에 대한 신경세포사멸과 신경세포 염증 반응 억제, 신경전달물질 및 시냅스 가소성 회복을 통한 기억력 개선에 대해 in vitro 효능검증을 수행하였다. 제주도 탈염 용암 해수 수경재배 새싹인삼 추출물에 대한 동결 건조를 진행 후 ginsenoside 함량 분석을 진행한 결과에서 ginsenoside Rb1(11.5 mg/g), Rg1(6.85 mg/g), Re(19.2 mg/g)의 합이 37.55 mg/g으로 측정되었고, 이는 식약처 건강기능식품 인삼 기능성분의 함량기준인 Rb1, Rg1의 합이 0.8~34 mg/g인 것과 비교했을 때, 탈염 용암 해수로 수경재배한 제주새싹인삼 동결 건조물은 개별 인정형 원료로서의 충분한 요건을 갖춘 것으로 사료된다. 또한 제주도 탈염 용암 해수 수경재배 새싹인삼 추출물 건조시료에 대한 세포독성 및 효능평가 결과에서 제주도 탈염 용암 해수 수경재배 새싹인삼추출물 건조시료는 RA-differentiated SH-SY5Y neuronlike cell에 48시간 동안 처리 시 1,250 μg/mL에서도 세포독성을 보이지 않았으며, 더욱이 Aβ1-42 oligomer(10 μM)를 72시간 동안 처리하여 세포독성을 유도한 세포실험 모델에서 비처리 대조군과 비교하여 제주도 탈염 용암 해수 수경재배 새싹인삼 추출물 건조시료를 처리한 실험군에서 세포사멸 관련 바이오마커들인 cleaved/total caspase-3 ratio, cleaved/total PARP, Bax/Bcl-2 ratio를 감소시켜 Aβ1-42 oligomer에 의한 신경세포사멸을 억제하는 효과를 보였다. 그리고 RA-differentiated SH-SY5Y neuron-like cell에 H₂O₂(300 μM)를 처리하여 신경세포 염증 반응을 유도한 세포실험 모델에서 비처리 대조군과 비교하여 제주도 탈염용암 해수 수경재배 새싹인삼 추출물 건조시료를 처리한 실험군에서 염증 반응과 관련된 NF-κB p65 단백질의 nuclear translocation 억제, COX-2 단백질 발현 감소, MAPK (ERK1/2, SAPK/JNK, p38 MAPK) 인산화를 감소시켜 H₂O₂에 의한 신경세포 염증 반응을 억제하는 효과를 보였다. 한편 RA-differentiated SH-SY5Y neuron-like cell에 Aβ1-42 oligomer(1 μM)를 처리하여 신경전달물질 및 시냅스 가소성 감소를 유도한 세포실험 모델에서 비처리 대조군과 비교하여 제주도 탈염 용암 해수 수경재배 새싹인삼 추출물 건조시료를 처리한 실험군에서 시냅스 가소성과 관련된 CREB 전사인자의 인산화를 통한 활성화로 신경전달물질인 BDNF 발현이 증가하는 효과를 보였으며, AChE 활성을 저해함으로써 Aβ1-42 oligomer에 의한 신경전달물질 및 시냅스 가소성 감소를 억제하는 효과를 보였다. 그러므로 제주도 탈염 용암 해수를 이용한 새싹인삼은 질환 특이적인 성분의 증가를 높여줌으로써 산업적 이용가치가 높을 것으로 판단되며, 이를 활용한 다른 약용작물 및 특용작물의 생산을 통한 건강기능식품 원료개발에도 충분히 활용 가능할 것으로 사료된다. This study investigated the neuroprotective effects of extract of Panax ginseng sprouts cultivated by hydroponics using desalinated magma seawater from Jeju island (Korea) on neurotoxicity in vitro. The chemical compositions of the Panax ginseng sprouts extracts were analyzed using LC-Q-orbitrap, and ginsenosides (Re, Rg1, Rg2, Rb1, and Rd) were detected. Analysis method based on HPLC was developed for determination of ginsenosides Rb1, Rg1, and Re. For the in vitro analysis, the extract inhibited Aβ1-42 oligomer-induced neurotoxicity through inhibition of cleaved caspase-3, cleaved PPAR, and Bax expression, as well as activation of Bcl-2 expression. The extract inhibited H₂O₂-induced neuroinflammatory responses through inhibition of nuclear translocation of NF-κB p65 and COX-2 expression, in addition to phosphorylation of MAPK (ERK1/2, SAPK/JNK, and p-38 MAPK). The extract stimulated BDNF expression through CREB activation by phosphorylation at Ser<SUP>133</SUP> and inhibited up-regulation of AChE activity by Aβ1-42 oligomer-induced neurotoxicity. Taken together, these results provide in vitro evidence that the experimental extract exhibits neuroprotective properties against Aβ1-42-induced neurotoxicity and oxidative stress, which suggests that the extract has potential to be developed into a new functional food for neuroprotection and learning/memory improvement in the global food market. Furthermore, it is expected that desalinated magma seawater from Jeju island (Korea) may be applied for the cultivation of other functional plants in order to enhance functional components.

