프로스타글란딘 점안 제제의 단안 시험을 통한 만족도 비교 연구 = A Monocular Trial Study between Different Prostaglandin Eyedrops of Different Formulas on Patient Satisfaction

국문 초록 (Abstract)

프로스타글란딘 점안 제제의 단안 시험을 통한 만족도 비교 연구목적: 세 가지의 서로 다른 프로스타글란딘 점안제의 안압 하강 효과와 부작용 및 주관적인 사용감에 대해 비교해 보고자 하였다.
대상과 방법: 정상 안압 녹내장, 원발 개방각 녹내장 혹은 고안압증을 처음 진단받은 환자를 대상으로 Xalost-s®, Taflotan-s® 또는 Lumigan® 중 두 가지의 약제를 무작위로 각 눈에 배정하여 안압 하강 효과, 부작용 및 주관적 점안감에 대해 전향적분석하였다. 치료 시작 후 1달, 4달 그리고 7달째 시력, 안압, 결막 충혈 및 각막 표면상태를 평가하였으며, 주관적 점안감 및부작용에 대해 평가하였다.
결과: Xalost-s® 사용군은 28안, Taflotan-s® 사용군은 29안, 그리고 Lumigan® 사용군은 29안이었다. 세 점안제 간 기저안압 및각 경과관찰 시점에서 세 약물 간 안압 하강 효과에 유의한 차이가 없었다(p = 0.72, 0.63, 0.47, 0.52). Xalost-s®, Taflotan-s®, Lumigan® 각각의 약제 만족도는 8.46 ± 1.99, 7.69 ± 2.30, 7.84 ± 1.72로 세 약제 모두 높게 평가되었으며, 부작용 및 불편감의정도는 2.71 ± 2.81, 2.76 ± 2.92, 2.86 ± 2.69점으로 낮게 평가되었다. Xalost-s®의 경우 약제 만족도가 가장 높고, Lumigan®은속눈썹 자람 및 안와 주변 색소침착관련 부작용이 가장 많았지만 통계적 유의성은 없었다.
결론: 세 가지 점안제 간 안압 하강 효과의 차이는 없었으며, 알려진 대표적인 프로스타글란딘 계열 약제 간 효과 및 불편감은전반적으로 비슷한 것으로 생각된다.

다국어 초록 (Multilingual Abstract)

Purpose: To compare efficacy, side effects and subjective experiences of three commercially available prostaglandin analogs.
Methods: This prospective study included treatment naive normal tension glaucoma, open angle glaucoma and ocular hypertension patients. Patients were randomly assigned to receive two of the following eyedrops: Xalost-s®, Taflotan-s®, or Lumigan®, and were instructed to instill one type of eyedrop in each eye. Evaluations were conducted at baseline, 1-month, 4-month, and 7-month intervals to assess intraocular pressure, treatment-related side effects and subjective experiences. Subjective experiences related to medication usage were evaluated with a maximum score of 10. Parameters were compared for each anti-glaucoma eyedrop and also for its combinations.
Results: A total of 86 eyes were analyzed. Clinical characteristics and intraocular pressure lowering efficacy were similar among three anti-glaucoma medication groups. Treatment satisfaction with Xalost-s®, Taflotan-s®, Lumigan® was high (8.46 ± 1.99, 7.69 ± 2.30, 7.84 ± 1.72, respectively), while discomfort ratings were low (2.71 ± 2.81, 2.76 ± 2.92, 2.86 ± 2.69, respectively). Although not statistically significant, Lumigan® was more associated with periocular side effects, and Xalost-s® demonstrated relatively higher drug satisfaction and comfort upon instillation.
Conclusions: No significant differences were found in treatment efficacy among three anti-glaucoma medication groups. Overall, side effects or discomfort of frequently used prostaglandin analogues appear to be comparable.
