조현병 환자를 위한 격려 중심 사회인지 재활프로그램 적용: 사례연구 = Application of Encouragement-oriented Social Cognition Rehabilitation Program for Schizophrenics: Case Study

This study is a preliminary study to verify the effect of additional application of encouraging factors emphasized by Adler school as a way to assist in social cognitive rehabilitation programs that have been used to improve social cognition abilities of schizophrenics. To verify this, 16 sessions of ‘encouragement-oriented social cognition rehabilitation program’ was conducted to 6 applicants who applied for participation in the local mental health and welfare center, and pre and post-examination were conducted to verify the effectiveness of the program using criterions related with encouragement and social cognition, and data collected through Consensual Qualitative Research (CQR) were examined. The study found that positive changes were made in their social interest area, which consisted of social interest behavior and social interest attitude, but an interpretation was need considering the limitations of case studies which are also preliminary studies. The analysis of the case interviews resulted in 2 areas, 16 sub-areas and 4 categories, and social knowledge typically improved in all cases participating in the program. Participants who experienced disappointment, frustration and defeat due to the deterioration and damage of their social cognitive abilities showed an improved interest in other people’s relationships and learning social knowledge as social beings after participating in the program. The possibility that repeated encouragement activities and participation in social cognition related programs affecting positive social behaviors and attitude changes in schizophrenics was confirmed through study participants. Based on these results, the limitations of this study and its implications for future research were discussed.

국문 초록 (Abstract)

본 연구는 조현병 환자의 사회인지 능력을 향상하는데 사용되어왔던 사회인지 재활프로그램에 아들러 학파에서 조력을 위한 방법으로 강조하는 격려 요인을 추가로 적용한 효과를 검증하기 위한 예비연구이다. 이를 위해 지역사회 정신건강복지센터에서 참가 신청한 6명을 대상으로 16회기로 구성된 ‘격려 중심 사회인지재활 프로그램’을 실시하였으며, 프로그램 효과를 검증하기 위해 격려 및 사회인지 관련 척도를 사용하여 사전․사후 검사를 진행했고, 합의적 질적 분석(CQR) 통해 수렴된 자료를 살펴보았다. 연구 결과, 사회적 관심 행동과 사회적 관심 태도로 구성된 사회적 관심 영역에서 긍정적인 변화가 나타났으나 사례연구이자 예비연구의 한계를 고려한 해석이 필요했다. 사례 면담 내용을 분석한 결과에서는 2개의 영역, 16개의 하위 영역, 4개의 범주로 도출되고, 프로그램에 참여한 모든 사례에서 전형적으로 사회적 지식(social knowledge)이 향상되었다. 사회인지 능력의 저하와 손상으로 인해 낙담과 좌절 및 패배감을 경험했던 참여자는 본 프로그램의 참여 후에 사회적 존재로서 타인과 인간관계에 대한 관심이 향상되고, 사회적 지식을 학습하는 모습을 나타냈다. 반복적인 격려 활동 및 사회인지와 관련된 프로그램 참여가 조현병 환자에게 긍정적인 사회적 행동 및 태도 변화에 영향을 줄 수 있다는 가능성을 연구 참여자들을 통해 확인할 수 있었다. 이러한 결과를 바탕으로 본 연구의 한계점과 향후 연구를 위한 함의를 논의하였다.
