The rapid development of psychotropic drugs and great advancement in psychopharmacology during the past recent years has altered the strategy of pharmacotherapy for major psychiatric disorders, and in turn, an active development of various clinical practice guidelines or algorithm has taken place. However, there could be problems with applying the foreign guidelines directly to our clinical situation, due to the differences in racial characteristics, socioeconomic conditions, government policy, and clinical practices. In addition, the changes of circumstances outside of clinical situation in Korea may distort clinical practices and may go even against the trend of recent psychopharmacology. As a solution to such problems, Korean Medication Algorithm Project for Major Psychiatric Disorders (KMAP) was launched with the support from the Korean Society of Psychopharmacology and Korean Society of Schizophrenia.
However, since clinical practice guidelines and algorithms contain not only the advantages, but also disadvantages and risks, sufficient consideration must precede the development of clinical practice guidelines and algorithms for the correct and appropriate use. Hence, this review provides a discussion on the definition of clinical guideline and its limitation, its advantages and the disadvantages, and the need for the development of clinical guideline in psychiatric field. Also, developing source, dissemination and implementation of clinical practice guidelines were also discussed. We hope that this review can make the purpose of KMAP clear and elicit the productive criticism on the newly developed Korean algorithm.

국문 초록 (Abstract)

최근 많은 정신과 약물들의 개발과 정신약물학의 비약적인 발전은 주요 정신질환에 대한 약물치료 전략을 변화시켰고, 약물치료에 대한 각종 임상지침서나 알고리듬들이 활발히 개발되고 있다. 그러나 인종간의 특성, 사회경제적인 여건, 정책 및 의료행위 등의 차이로 인하여 외국의 지침서들을 국내에 바로 도입하기에는 무리가 있다. 또한 최근에 급격히 변화하는 국내의 진료외적 환경은 자칫 의료행위를 왜곡시키며 시대적인 흐름과도 역행될 가능성 마저 있다. 이를 타개하기 위한 방안 중의 하나로 주요 정신질환의 약물치료에 대한 국내용 알고리듬 개발사업이 ‘대한정신약물학회’와 ‘대한정신분열병학회’의 후원으로 시작되었다. 그러나 임상지침서나 알고리듬은 장점뿐만 아니라 많은 단점과 위험성도 동시에 지니고 있기 때문에 이를 올바르고 적절히 사용하기 위해서는 충분한 고찰이 선행되어야 한다. 이에 본 종설에서는 알고리듬을 포함하는 임상지침서의 정확한 정의와 그 한계점, 장단점 그리고 정신과 영역에서 임상지침서 개발의 필요성을 정리해 보았다. 동시에 개발의 주체와 방법, 보급 및 적용에 대해서도 알아보았다. 이를 통해 저자들은 본 개발 사업의 취지를 바로 알리고, 개발된 한국형 알고리듬에 대한 생산적인 비판의 의견을 듣고자 한다.
