연구 배경 : 급격하게 증가하는 노인 인구에 따라 노인의 건강문제 중 저체중이 큰 비율을 차지하고 있으며 저체중 노인은 일반 노인에 비해 우울에 취약하다. 우울은 노인의 자신의 건강상태를 주관적으로 평가한 주관적건강상태와 긴밀한 관계를 가진다. 그러나 저체중노인에서 주관적건강상태가 우울에 미치는 영향에 대한 연구는 부족한 실정이다.
연구 목적 : 저체중노인의 주관적건강상태, 우울의 정도를 파악하고 요인간의 연관성을 파악하여 저체중노인의 우울의 위험 요인을 식별하고 향후 우울 발생 예방에 도움이 되는 정보를 제공하기 위한 근거자료가 되고자 한다.
연구 방법 : 본 연구는 2014년, 2016년, 2018년, 2020, 2022년 국민건강영양조사의 자료를 이용하여 분석을 하였으며, 위계적회귀분석을 실시하여 저체중노인의 주관적건강상태가 우울에 미치는 영향을 파악하였다.
연구 결과 : 본 연구의 대상자는 178명이었고 저체중노인에서 주관적건강상태, 성별, 연령, 주관적 체형인식, 스트레스가 우울에 영향을 미치는 요인으로 나타났다. 위계적 회귀분석 결과 독립변수인 주관적건강상태와 저체중노인의 우울의 증가간 연관성이 관찰되었고, 통계적으로 유의하였다. 주관적건강상태가 좋은 경우에 비해 나쁜 경우(B=0.934, p<0.001) 우울 정도가 유의하게 증가하였다.
논의 및 결론 : 저체중노인의 주관적건강상태가 우울에 영향을 미치는 것으로 나타났다. 따라서 저체중노인이 겪는 우울 증상을 감소시키기 위해서 주관적건강상태를 개선할 필요가 있으며, 이를 위한 중재 개발 및 반복 연구가 필요하다. 노인에 있어 우울의 예방 및 관리는 간호의 영역에서 그 중요도가 높고, 특히 보다 우울에 고위험인 저체중노인을 대상으로 우울의 위험요인을 분석하고 주관적건강상태의 영향을 파악하여 최종적으로 저체중노인의 삶의 질을 높이고 사망률을 줄일 수 있는 중재를 마련하는데 근거 자료를 제공할 수 있다.

Background : With the rapidly growing population of older adults, underweight problems account for a significant proportion of health problems in older people. Underweight older adults are more vulnerable to depression than the general older adults. Depression is closely related to self-rated health, which is an individual's self-assessment of their health. However, there is limited research that investigates the effect of self-rated health on depression in underweight older adults.
Objective : The aim of this study is to understand self-rated health and depression of underweight older adults, to identify the association between these factors, to provide baseline data to help identify the risk factors of depression in underweight older adults and to help prevent future onset of depression.
Method : This study was analyzed using data from the 2014, 2016, 2018, 2020 and 2022 National Health and Nutrition Examination Surveys. 178 underweight older adults were identified from the national surveys and were used as the working sample of this study. A hierarchical regression analysis was conducted to determine the effect of self-rated health on depression in the underweight older adults.
Results : Self-rated health, sex, age, perceived body shape and stress were found to be factors influencing depression in underweight older adults. The results of the hierarchical regression analysis demonstrated a statistically significant positive association between self-rated health and depression among underweight older adults. Specifically, compared to those with good self-rated health, those with poor self-rated health showed a significant increase in depression (B=0.934, p<0.001).
Conclusion : The study found that the self-rated health of underweight older adults had a significant effect on their level of depression. Therefore, for underweight older adults, improving self-rated health is pivotal to reducing depressive symptoms. The results of this study highlights the need for intervention development and further replication studies. Preventing and treating depression in older adults is particularly important in the nursing sector. Understanding risk factors for depression is especially vital for underweight older adults, as they are at higher risk of depression. This study suggests self-rated health as a central risk factor. The results of this study ultimately provide the basis for developing interventions to improve quality of life and reduce mortality in underweight older adults.
