This study examined the relationship between adverse childhood experience(ACE) and posttraumatic stress disorder(PTSD) among adult victims of sexual assault who utilized victim support programs. Seventy-three adult victims of sexual assault which occurred within 3 months and 143 comparison women participated in the study. For last two decades, extensive research has been conducted on the relationship between ACE and physical as well as mental health. However, research using ACE is at the beginning stage in Korea, and little is known about ACE among the adult victims of sexual assault. This study examined ACE as a risk factor for PTSD, investigating both independent and combined effects on PTSD among the victims. The results showed that adult victims of sexual assault had experienced childhood adversity two times more than their comparison group. The rate of high risk profile (having over four ACEs) was even higher among the victims. ACE was a significant predictor of the level of PTSD symptoms even after taking recent experiences of sexual violence into account. Based on the result, implications for policy and clinical intervention for victims of sexual assault were discussed.

국문 초록 (Abstract)

본 연구는 피해자 지원기관을 내방한 73명의 성인 성폭력 피해자들과 성폭력 피해경험이 없는 대조군 143명을 대상으로 아동기 역경경험이 성폭력 피해자의 외상후스트레스 장애에 미치는 영향을 검증했다. 지난 20년간 아동기 역경경험(Adverse Childhood Experiences; ACE)과 신체 및 정신건강의 관계에 대한 광범위한 연구가 수행되어왔으나, 국내에서 아동기 역경경험을 이용한 연구는 이제 시작단계이고, 이 중 성인 성폭력 피해자를 대상으로 한 연구는 거의 없는 편이다. 본 연구는 성폭력 피해자 외상후스트레스 장애의 위험요인으로서 아동기 역경경험의 독립효과와 결합효과 등을 검증했다. 연구결과, 성폭력 피해자군은 대조군보다 2배 이상 많은 아동기 역경경험을 보고했고 아동기 역경경경험이 4개 이상인 고위험집단 비율도 월등히 높았다. 아동기 역경경험은 성폭력 피해를 고려했을 때도 피해자들 외상후스트레스 장애의 유의미한 예측요인이었으며, 아동기 역경경험의 하위유형에 따라 피해자의 외상후스트레스 장애에 미치는 효과는 달랐다. 연구결과를 바탕으로 아동기 역경경험과 성폭력 피해자 지원에 대한 이론적, 실천적 함의를 논하였다.
