Musculoskeletal infection is one of the most troublesome complications of orthopaedic surgery. Therefore, early detection of infection and immediate intensive care is necessary.
It has been generally accepted that for the early detection of postoperative infection it is important to observe clinical symptoms and patient signs as well as. to check leukocyte count, erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP)
We analyzed 20 patients with arthroplasty, 20 patients who had multiple fractures and 20 patients who had developed musculoskeletal infections and from Jan. 1993 to Dec. 1995. We evaluated WBC, ESR and CRP levels during preoperative and postoperative periods.
The result were as follows :
By the 2nd week, postoperative WBC and CRP counts in the arthroplasty and multiple fracture groups had normalized, but the ESR counts had not.
By the 2nd week postoperative for the musculoskeletal infection groups, serial and quantitative measurement of serum CRP can be very helpful in monitoring infection activity. Awareness of these natural CRP responses after orthopaedic surgery may help in the diagnosis of early posttraumatic and postoperative complications, especially infections.