      • 의료분쟁에 관한 보건정책학적 고찰 : 응급의료종사자를 중심으로

        The medicolegal problem can be occurred in all medical field. Especially pre-hospital stage can be more exposed to the legal claims due to the very nature of EMT business and characteristics of ER patient or their family member. All Emergency Medical Technician should be concerned about the law associated with emergency care for handling the medicolegal problem, so the legally risky situations that may be occurred in pre-hospital stage and ER practice. This study reviewed malpractice claim of emergency patients filed in at Association of malpractice patients' family and two tertiary level hospitals. Problems related to treatment and misdiagnosis. Especially issues concerning emergency medical service system including of inadequate transport, delay in triage and transport accounted for many cases of all claims. This alerts us to the seriousness of medical accidents of emergency patient. This paper suggests several items that all E.M.T and every member of ED health care team always have to remember the medicolegally risk situations, must be trained in understanding the patients' wants and desires and should have the knowledge of the law associated with emergency health care. Develop the system that can share the informations about the medicolegal events which were experienced by each ED healthcare institutes.

      • Utstein Style에 의한 병원전 심정지환자의 심폐소생술 성적보고양식 개발

        To save out-of-hospital cardiac arrest cases is a major concern for any community. Resuscitation has become an important multidisciplinary branch of medicine, demanding a spectrum of skills and attracting a plethora of specialities and organizations. Since the "Utstein Style" was advocated in 1991, many reports about out-of-hospital cardiac arrest have been published based on this guideline. These differences prevent valid inter-hospital and international comparisons. However, we do not know the true effectiveness of resuscitation. There are no guideline for reviewing, reporting, conducting research on resuscitation in Korea. The "Utstein Style" is internationally recommended guidelines for reporting outcome data from resuscitation events. This study was designed to evaluate the overall survival rate and to understand the factors influencing hospital discharge when cardiopulmonary resuscitation(CPR) is attempted on out-of-hospital cardiac arrest victims, and especially to provide basic date for the unified reporting guideline of resuscitation in Korea.

      • 한국 재난관리체계의 문제점과 개선방향

        The scale of disasters is very large, also the influence of disasters is irreversible. Korean people has continuously learned how to deal with disaster management law. However, as time goes by after the many disaster, the concern with the disaster management by the people seems to become old and banal topic which nobody has interest in. If so, can it be said that our society is safe from the disaster? This study start from critical mind of asking this question. This study puts an emphasis on the problems such as following; To realize whole concept of disaster management provided in the basic civil defense law, the combined disaster management should be required to be established, the combined law should play a integral part for prevention for the disaster and should be a concrete basis of various laws for disaster management. Establishing a consolidated organization for disaster managing and united response organization is necessary. This consolidated organization is not temporary one such as civil disaster control headquarter but permanent one such as Disaster Management Office which controls an over all aspects of disaster management. Temporary organization should not be divided into natural calamity and man-made calamity but integrated one as a single control tower for all sorts of disasters. The police and army under fire fighting system should be reconsidered. It is necessary to make leadership system on the disaster place more substantial centered around present fire fighting system. To do so, more clear division of duty is required. The working condition should be improved so that all public servants in this agency can play their roles with pride and worth. The resource owned the existing administrative organization must be used by an appointment system. And to manage a large scale of disaster, it is necessary to have interorganizational network system which many specialized organization including volunteer group are intergrated. And the natural disaster happen, many professionals and volunteers are distributed to the spots of disaster in the right man in the right place. Finally, citizen's consciousness about safety supervision needs to be recover.
