한국노인정신질환의 현황 = The De Facto Mental Illness in Korea Elderly

국문 초록 (Abstract)

노년기 우울장애, 치매 등의 정신질환은 만성적이거나 반복적으로 재발되어 일상생활 능력에 상당한 장애를 유발하며 따라서 노인 보건 및 복지 정책의 체계적인 마련이 중요한 보건정책 과제로 대두되고 있다.
치매의 경우 전체 유병률은 6.26∼11.3%, 알쯔하이머형 치매 4.17∼5.3%, 혈관성 치매 1.05∼2.5%로 보고 되었다. 65세 이상 노인 중 4.2∼7.5%는 주요우울증을 가지고 있으며 2.0∼3.9%는 기분부전증을 가지고 있는 것으로 보고되었으며, 우울증상을 호소하는 경우는 연구 방법 및 대상에 따라서 7∼23.5%까지 다양하였다.
주정중독과 수면장애, 그리고 기타 정신건강 관련 질환들은 아직은 그리 많은 역학 연구가 이루어지지 않았지만 우울증 또는 치매와 마찬가지로 임상적으로 중요한 영역이다.
본고에서는 최근까지 이루어진 국내 노인 정신장애의 역학 자료들을 토대로 우울증과 치매를 비롯한 각종 장애의 유병률에 관하여 검토하고 이를 토대로 향후 한국에서의 노인정신의학의 교육·훈련 및 노인인구에 대한 정신건강 증진 대책을 수립하고 추진하는데 기초자료를 수집하고자 하였다.

Depression, dementia, and other psychiatric disorders of the aged persons are usually chronic and recurrent. These make the daily lives of the eldrly very troublesome. These mental disorders will demand more and more attention from the elderly health care systems.
Dementia is present in 6.26-11.3% of Korean elderly. Prevalence of Alzheimer's type is 4.17-5.3%, and that of the vascular dementia is 1.05-2.5%. Prevalence of major depressive disorder in the elderly aged 65 years and older is 4.2-7.5%, and that of dysthymic disorder is 2.0-3.9%. Depressive symptoms are reported up to 7-23.5% of older adults according to the survey methods and sample groups.
Alcoholism, sleep disorders and other mental health related conditions are also important concerns but have not been as well studied as depression and dementia.
In this review, we examined the prevalence and incidence of major mental disorders based on the national epidemiological data of Korean elderly. Therefore we hope that this perspective gives supports to the development of educational plans of geriatric psychiatry. And, hopefully, the present data help the government and NGOs establish and promote the policy for the health of the aged population.
