구강운동훈련에 대한 인식조사: 언어재활사 중심으로 = Survey of the Use of Oral-Motor Exercise Among Korean Speech-Language Pathologists

국문 초록 (Abstract)

이 연구는 국내 언어재활사들을 대상으로 구강운동훈련(OME)의 임상적 적용 현황과 그 효과에 대한 인식을 조사하기 위해 수행되었다.
구글 모바일을 통해 진행된 20개 문항의 설문조사에는 90명의 언어재활사가 응답하였으며, 그 결과 97.8%가 OME의 임상적 적용 경험이 있음을 보고하였다. 특히, 응답자의 81%는 기능적 조음음운장애 대상으로 OME를 적용한다고 보고하였으며, 응답자의 58.9%가 말소리 개선 효과를 경험했다고 보고하였다. OME 적용과 관련된 활동 중에서는 '혀 강화'가 응답률 74.4%로 가장 높았으며, 기대 효과도 73.3%로 나타났다. 또한, OME의 핵심 개념으로는 '운동구어훈련(motor-speech exercises)'에 대한 인식이 46.7%로 가장 높았으며, 현장에서는 70%의 언어재활사가 구강운동과 말소리를 연계하여 실시하고 있음을 보고하였다. 가장 많이 적용하는 활동으로는 '입술 강화'가 65.6%로 가장 많았다. 그리고 체계적인 교육의 필요성을 느끼는 비율은 84.5%로 나타났다.
본 연구 결과 임상에서 OME를 빈번하게 실시하고 있음에도 불구하고, 적용 대상 범위가 제한되고 EBP 근거가 크게 요구됨을 확인하였다. 이를 기반으로 향후 연구에서는 다양한 현장 전문가와 언어재활 서비스 대상자들의 만족도와 효과 유무에 대한 연구가 필요할 것이다. 이러한 연구적 노력은 임상 현장에서의 의사결정 과정에 실질적인 도움을 주고, 언어재활 서비스의 질을 향상시키는 데 중요한 기여를 할 것으로 기대된다.

This study was conducted to investigate the clinical application and perception of effectiveness of Oral Motor Exercises (OME) among speech-language pathologists in South Korea. A survey consisting of 20 items was administered via Google Mobile to 90 speech-language pathologists, revealing that 97.8% had clinical experience with OME. Notably, 81% of the respondents reported using OME for clients with functional articulation disorders, with 58.9% observing improvements in speech sound production. The activity 'tongue strengthening' received the highest engagement at a 74.4% response rate, with a perceived effectiveness of 73.3%. Additionally, 'motor-speech exercises' were recognized as the core concept of OME by 46.7% of furthermore, 70% of practitioners reported integrating oral motor activities with speech sound work in their clinical practice. 'Lip strengthening' was the most frequently applied activity, which was noted by noted by 65.6% of the respondents.
Despite the widespread implementation of OME, limitations related to target applicability and the grounding in Evidence-Based Practice (EBP) were identified. Consequently, 84.5% of the respondents acknowledged the need for systematic education to enhance the effective ness of OME applications.
The results of this study confirmed that although OME is frequently performed in clinical practice, the scope of application is limited and evidence for Evidence-Based Practice (EBP) is greatly needed. Based on this, future research will need to investigate the satisfaction and effectiveness perceived by various field experts and speech rehabilitation service recipients. These research efforts are expected to provide practical help in the decision-making process in clinical settings and make an important contribution to improving the quality of speech rehabilitation services.
