만성피로증후군의 수면장애는 특징적인 증상인가 ? = Are Sleep Disturbance Symptoms of Chronis Fatigue Syndrome Distinct ?

Background Sleep complaints of chronic fatigue syndrome(CFS) are common. We have conducted this study to know whether thesleep disturbance symptoms of CFS are distinct from major depression(MD) or not.Methods The study subjects were 12 major depression(MD) and 10 CFS patients attending the community based primary caresettings from May 5 to May 17 in 2001. We analysed the findings of 10 sleep disturbances symptoms of CFS and MD.Results CFS patients reported significantly more 'consistently broken restless unrefreshing sleep' than MD(p=0.04). Althoughstatistically not significant, CFS reported more the symptoms such as 'strange sensation in legs relieved with movements','sleep-apnea related symptoms' 'resistable persistent daily drowsiness that can be followed napping' than MD.

국문 초록 (Abstract)

-연구배경 만성피로증후군은 다양한 증상을 특징으로 하는 질환이며 수면장애는 흔하다고 알려져 있다 . 아직 원인을 명확히 알 수 없는 만성피로증후군의 수면장애와 다른 증상들과의 관련성은 불확실하며 동반되는 다른 질환 , 특히 우울증에서 보이는 수면장애와 차이가 있는지 논란이 있다 . 이에 저자들은 만성피로증후군의 수면장애의 특성을 분석하고 주요 우울증의 수면장애와 다른 특성이 있는지 조사하기 위하여 본 연구를 시행하였다 . 방 법 2001년 5월 7일부터 5월 17일까지 전국의 8개 지역사회 일차의료기관을 방문한 18세 이상의 주요우울증 및 만성피로증후군으로 진단된 환자 22명이 본 연구의 대상이었다 . 10가지 수면장애증상에 대한 설문을 통하여 대상들의 수면장애의 특성을 분석하였으며 만성피로증후군과 주요 우울증의 수면장애증상을 비교분석하였다 .결 과 만성피로증후군에서 ‘수면 중 자주 깨고 잠에서 깬 후 개운한 느낌이 없는 증상 ’은 80% 의 빈도를 보였으며 , 30% 이상의 빈도를 보인 수면장애증상은 ‘낮 시간에 항상 졸리운 증상 ’, ‘수면 중 심하게 코를 고는 증상 ’, ‘수면 중 다리에 이상한 감각 및 움직이면 없어지는 증상 ’ 등이었다. 그리고 주요 우울증에 비하여 유의하게 높은 빈도를 보인 증상은 ‘수면 중 자주 깨고 잠에서 깬 후 개운한 느낌이 없는 증상 ’ 이었다(p=0.04). ‘수면 중 다리에 이상 한 감각 및 움직이면 없어지는 증상 ’은 상대적으로 높은 빈도를 보였지만 통계적으로 유의한 차이가 없었으며 (p=0.07), 수면무호흡증을 시사하는 2가지 증상과 ‘낮 시간에 졸리운 증상 및 깜박 졸음 ’ 증상도 주요 우울증에 비하여 만성피로증후군에서 높은 빈도를 보였지만 통계적으로 유의한 차이는 없었다.
