Perilymph fistula(PF) is defined as the pathologic escape of perilymph from the oric capsule. PF occurs in a patients after various traumatic experiences - lifting, straining, coughing or other vigorous activities, barotrauma, stapedectomy and perilymphatic hypertension etc.
Patients with PF suffer from sudden or fluctuating sensorineural hearing loss, vertigo or dysequilibrium, and tinnitus. Its symptoms are those of a cochlear lesion and / or vestibular lesion, depending on the affected region.
No diagnostic procedure exists, except for surgical exploration, which either establishes or excludes the diagnosis of a perilymph fistula. Surgical exploration may not be totally reliable since fistulas can leak intermittently.
The goal of management is to restablish continuity of the fistulous membrane whether by natural healing or surgical repair.
The author reports 2 cases of PF following trauma with review of the literature.
