Pendulum 장치를 이용한 상악 대구치의 원심이동 증례 = MAXILLARY MOLAR DISTALIZATION WITH A PENDULUM APPLIANCE

국문 초록 (Abstract)

상악골과 치조골 전돌을 가진 II급 부정교합 증례 중 상악 치아 발치의 적응증이 아닌 경우와 하악 구치의 전방 이동을 허용할 수 없는 경우 상악대구치의 원심이동기법을 사용하게 된다.
Pendulum 장치는 환자의 협조도 없이 상악 대구치를 원심 이동시킬 수 있는 효과적인 장치로 소구치를 안정화 시키며，구 개측 고정원을 이용할 수 있다. 하지만 구치부 정출로 하안면 고경이 증가할 수 있고，전치부 전방 이동이 동반되며，추벽부 조직에 손상을 줄 수 있으므로 장치의 사용과 환자 선택에 선중을 기해야 한다.
이에 pendulum 장치를 사용하여 상악 대구치를 원심 이동시킴으로써 공간 문제를 해결하고， 대구치 관계를 개선한 증례 들을 보고하는 바이다.

다국어 초록 (Multilingual Abstract)

Maxillary molar distalization is a treatment approach for patients with Class II malocclusions who do not require extractions and mesial movements of mandibular molars.
The pendulum appliance is effective for distalization of the maxillary molars and independent of patient cooperation. This appliance can stabilize the maxillary premolars and use the palatal rugae area as an additional anchorage. However. caution is needed to control collateral effects, including increase of lower facial height, incisor protrusion and damage to the rugae area.
This article reports the cases in which maxillary molar distalization achieved by pendulum appliance resolves the space problems and corrects the molar relationships.
