사람중심 치매커뮤니케이션 가이드라인 개발 연구 : 델파이조사를 중심으로 = The Study on Developing Guideline on Person Centered Dementia Communication: Delphi Survey

국문 초록 (Abstract)

치매인 돌봄에 있어서 커뮤니케이션의 어려움은 케어자 소진의 가장 큰 원인의 하나로 꼽히고 있다. 사람중심케어를 실천하는데 있어 치매인과의 커뮤니케이션의 중요성이 강조되지만, 아직까지 국내에서 이에 대한 가이드라인이 별도로 만들어지지 않은 상황이다. 이에 본 연구에서는 치매커뮤니케이션 가이드라인 개발을 위해 치매케어전문가들을 대상으로 델파이 조사를 실시하였다. 기존자료에 대한 문헌검토를 통해 초안을 작성한 뒤 이에 해당하는 현장사례를 수집, 내용분석을 실시해 새로운 문항을 더해 나가는 방법을취하였다. 이 과정에서 전문가그룹을 대상으로 현장 사례 수집 및 문항에 대한 의견을 반복해 듣는 델파이조사와 포커스그룹인터뷰를실시하였으며, 최종적으로 7개 원칙과 29개 문항의 가이드라인을 확정하였다. 7가지 원칙은 치매인에 대한 존중, 커뮤니케이션 준비, 커뮤니케이션을 즐거운 경험으로 만들 것, 치매의 특성에 맞는 대화, 이상심리행동증상에의 대응법, 비언어적인 방법 활용, 커뮤니케이션을 전체 케어와 연관할 것 등으로 이루어진다. 본 연구를 통해 작성된 가이드라인은 존중, 즐거움 강조 등 사람중심의 원칙에 입각해있다는 점에서 기존 가이드라인의 원칙 및 내용과 크게 다르지 않으나, 이상심리행동증상을 환자의 커뮤니케이션 시도로 해석하고이에 대한 적극적인 대응법을 제시한 점, 커뮤니케이션을 통해 파악한 치매인의 필요 및 욕구를 케어플랜 작성 및 케어매니지먼트에반영할 것을 포함한 점에서 차이점을 가진다. 본 연구를 통해 작성된 가이드라인이 향후 치매인 돌봄에서 적극적으로 활용되는 한편효과성 및 실천가능성 측면에서 검증, 수정⋅보완될 것을 제언한다.

다국어 초록 (Multilingual Abstract)

Communication breakdown is considered one of the biggest causes of dementia carers burnout. This study aims to write the communication guideline for dementia carers on the basis of Person Centered Care. The guideline development begins with literature review on existing guidelines and related materials. Delphi study was then conducted on experts of dementia care to expand and improve the draft guideline. With repeated questionings on experts of Delphi survey and focus group interviews to add, merge on and edit the existing items, 7 principles and 29 items were finally confirmed. The seven principles consist of respect for people with dementia, communication preparation, making communication a pleasant experience, appropriate communication skills, responding to BPSD, using non-verbal methods, and linking communication with overall care. The guideline drawn from this study are not markedly different from the existing guidelines in that all the guidelines are based on person-centered principles. The difference is found in that this guideline considers BPSD as the non-verbal communication, suggests intervention through communication on BPSD, and relates communication into the care plan and care management. Authors suggest this guideline be practiced in dementia care, while being continuously revised and supplemented in terms of effectiveness and practicability.
