장기요양보호노인의 사회복지서비스 이용에 관한 연구 : 부양자의 부양부담을 중심으로 = (A) Study on the Use of Social Welfare Service of Elderly under Long Term Care : Focus on the Burdens of Supporters

국문 초록 (Abstract)

본 연구는 우리 나라의 장기요양보호노인 부양자의 부양부담을 유형별로 구분하여 부양부담을 느끼는 이유는 무엇이며, 부양부담의 수준은 어느 정도인지를 파악하고, 이들이 원하는 사회복지적 욕구를 파악하고자 한다. 또한 장기요양보호가족을 위한 노인복지프로그램의 향상 및 개발에 필요한 기초자료를 제공하고자 한다.
본 연구문제를 구체적으로 제시하면 다음과 같다.
첫째, 장기요양보호노인의 특성과 주 부양자의 특성에 따른 장기요양보호서비스의 이용욕구와 이용행태에 미치는 영향을 파악한다.
둘째, 장기요양보호노인 부양자의 각 유형별 부양부담이 장기요양보호서비스의 이용욕구와 이용행태에 미치는 영향력의 정도를 파악한다.
셋째, 노인의 특성과 주 부양자의 특성, 유형별 부양부담이 장기요양보호서비스 이용욕구와 이용행태에 미치는 영향력의 정도를 파악한다.
본 조사연구의 분석결과는 다음과 같다.
첫째, 부양부담에 영향을 미치는 요인을 각 유형별 부양부담으로 나누어 살펴보았다. 신체적 부양부담에는 주 부양자의 연령, 직업, 그리고 노인과의 관계가 부양부담에 영향을 미치는 것으로 나타났다. 사회적 부양부담에서는 주 부양자의 연령, 학력, 그리고 피 부양노인의 수입, 경제적 능력, 심신기능손상이 심할수록 부양부담이 높은 것으로 나타났다. 정서적 부양부담에 영향을 미치는 요인은 주 부양자의 직업과 피 부양노인의 수입, 경제적 능력, 정신적 기능손상이 부양부담에 영향을 미치는 것으로 나타났다. 경제적 부양부담에서는 주 부양자의 학력, 직업, 월 평균 가족 총 수입, 그리고 피 부양노인의 수입, 경제적 능력이 영향을 미치는 것으로 나타났다.
둘째, 주 부양자의 부양부담이 장기요양보호서비스의 이용욕구 및 이용행태에 영향을 미치는 영향을 살펴 본 결과, 장기요양보호서비스의 이용욕구 및 이용행태는 각각 유형별 부양부담이 높을수록 이용욕구 및 이용행태가 높게 나타나고 있음을 알 수 있었다. 먼저 재가보호 서비스의 이용욕구에서는 경제적 부양부담이 높을수록 재가보호서비스의 이용욕구가 높아지는 것으로 나타났다. 지역 및 시설보호서비스의 이용욕구에서는 정서적 부양부담이 높을수록 서비스의 이용욕구가 높아지는 것으로 나타났다. 또한 재가보호 서비스의 이용행태는 사회적 부양부담이 높을수록 재가보호 서비스의 이용행태가 높아지는 것으로 나타났다. 지역 및 시설보호서비스의 이용행태는 경제적 부양부담이 높을수록 서비스의 이용행태가 높아지는 것으로 나타났다.
이상과 같은 연구 결과를 통해 다음과 같은 제언을 하고자 한다.
첫째, 장기요양보호서비스에 대한 욕구를 지닌 숨은 대상자들을 발견해야 한다.
둘째, 장기요양보호서비스에 대한 새로운 인식의 전환이 필요하다.
셋째, 노인을 부양하는 주 부양자(또는 가족)들이 선호하는 서비스의 유형은 지역사회보호 서비스의 필요성이 높게 나타났다.
넷째, 주 부양자 및 부양가족들의 부양부담을 완화시킬 수 있는 정책들이 강화되어야 하는 것으로서 노인 장기요양보호 서비스의 대상은 노인은 물론 그 가족까지 포함시켜야 할 것이다.

다국어 초록 (Multilingual Abstract)

An purpose of this research is to categorize supporter's burden of taking care of long-term care elderly to investigate about causes and level of caregiving burdens and their demands on social welfare. Another purpose is to provide fundamental data and information needed in ameliorating or developing social welfare programs for families of elderly under long term care.
Following are topics of the study.
First, an purpose is to understand about the effect of features of elderly under long term care and their supporters on the aspiration and behavior of using long term care service.
Second, an purpose is to investigate about effects of different types of caregiving burdens of those taking care of elderly under long term care on the aspiration and behavior of using long-term care.
Third, an purpose is to study about the effect of characteristics of the aged, supporters and types of caregiving burdens on the aspiration and behavior of using long term care service.
Following are the results of this investigation.
First, as a result of investigating about the factors that influence caregiving burdens according to the types of caregiving burdens, in terms of physical burden, it was proven that supporter's age, job and relationship with the aged had significant effect on caregiving burdens. In terms of social burden, supporter's age, educational background, income, economic level and physical weakness of the aged caused higher level of caregiving burdens. Sentimental burdens are caused by supporter's job, income, economic ability and mental weakness of the aged. In terms of economic burden, supporter's educational background, job, an average of family's monthly income, income and economic ability of the aged had significant effect.
Second, as a result of investigating about an effect of supporter's burdens on the aspiration and behavior of using long-term care service, it was indicated that higher level of burden caused higher level of aspiration and behavior of using the service. First, in terms of the aspiration to use home-stay care service, higher level of economic burden caused higher level of the aspiration on the use of home-stay care service. In terms of the aspiration to use regional and facility care services, higher level of sentimental burden caused higher level of the aspiration to use the service. Also, it was proven that higher level of social burden caused higher level of using home-stay care service. In terms of using regional or facility care service, higher level of economic burden caused higher level of using the services.
Following are the proposals made through the results of this research.
First, hidden subjects with aspirations on long term care services must be found.
Second, new perception on long term care services is necessary.
Third, high necessity level of community care services was shown about the types of services preferred by supporters (or families) taking care of the aged.
Fourth, policies that could improve caregiving burdens of supporters or families of the aged must be strengthened; subjects of long term senile care services must include the aged and their families.
