日本における認知症ケアの問題と「徘徊」の隱喩的分析 = The care for elderly people with dementia in Japan and the metaphorical analysis of the structure of “Wandering”

2000年代以降、認知症ケアは「認知症」と呼ばれる人々を「相互作用の主體」と捉える新しい理解に基づいて大きく變わった。そして、この間「認知症」の生活世界あるいは主觀的世界について明らかにする硏究や報告が相次いで發表されている。本硏究はこのような日本の認知症介護における「認知症高齡者の世界」という觀点から、どのように認知症高齡者の行動を理解するかという点について考察した。
第一章では、70年代から2000年代における醫療福祉政策の歷史を槪觀した。また、とくに2000年代以降の認知症高齡者像の變化について井口高志(2007)の「新しい認知症ケア」の議論について言及した。第二章では、日本の認知症ケアにおける「認知症高齡者の世界」に關する先驅的な硏究を二つ紹介した。一つは、精神科醫の室伏君士による「痴怠老人の精神世界―とくに健忘型痴怠について(1999)」という論文で議論されている「虛構的加工による精神世界を生きる態度」についてであり、もう一つは阿保順子による『認知症の人々が想像する世界』における徘徊を巡る記述である。第三章では、三好春樹(2009)の「解釋技術としての隱喩」という隱喩を用いたケア理論を參照し、その內實について分析した。またこのような隱喩について認知言語宅的な觀点からも考察を加えた。とくに、第二章で論じた阿保の事例「南川さん」について、隱喩論の前提とする能力である「比較」と「イメ一ジ寫像」から分析をおこなった。

다국어 초록 (Multilingual Abstract)

The purpose of this paper is to offer new perspectives on communication with person with dementia. In order to achieve this purpose, I attempt to apply the theory of metaphor in cognitive linguistics to the interpretation of “wandering”.
In Japan, some psychiatrists and other health-care workers have expressed and shared their views on the “living world” of dementia. They assert that the elderly people with dementia sometimes seems to behave and live in another world. At the same time, these experts at caring for dementia have insisted that it is important to understand and the “living world” which people with dementia live in. This paper is based on these preceding studies.
Chapter 1 begins with a brief overview of the Japanese social welfare program from 1970s to 2000s. In addition to this, I will reference a research on the change of image of the person with dementia. Chapter 2 is devoted to discussing two pioneering research projects in the field of medical care. A detailed account of their theories on the “living world” which people with dementia have will be presented. Chapter 3 deals with the structure of “wandering”. I will consider it from the point of view of metaphor.
