N/A Patients suffering from idiopathic or self-induced edema are uniformly characterized by chronic use of furosemide, which leads to vicious cycle of edema. Among chronic furosemide users who don't have any other specific edema forming diseases, 9 patients from the outpatient clinic(OC) and 6 patients examined at the emergency room(ER) used it mainly for weight reduction and for cyclical edema, respectively. All of the ER group patients were presented with severe hypokalemia(2.04±0.2mEq/L; range 1.3 to 2.7 mEq/L) and alkalosis(748±0.01; range 7.44 to 7.51) but none from the OC group showed such results. Other baseline parameters including Plasma renin activity(PRA) and aldosterone level on recumbency, and FEn₂were similar in both groups. In contrast, daily working hours(6.1±0.5 vs 10±0.6hr, p<0.01), average body weight gain between AM and PM(0.4±0.1 vs 0.9±0.lkg, p<0.01), peak weight gain interval(9±0.8 vs 5±0.1day, p<0.05), PRA(7.6±1.5 vs 23.5±7.2ng/ml/h, p<0.05) and aldosterone level(22.1±4.2 vs 64.8±10.4 ng/dl, p<0.01) on ambulation, and FEk. on normokalemia(ll±2A vs 36±7.7%, p<0.01) were statistically different between the two groups. In comparison to the OC group, both the amout of urine(617±39 vs 358±26ml, p<0.01) and the percent change of PRA(-14±4 vs -3±2%, p<0.05) and al-dosterone level(-17±5 vs -4±3%, p<0.05) after saline loading(lL over 1hr, IV) following ambulation were smaller in the Elt group. Moat of the ER group patients(5/6) required aldosterone antagonist (spironolactone) added to K(+) supplement, but all of the OC group patients were managed to maintain an edema-free status with conservative treatment. In conclusion, patients with idiopathic edema seem to have more fluid transudation out of intravascular space during orthostasis with a prominent degree of deranged renin-aldosterone axis and K(+) metabolism than those with self-induced edema.
Determination of Parathion Metabolite, p-Nitrophenol in Urine of Parathion Factory Workers

Parathion is an organophosphate pesticide being legally applied for the purpose of agriculture and is being manufactured in Korea. A gas chromatography/mass spectrometric method was developed for the determination of parathion urinary metabolite, p-nitrophenol. p-Nitrophenol was extracted from weak acidic urine, and then measured by gas chromatography-mass spectrometry (selected ion monitoring). The recovery of pnitrophenol in the overall procedure was 88.2%. The detection limit of the assay was 1.0 $\mu$ g/L based upon assayed urine of 2.0 mL. The method was applied to the determination of p-nitrophenol in urine of workers of a parathion industry. Spot urines of workers of a parathion industry were sampled at the end of shift and pnitrophenol was analyzed using above developed method. p-Nitrophenol could be detected in all of the urine samples at concentrations varying from 3.0 to 681 $\mu$ g/L.
A Multicast polling Scheme for Idle Station in IEEE 802.11 Wireless Networks

IEEE 802.11 Point Coordination Function (PCF) mode is defined to support time bounded traffic, such as VoIP in wireless LANs. The poll scheduling plays an important role in IEEE 802.11 PCF mode operation. This paper proposed a Multicast Polling Scheme to increase the performance of wireless LANs. Moreover, we proposed a polling schedule scheme for our proposed multipoll to serve real-time traffic. The results show that the proposed mechanism is more efficient than the original IEEE 802.11 PCP.

As a preliminary investigation into the effect of environmental factors control for gonadal development, we examined the involvement of photoperiod and water temperature in ovarian development of Epinephelus. akaara. For the induction of sexual maturation, E. akaara reared in recirculating aquaculture system (RAS). During November 2013, the photoperiod and water temperature was adjusted to 12L:12D and 18℃, respectively. In the photo-thermal treatment group, every 3 weeks daylight was increased as follows a 13L:11D and 14L:10D, and control group was maintained under natural condition. After 9 weeks, water temperature was increased 23℃ in photo-thermal treatment group. The sampled fish every 3 weeks revealed increase in gonadosomatic index (GSI; 5.18±1.38), oocyte diameter and vitellogenic oocytes (423.9±36.1 ㎛) were observed in gonads 12 weeks under photo-thermal treatment group. However, ovarian development was maintained immature stage in control group. In this environmental factors manipulation trial, seventy one of the 95 females (578.4 ± 25.4 g in mean body weight, 31.0 ± 0.5 cm mean total length) treated with HCG injection (doses 500 IU/kg BW) were induced ovulation by artificial stripping. The total volume of ovulated eggs were 3,470 ml and the total volume of fertilized eggs was 3,295 ml. The fertilization rate and hatching rate were 95% and 98%, respectively. These results suggest that the photoperiod as well as water temperature are major environmental factors in triggering the gonadal development of E. akaara.

Background/Aims: Molecular diagnostic methods have enabled the rapid diagnosis of drug-resistant mutations in hepatitis B virus (HBV) and have reduced both unnecessary therapeutic interventions and medical costs. In this study we evaluated the analytical and clinical performances of the HepB Typer-Entecavir kit (GeneMatrix, Korea) in detecting entecavir-resistance-associated mutations. Methods: The HepB Typer-Entecavir kit was evaluated for its limit of detection, interference, cross-reactivity, and precision using HBV reference standards made by diluting high-titer viral stocks in HBV-negative human serum. The performance of the HepB Typer-Entecavir kit for detecting mutations related to entecavir resistance was compared with direct sequencing for 396 clinical samples from 108 patients. Results: Using the reference standards, the detection limit of the HepB Typer-Entecavir kit was found to be as low as 500 copies/mL. No cross-reactivity was observed, and elevated levels of various interfering substances did not adversely affect its analytical performance. The precision test conducted by repetitive analysis of 2,400 replicates with reference standards at various concentrations showed 99.9% agreement (2398/2400). The overall concordance rate between the HepB Typer-Entecavir kit and direct sequencing assays in 396 clinical samples was 99.5%. Conclusions: The HepB Typer-Entecavir kit showed high reliability and precision, and comparable sensitivity and specificity for detecting mutant virus populations in reference and clinical samples in comparison with direct sequencing. Therefore, this assay would be clinically useful in the diagnosis of entecavir-resistance-associated mutations in chronic hepatitis B.

We analyzed variations at thirteen Combined DNA Index System (CODIS) short tandem repeat (STR) loci (CSF1PO, FGA, THO1, TPOX, vWA, D3S1358, D5S818, D7S820, D8S1179, D13S317, D16S539, D18S51, and D21S11) in a sample from 130 unrelated individuals in the Korean population. Allele and genotype frequencies were determined using commercial PCR amplification kits. The Exact Test demonstrated that all loci were found to be no deviations from Hardy-Weinberg expectations (P>0.05). The distribution pattern of allele frequencies of the 13 STR loci reflected a common genetic affinity in contemporary populations from east Asians, but the Koreans are more closely related with the Chinese than to the Japanese and Vietnamese in east Asians surveyed so far. Based on the results of allele frequencies of the 13 CODIS STR loci, Koreans studied here were the most significantly different from those of African-American and Caucasian. For forensic testing, the power of discrimination (PD) index ranged from 0.760 at TPOX to 0.961 at FGA. The combined probability of match (PM) calculated from all 13 core CODIS STR loci was 1.94×10 exp (-14), which is highly informative. Therefore, the Korean 13 CODIS STR data could be useful for the regional specific and prerequisite references to the forensic community.

BACKGROUND: Probiotics might reduce gut-derived microbial lipopolysaccharide (LPS) by restoring bowel flora in patients with alcoholic hepatitis (AH). We evaluated the therapeutic effects of probiotics in patients with AH. PATIENTS AND METHODS: Between September 2010 and April 2012, 117 patients (probiotics 60 and placebo 57) were prospectively randomized to receive the 7 days of cultured Lactobacillus subtilis/Streptococcus faecium (1500 mg/day) or placebo. All patients were hospitalized and were not permitted to consume alcohol for the 7 days of the study. Liver function test, proinflammatory cytokines, LPS, and colony-forming units by stool culture were examined and compared after therapy. RESULTS: In both groups, the mean levels of aspartate aminotransferase/alanine aminotransferase, alkaline phosphatase, &ggr;-glutamyl transpeptidase, bilirubin, and prothrombin time were significantly improved after 7 days of abstinence. In the probiotics group (baseline and after), albumin (3.5±0.7 and 3.7±0.6 g/dl, P=0.038) and tumor necrosis factor-α (121±244 and 71±123 pg/ml, P=0.047) showed differences. In addition, the number of colony-forming units of Escherichia coli was significantly reduced (435±287 and 168±210, P=0.002). In the placebo group, the level of LPS (1.7±2.8 and 2.0±2.7 EU/ml) was significantly increased. In the intergroup comparison, significant differences in the levels of tumor necrosis factor-α (P=0.042) and LPS (P=0.028) were observed between the groups. CONCLUSION: Immediate abstinence is the most important treatment for patients with AH. In addition, 7 days of oral supplementation with cultured L. subtilis/S. faecium was associated with restoration of bowel flora and improvement of LPS in patients with AH.
Role of Nitric Oxide in Pepsinogen Secretion from Rat Gastric Chief Cells

Abstract-Nitric oxide (NO), a cellular messenger synthesized from L-arginine by NO synthase (NOS, EC.1.14.13.39), is considered to be a regulator of gastric secretion. In the present study, the role of NO in the regulation of exocrine secretion was investigated in rat gastric chief cells. Treatment of chief cells with carbachol resulted in an increase in the arginine conversion to citrulline, the amount of NO_X, the release of pepsinogen, and the level of cGMP. Especially, carbachol-stimulated increase of arginine to citrulline transformation, the amount of NO_X, cGMP level and the release of pepsinogen were partially reduced by the natural NOS inhibitor, N^G-monomethyl-L-arginine (MMA) and N^G,N^G-dimethyl-L-arginine (DMA). Furthermore, MMA-and DMA-induced decrease of pepsinogen secretion showed dose-dependent pattern. Activation of NOS is one of the early events in receptor-mediated cascade of reactions in gastric chief cells and NO, not completely, but partially mediates gastric secretion. Agonist-stimulated pepsinogen secretion in chief cells has been considered to be mediated in adenosine 3',5'-cyclic monophosphate pathway and/or guanosine 3',5'-cyclic monophosphate (cGMP) pathway. Taken together, the above results suggest that partial decrease of exocrine secretion following treatment of NOS inhibitor may result from the inactivation of NOS and subsequent guanylate cyclase, and NO/cGMP pathway may play a pivotal role in exocrine secretion.
